Stroke has been emerged as one of major health problems for women worldwide. Increasing of knowledge in this field has provided new data in this area which contribute to finding of new risk factors and could improve stroke treatment. A large number studies indicated a variety of risk factors including sex, age, race, smoking, diabetes, and physical inactivity could be involved in stroke in women. The understanding of various aspects involved in women stroke including risk factors, outcomes, stroke recovery, and prevention could provide new therapeutic platforms which could likely lead to better treatment in women stroke. Diagnosis is one of important steps in stroke therapy. It has been showed that a variety of biomarkers such as microRNAs (miRNAs), tPA, and von willebrand factor could be used as therapeutic and diagnostic biomarkers in stroke therapy. Here, we summarized various aspects of stroke in women which could help to recognition risk factors, outcomes, care, treatment, and novel biomarkers involve in treatment of stroke. J. Cell. Stroke in Women: Risk Factors and Clinical Biomarkers.

Atrial fibrillation (AF) confers a raised risk of stroke and death, and this risk of adverse events is increased by the coexistence of other cardiovascular risk factors. The pathophysiology of AF is complex, involving the role of inflammation, structural remodelling with apoptosis, inflammation or fibrosis. These changes confer a prothrombotic or hypercoagulable state in this arrhythmia. Despite being easy to use for decision-making concerning oral anticoagulant therapy in AF, clinical risk scores used for stratification have shown modest capability in predicting thromboembolic events, and biomarkers may improve our identification of 'high risk' patients. Biomarkers, whether measured in the peripheral blood, urine or imaging-based may improve our knowledge of the pathophysiology of AF. Importantly these biomarkers could help in the assessment of AF prognosis. The aim of this review was to summarise the published data about biomarkers studied in AF, with focus on data from randomised prospective clinical trials and large community-based cohorts. We will also review the application of these biomarkers to prognosis on the main schemes used to help stratify risk in AF.

Biomarkers in atrial fibrillation: an overview

One promising field in neurovascular diseases investigation is the use of biomarkers to guide stroke etiology diagnosis and classification. Since treatment differs among etiologic subtypes and nowadays many patients receive a diagnosis of undetermined stroke, biomarkers might become an important additional diagnostic tool. In this review we update current knowledge about biomarkers related with cardioembolic stroke etiology (such as BNP and D-dimer proteins, or PITX2 and ZFHX3 genes), that in the future, might allow rapidly guiding other diagnostic tests and accelerating the onset of an optimal secondary prevention.

There are limited data on the influence of genetic polymorphisms in atrial fibrillation (AF) stroke risk. We hypothesized that a functional haemostatic polymorphism, that is, the factor VII −323 Del/Ins polymorphism, would influence the prothrombotic state associated with AF, as well as stroke risk. Other functional polymorphisms were also tested.Methods. We performed a cross-sectional study of 119 AF patients, who were compared to 96 patients with stroke secondary to AF. In the first patient group, we analysed plasma prothrombin fragment 1+2 levels (F1+2, an index of thrombin generation) to reflect the prothrombotic state of AF.Results. AF patients carrying the −323 Ins allele had lower plasma F1+2 levels (P=0.015). After multivariate analysis adjusted by age, sex and clinical risk factors, advanced age and 807C/T polymorphism of glycoprotein Ia (GPIa) gene were associated with higher risk of ischaemic stroke (OR: 1.06; P=0.003 and OR: 1.91; P=0.025), whilst FVII Ins −323 allele was associated with lower...
