Electrical left ventricular mapping in patients with and without CKD: Differences of voltage

Accentuated sympathetic nerve activity is a strong danger feature for cardiovascular major episodes, such as cerebral and myocardial ischemia/infarction.  Peripheric sympathetic nerve activity is emphasized in the hypertensive state, congestive cardiac failure, OSA, heaviness, diabetes, and chronic kidney disease (CKD) subjects. Leastwise a half of individuals with hypertension present augmented sympathetic nerve activity. Our group believes that RSD can reduce AF recurrence in patients with CKD and OSA by modulation of the sympathetic hyperactivity present in these diseases. The goal of this prospective study was to compare the impact of different treatments including CPAP, PVI, RSD, and some combination of them in controlled hypertensive patients with paroxysmal AF, moderate CKD, and severe OSA. Our data suggest that the patients underwent RSD associated to CPAP treatment or PVI, or both of them presented an improvement in the renal function, a decrease in AHI and a lower chance to present AF recurrence than the ones that did not suffer this intervention. Although encouraging, our data are preliminary and need long-term validation in a large population.

Different Strategies to Treat Paroxysmal AF in Patients with Moderate CKD and Severe OSA: a sub group Analysis

Background: The implantation of ICDs left pectorally is the conventional normal practice, but pathological reasons may obly to insert the devices on the right side. The aim of our evaluation was to define the outcome of right-sided implantation (n=52) on defibrillation effectiveness in comparison to the left-sided ICD implantation (n=210). Methods: A cohort of patients received standard therapy for primary or secondary prevention of sudden cardiac death (SCD) in patients with the structural cardiac disease, lay open to the ICD-DR implantation. The 262 individuals who had all the inclusion criteria were comprised in the assessment. Results: The defibrillation threshold testing (DFT), at the end of implantation showed that the mean energy to revert a programmed induced sustained ventricular tachycardia/ventricular fibrillation was 33.4±6.3 J for the patients that had the ICD implanted on the right side, and 23.9±5.3 J for the ones that presented the ICD positioned on the left side, P<0.0001. However the mean and the sum of shock events recorded by ICD during 1 year of monitoring, according to the side of ICD implantation, did not show any difference. Conclusion: Our results show that ICD implantation on the right side caused an elevated DFT in comparison to the left side insertion. This study also reported that there is no difference regarding safety and effectiveness about the amount of appropriate and inappropriate shock therapies, the mean and the sum of shock events recorded by ICD during 1 year of monitoring, according to the side of ICD implantation.

Effectiveness Evaluation of ICDs Implanted in the Right Side vs. Left Side

In the United States, statistics indicate that there are approximately one million annual deaths from cardiovascular disease, of which 330,000 are the result of sudden death [1,2]. It is well established in the literature the relationship between structural heart disease and the occurrence of SCD. In over 70% of cases, the underlying heart disease is the myocardial ischemia [3]. The importance of automatic implantable cardioverter-defibrillator (ICD) has been demonstrated in patients with previous myocardial infarction and severe systolic left ventricular dysfunction (secondary prevention) [4,5]. Survivors of cardiac arrest or those with sustained ventricular tachycardia at high risk of recurrence of such events [6]. The therapies used include antiarrhythmic drugs, surgical resection, endocardial catheter ablation and use of the implantable electronic cardiac device.

Acute effects of renal sympathetic denervation guided by renal nerve stimulation in CKD patients with ICD

Objective To evaluate the efficacy and safety of lower ablation indexes (AI) guided pulmonary vein isolation (PVI) in treating paroxysmal atrial fibrillation (AF). Methods Ninety patients with paroxysmal AF scheduled for radiofrequency ablation were randomly divided into three groups. The AI targets for PVI were as follows: In group A/B/C, 550/500/450 for roof and anterior wall, and 400/350/300 for posterior/inferior wall. The first-pass PVI rate, ablation time, complications and recurrence of atrial tachyarrhythmia (ATa) were compared. Results The mean age was 62.5 years (male: 63.0%), mean body mass index (BMI): 24.35 ± 3.66 kg/m2. The baseline characteristics were comparable. There was no significant difference in the first-pass PVI rate among the three groups (left-sided-PV: 66.7% vs. 80% vs. 73.3%, P = 0.51; right-sided-PV: 70% vs. 83.3% vs. 73.3%, P = 0.64), also with similar gap rate during the procedural waiting time. At 1-year follow-up there was no significant difference in the recurrence rate of ATa among the three groups (10% vs. 13.3% vs. 13.3%, P = 1.00). The ablation time in the Group C was significantly less than that in the other two groups (47.8 min. vs. 47.0 min. vs. 36.6 min, P < 0.001). Higher AI seemed to link a non-significant trend toward higher rate of pericardial effusion (group A + B vs. group C:6.7% vs. 0%, P = 0.30), although the rate of overall complications was not different among the three groups. Conclusion This randomized study shows that, a relatively lower target AI guided ablation may be similarly effective to achieve PVI with significantly reduced ablation time and obtain similar clinical outcome in treating paroxysmal AF in Asian population.
