Objective: The objective of this study was to determine the costs associated with influenza and the cost effectiveness (net costs per life-year gained) of influenza vaccination in The Netherlands. Design and setting: The economic evaluation comprised a cost-of-illness assessment and a cost-effectiveness analysis, both of which were conducted from the healthcare perspective in The Netherlands. The modelling framework for the economic evaluation linked epidemiological aspects of influenza (e.g. incidence, mortality, years of life lost) to vaccination coverage and healthcare resource use. Healthcare resource use was specified for hospitalisations, general practitioner visits and drugs. Intervention: The intervention assessed in the cost-effectiveness analysis was influenza vaccination. Main outcome measures and results: The costs of influenza were estimated to be 31 million euros (EUR) for the influenza season 1995/96 in The Netherlands (EUR1 ≈ $US1.1). For the extended programmein 1997/98, i.e. all elderly people, the cost-effectiveness ratio was estimated at EUR1820 per life-year gained. Subgroup analysis demonstrated that the programme had a more favourable cost effectiveness among the chronically ill elderly population (cost saving) than among the rest of the elderly population (EUR6900 per life-year gained). Conclusion: Influenza vaccination has a cost-effectiveness ratio that is better than or comparable to that of other implemented Dutch programmes in the prevention of infectious diseases.

A second Dutch national Survey of General Practice was carried out in 2001 with the aim of providing actual information about the role of general practice in the Dutch health-care system for researchers and policy makers. Data were collected on different levels (patients, general practitioners, practices) and included morbidity (self-report and presented to general practitioners), diagnostic and therapeutic interventions, doctor–patient communication, and background characteristics. Compared to 1987 (the first National Survey), Dutch general practitioners had organized their work more efficiently. Patients were less satisfied (78% satisfied) about the organizational aspects of general practice care than about the care actually provided (90% satisfied). Dutch general practitioners provide high-quality care: on average their performance was in 74% of cases in accordance with national guidelines. Communication in general practice had become less “social” and more medically oriented compared to 1987. General practice still acts in a gatekeeper role; this is illustrated by 96% of contacts handled solely by the general practitioner.

Sinds het verdrag van Maastricht (februari 1992) heeft de Europese Commissie nieuwe verantwoordelijkheden ten aanzien van Public Health. In een aantal beleidsrapporten van de EU is de noodzaak gesignaleerd om de coordinatie van beleidsontwikkeling en programmering verder te ontwikkelen en samenwerking tussen lidstaten te stimuleren. Consultatie en participatie mechanismen via diverse organisaties en netwerken worden daarbij als belangrijk gezien. De activiteiten van de EU zullen zich richten op harmonisering van de data verzameling, op de evaluatie en verspreiding van informatie over gezondheid en op het uitwisselen van kennis en ervaring tussen de lidstaten en, uiteindelijk, op de formulering van een gezamenlijke strategie en doelstellingen voor het gezondheidsbeleid. Om deze activiteiten te ondersteunen wordt een Europese Volksgezondheid Toekomst Verkenning (Public Health Status and Forecasts: PHSF) voorgesteld, waarin: - een brede, verklarende en vergelijkende analyse van de volksgezondheid in de EU ; - toekomstverkenningen voor belangrijke Public Health vraagstukken, gebaseerd op demografische extrapolaties, trend analyses and dynamische systeem analyse. Deze 'outline' bevat het ontwerp voor zo'n beleidsgericht, breed opgezet, PHSF-rapport, dat geproduceerd moet worden via een omvangrijk Europees samenwerkingsverband. De ruggegraat van het voorgestelde PHSF-rapport wordt een conceptueel model, dat relaties tussen de diverse elementen van Public Health beschrijft. Een systematische analyselijn wordt beschreven om tot een vergelijkende studie van belangrijke aspecten van de volksgezondheid en de achterliggende determinanten te komen voor de EU en haar lidstaten. Een dergelijke analyse zal de belangrijkste trends en ontwikkelingen en achterliggende oorzaken aangeven en daarnaast een indruk geven van in de toekomst mogelijk te behalen gezondheidswinst door specifieke beleidsmaatregelen. In deze benadering zullen geaggregeerde gezondheidsmaten, zoals gezonde levensverwachting, worden meegenomen, waarvoor andere gezondheidsgegevens (sterfte, ziekteprevalentie, kwaliteit van leven) nodig zijn en die waar mogelijk gekoppeld zullen worden aan gegevens over determinanten van gezondheid. De toegevoegde waarde van een geslaagd PHSF-rapport is het onstaan van een brede, uniforme en consistente gegevensset, die indicatoren bevat die relevant zijn voor de volksgezondheid in de Europese Unie. Dit kan de lidstaten ondersteunen bij het aangaan van samenwerkingsverbanden.Het rapport zal voortbouwen op de informatie die is gegeven in eerdere, meer beschrijvende, documenten en zo een aanzet geven tot een regelmatige rapportage over Public Health in de EU, een noodzaak die ook door de Commissie wordt onderschreven.In een haalbaarheidsstudie volgend op dit ontwerprapport moet de aanpak in meer detail worden uitgewerkt. Daarbij moeten instituten en netwerken worden ingeschakeld, die een belangrijke rol spelen bij het verzamelen, analyseren en vergelijken van internationale gezondheidsgegevens. De juiste experts moeten worden benaderd en databehoefte en bronnen moeten worden gespecificeerd. De haalbaarheidsstudie moet ook de verdere details uitwerken van de organisatie, kosten en planning van een PHSF-rapport voor de Europese Unie.
