Acute Brain Failure: Pathophysiology, Diagnosis, Management, and Sequelae of Delirium

Despite recommendations from professional societies and patient safety organizations, the majority of ICU patients worldwide are not routinely monitored for delirium, thus preventing timely prevention and management. The purpose of this systematic review is to summarize what types of implementation strategies have been tested to improve ICU clinicians’ ability to effectively assess, prevent and treat delirium and to evaluate the effect of these strategies on clinical outcomes. We searched PubMed, Embase, PsychINFO, Cochrane and CINAHL (January 2000 and April 2014) for studies on implementation strategies that included delirium-oriented interventions in adult ICU patients. Studies were suitable for inclusion if implementation strategies’ efficacy, in terms of a clinical outcome, or process outcome was described. We included 21 studies, all including process measures, while 9 reported both process measures and clinical outcomes. Some individual strategies such as “audit and feedback” and “tailored interventions” may be important to establish clinical outcome improvements, but otherwise robust data on effectiveness of specific implementation strategies were scarce. Successful implementation interventions were frequently reported to change process measures, such as improvements in adherence to delirium screening with up to 92%, but relating process measures to outcome changes was generally not possible. In meta-analyses, reduced mortality and ICU length of stay reduction were statistically more likely with implementation programs that employed more (six or more) rather than less implementation strategies and when a framework was used that either integrated current evidence on pain, agitation and delirium management (PAD) or when a strategy of early awakening, breathing, delirium screening and early exercise (ABCDE bundle) was employed. Using implementation strategies aimed at organizational change, next to behavioral change, was also associated with reduced mortality. Our findings may indicate that multi-component implementation programs with a higher number of strategies targeting ICU delirium assessment, prevention and treatment and integrated within PAD or ABCDE bundle have the potential to improve clinical outcomes. However, prospective confirmation of these findings is needed to inform the most effective implementation practice with regard to integrated delirium management and such research should clearly delineate effective practice change from improvements in clinical outcomes.

A systematic review of implementation strategies for assessment, prevention, and management of ICU delirium and their effect on clinical outcomes

Researchers have collected Twitter data to study a wide range of topics. This growing body of literature, however, has not yet been reviewed systematically to synthesize Twitter-related papers. The existing literature review papers have been limited by constraints of traditional methods to manually select and analyze samples of topically related papers. The goals of this retrospective study are to identify dominant topics of Twitter-based research, summarize the temporal trend of topics, and interpret the evolution of topics withing the last ten years. This study systematically mines a large number of Twitter-based studies to characterize the relevant literature by an efficient and effective approach. This study collected relevant papers from three databases and applied text mining and trend analysis to detect semantic patterns and explore the yearly development of research themes across a decade. We found 38 topics in more than 18,000 manuscripts published between 2006 and 2019. By quantifying temporal trends, this study found that while 23.7% of topics did not show a significant trend (P = 0.05), 21% of topics had increasing trends and 55.3% of topics had decreasing trends that these hot and cold topics represent three categories: application, methodology, and technology. The contributions of this paper can be utilized in the growing field of Twitter-based research and are beneficial to researchers, educators, and publishers.

Twitter and Research: A Systematic Literature Review Through Text Mining

Purpose To explore best practices for increasing cultural competency and reducing health disparities, the authors conducted a scoping review of the existing literature. Method The review was guided by 2 questions: (1) Are health care professionals and medical students learning about implicit bias, health disparities, advocacy, and the needs of diverse patient populations? (2) What educational strategies are being used to increase student and educator cultural competency? In August 2016 and July 2018, the authors searched 10 databases (including Ovid MEDLINE, Embase, and Scopus) and MedEdPORTAL, respectively, using keywords related to multiple health professions and cultural competency or diversity and inclusion education and training. Publications from 2005 to August 2016 were included. Results were screened using a 2-phase process (title and abstract review followed by full-text review) to determine if articles met the inclusion or exclusion criteria. Results The search identified 89 articles that specifically related to cultural competency or diversity and inclusion education and training within health care. Interventions ranged from single-day workshops to a 10-year curriculum. Eleven educational strategies used to teach cultural competency and about health disparities were identified. Many studies recommended using multiple educational strategies to develop knowledge, awareness, attitudes, and skills. Less than half of the studies reported favorable outcomes. Multiple studies highlighted the difficulty of implementing curricula without trained and knowledgeable faculty. Conclusions For the field to progress in supporting a culturally diverse patient population, comprehensive training of trainers, longitudinal evaluations of interventions, and the identification and establishment of best practices will be imperative.

Toward Cultural Competency in Health Care: A Scoping Review of the Diversity and Inclusion Education Literature

Background Adults (age > 50 years) admitted to the surgical intensive care unit (SICU) are at high risk for delirium. Little is known about the role traumatic injury plays in the development of delirium because these patients have often been excluded from studies. Identification of specific risk factors for delirium among older adults following injury would be useful to guide prevention strategies. We attempted to identify modifiable factors that would predict delirium in an older trauma population admitted to the SICU. Methods Data were collected prospectively from July 2012 to August 2013 at a Level I trauma center on consecutive trauma patients, older than 50 years, admitted to the SICU. Patients who died in the SICU were excluded. Delirium was assessed every 12 hours using the Confusion Assessment Method for the ICU scale. Demographic, injury, social, and clinical variables were reviewed. Bivariate analysis determined significant factors associated with delirium. A multivariate logistic regression model was used to predict delirium risk. After preliminary results, additional analysis compared patients with chest injury (defined as chest Abbreviated Injury Scale [AIS] score ≥ 3) with those without. Results A total of 115 patients met criteria, with a mean age of 67 years, Injury Severity Score (ISS) of 19, and Glasgow Coma Scale (GCS) score of 14. The incidence of delirium was 61%. Variables present on admission, which were positive predictors of delirium, were as follows: age, ISS greater than 17, GCS score less than 15, substance abuse, and traumatic brain injury (defined as head AIS score ≥ 3). Chest injury (defined as chest AIS score ≥ 3) was a negative predictor of delirium. Significant risk factors influenced by clinical treatment included doses of opioids and propofol, restraint use, and hours deeply sedated (Richmond Agitation Sedation Scale [RASS] score ≤ -3). Clinical treatments with negative predictability were ventilator-free days/30 (vent-free), benzodiazepine-free days/30 (benzo-free), and restraint-free days/30. In a regression model considering age, vent-free days, chest injury, traumatic brain injury, GCS score, benzo-free days, and hours sedated, only age (odds ratio [OR], 1.1; 95% confidence interval [CI], 1.01-1.1; p = 0.03) was a predictor of delirium, while vent-free days (OR, 0.79; 95% CI, 0.65-0.96; p = 0.02) and chest injury (OR, 0.3; 95% CI, 0.09-0.83; p = 0.02) were significant negative predictors of delirium. Patients with chest injury had lower delirium incidence (44%) versus those without (75%) (p = 0.002) despite similar GCS score, ISS, and clinical variables. Conclusion Delirium is common in older trauma patients admitted to the SICU, and for every year for those older than 50 years, the chance of delirium increases by 10%. While higher ISS increases delirium risk, we identified several modifiable treatment variables including days patients were deeply sedated, mechanically ventilated, and physically restrained. Interestingly, patients with chest injury experienced less delirium, despite similar injury severity and clinical variables, perhaps owing to frequent health care provider interactions.
