TL;DR: This case highlights the importance of ensuring that the correct EUS-FNA procedure is followed, and after confirmation of a needle fracture, endoscopic removal can be accomplished with caution using the proper endoscopic accessories, in order to avoid potential damage to both the patient and the echoendoscope.

TL;DR: A 40-year-old woman who underwent laparoscopic cholecystectomy with a hepaticojejunal Roux-en-Y anastomosis for a congenital Todani’s type IV common bile duct cyst is referred to us for further evaluation, and a novel cautery-tipped stent delivery system is decided that allows the single-step EUS-guided placement of a lumen-apposing fully covered metal stent.
