TL;DR: Weight loss during index admission and cardiac index/cardiac output increase were the only factors associated with LVEF improvement from baseline to month 3, and there was no difference between ischemic and nonischemic etiology.

Dissecting heart failure.
TL;DR: A 46-year-old man presented with gradually increasing dyspnea and orthopnea, his physical examination and laboratory findings were consistent with heart failure, and the only unusual feature was a diastolic murmur, which prompted echocardiographic evaluation.
