This State of the Science review provides an overview of the history and findings of cognitive processing therapy (CPT), one of the most recommended treatments for posttraumatic stress disorder, acute stress disorder, and comorbid conditions. After an introduction to CPT and the randomized controlled trials that have been conducted, the effects of CPT on comorbid conditions are reviewed, as well as new combination treatments. Cognitive mediators of change are described. Different formats for CPT that have been developed are described, as well as patient, therapy, and therapist factors in outcome, applicability across diverse populations, efforts to disseminate CPT, and ongoing studies into the future. 

State of the Science of Cognitive Processing Therapy

It is normal to feel anxious, terrified, and agitated after being involved in a potentially dangerous or traumatic incident. Most emotions disappear after a few weeks have passed. Some people continue to have these emotions for many months or even years following the traumatic event. This article looks at how post-traumatic stress disorder (PTSD) might manifest differently in men and women. It was discussed how PTSD develops, what causes it, how it affects women, and what treatments are available. PTSD is far more common in women than in males. PTSD can result in feelings of guilt and worry, as well as shame, hopelessness, depression, and frustration. When someone suffering from PTSD is treated with empathy, compassion, understanding, and acceptance, she feels safe and secure, which has been linked to the development and improvement of PTSD symptoms. With the aid of loved ones, it is possible to overcome feelings of helplessness, grief, and hopelessness. Patients suffering from PTSD may benefit from the encouragement and support of their friends and family. With the help of their loved ones, women may enhance their health and well-being.

Supporting Women to Heal from Trauma and Post-Traumatic Stress Disorder

"Earlier research has established that posttraumatic stress disorder (PTSD) and substance use disorder (SUD) frequently coexist. Aims: Cognitive Processing Therapy was compared to Sertraline and a placebo in an RCT for treating patients with comorbid SUD and PTSD. Methods: 150 patients with SUD and PTSD were interviewed by clinicians and asked to fill out the Clinician-Administered PTSD Scale (CAPS-5), Posttraumatic Stress Disorder Checklist (PCL-5), Beck Depression Inventory (BDI-II), Timeline Follow Back Interview (TLFB), and Brief Addiction Monitor (BAM). Patients were randomly assigned to the following conditions: CPT (n=50), Sertraline (n=50), or Placebo (n=50). Pretreatment, posttreatment, six and, twelve-month follow-up assessments were conducted. Results: When compared to the sertraline group, CPT resulted in much higher reductions in CAPS scores at posttreatment assessment (d=0.93, p < .000). When compared to the control group, CPT considerably reduced PTSD symptoms (the effect size, d=1.9, p < .000). Sertraline resulted in many significant decreases in CAPS when compared to control groups (the effect size , d=1.11, p<.000). At posttreatment, SUD and depression severity were significantly reduced in both CPT and Sertraline groups. After six and twelve months of follow-up, these differences persisted. Conclusion: Comparatively to the control group, CPT and Sertraline significantly decreased PTSD, SUD, and depression."

Cognitive Processing Therapy Versus Medication for the Treatment of Comorbid Substance Use Disorder and Post-Traumatic Stress Disorder in Egyptian patients (Randomized Clinical Trial)

The relevance of the topic is due to the growing use of CBT in psychotherapeutic practice and the need for a better understanding of its effectiveness, including the role of positive thinking. This work highlights the importance of positive thinking in promoting mental health and well-being. It argues that positive thinking is a practical tool for developing adaptive cognitive strategies, rather than an unattainable ideal. In CBT, positive thinking is not just about replacing negative thoughts with positive ones, but is a component of a comprehensive approach to re-evaluating an individual's experience. Such an approach contributes to the formation of a more realistic and adaptive perception of oneself and the world. The main methods and techniques of CBT, including cognitive restructuring and behavioral experiments, utilize positive thinking as a tool for correcting destructive thoughts and behaviors. This article discusses different perspectives on the effectiveness of positive thinking in CBT, weighing the arguments for and against its potential limitations. Positive thinking is an essential component of Cognitive Behavioral Therapy. It contributes to improving an individual's psychological well-being and developing psychological resilience, which enables more effective coping with life challenges and stresses. However, to achieve maximum effectiveness, the application of positive thinking must be closely integrated with a comprehensive treatment strategy that considers the individual needs and characteristics of each client. This approach requires a profound understanding of the mechanisms of cognitive distortions and their impact on a person's emotional state, as well as knowledge of how to most effectively apply positive thinking to correct these distortions. 

Harnessing Positive Thinking: A Cognitive Behavioural Approach to Stress Management

Psychological Interventions for Post-traumatic Stress Disorder in Adults

The purpose of this study is to examine the efficacy of cognitive processing therapy (CPT) in treating posttraumatic stress disorder (PTSD), depression and anxiety among Syrian refugees who have experienced war trauma. Clinicians interviewed 94 patients and asked them to complete the Clinician-Administered PTSD Scale three times: pretreatment, posttreatment and at the 12-month follow-up. In addition, PTSD, depression and anxiety were assessed using the PTSD Checklist, Beck Depression Inventory and Beck Anxiety Inventory at pretreatment, weekly, posttreatment and 12-month follow-up. At posttreatment evaluation, CPT led to decreases in PTSD, depression and anxiety (Ps < 0.000). These decreases continued after a year of follow-up (Ps < 0.000). Among Syrian refugees, the symptoms of PTSD, depression and anxiety all showed significant improvement after receiving CPT.
