A midline diastema usually is part of normal dental development during the mixed dentition. However, several factors can cause a diastema that may require intervention. An enlarged labial frenum has been blamed for most persistent diastemas, but its etiologic role now is understood to represent only a small proportion of cases. Other etiologies associated with diastemas include oral habits, muscular imbalances, physical impediments, abnormal maxillary arch structure, and various dental anomalies. Effective diastema treatment requires correct diagnosis of its etiology and intervention relevant to the specific etiology. Correct diagnosis includes medical and dental histories, radiographic and clinical examinations, and possibly tooth-size evaluations. Appropriate treatment modalities have been described. Timing often is important to achieve satisfactory results. Removal of the etiologic agent usually can be initiated upon diagnosis and after sufficient development of the central incisors. Tooth movement usually is deferred until eruption of the permanent canines, but can begin early in certain cases with very large diastemas.

Background: Malocclusion has large impact on individual and society in term of discomfort, Quality of Life [QoL] Aims and Objectives: To assess prevalence of malocclusion and orthodontic treatment needs among middle and high school children of Davangere city, India by using Dental Aesthetic Index [DAI]. Materials and Methods: A Descriptive cross sectional study was conducted among 1000, in the age group 12 to 15 year old school children studying in middle and high schools of Davangere city, India. 10 schools were selected by simple random sampling procedure and 100 study subjects were selected proportionately for males and females by using systematic random sampling procedures in each school. Data recorded using proforma consisted DAI components. The collected data was subjected to statistical analysis. Statistical Analysis: The Chi-square test [x 2 ] was used for comparison of severity of malocclusion. Analysis of Variance [ANOVA] test was used for comparison of mean DAI scores between the age groups and in DAI scores. 'Z' test was used for comparing the mean DAI scores between sex group. Results: Of the 1000 children examined, 518[ 51.8%] were males and 482[ 48.2%] were females. 80.1% school children had ≤ 25 DAI scores with no or minor malocclusion requiring no or little treatment, 15.7% had 26-30 DAI scores with definite malocclusion requiring elective treatment, 3.7% had 31-35 DAI scores with severe malocclusion requiring highly desirable treatment and 0.5% had ≥ 36 DAI scores with handicapping malocclusion requiring mandatory treatment. Conclusions: 80.1% school children had no or minor malocclusion which required no or slight treatment, 19.9% had definite/handicapping malocclusion requiring definite/mandatory orthodontic treatment.

The importance of the presence of a maxillary midline diastema resides in its position and the concern it causes to patients. This specific diastema has been attributed to genetic and environmental factors, even though it is often a normal feature of growth, especially in primary and mixed dentition. The need for treatment is mainly attributed to esthetic and psychological reasons, rather than functional ones. Although it is often the case, treatment plans should not be selected empirically but rather should be based on adequate scientific documentation. Possible therapeutic approaches include orthodontics, restora- tive dentistry, surgery and various combinations of the above. The ideal treatment should seek to manage not only the diastema in question but also the cause behind it. Irrespective of the treatment alternative selected, permanent retention of stable results should be considered as a treatment objective. The aim of this paper is to underscore the main etiological factors for the presence of a maxillary mid- line diastema and to illustrate the clinical and laboratory examinations required to recognize these factors. Furthermore, alternative treatment options are discussed depending on the etiology of the problem. A serial study of changes in spacing between teeth during the period of facial maturation, finding tendencies toward closure of buccal spaces and opening of anterior spaces. Spacing in females is equally distributed between upper and lower arches; in males it is more prevalent in the upper arch.

Characteristics and stability of spaced dentition.

This epidemiologic study deals with the incidence of spaced dentition in 1279 students between the ages of 12 and 18 years. The sample includes only those with a full complement of permanent teeth (third molars were not considered) and with no apparent etiological conditions that might lead to disruption of dental arch continuity. The sample was evenly divided by age and sex. Frequency of spaced dentition was high, occurring in 51.8% of males and in 45.5% of females. Prevalence was less in higher age groups, and the sexual dimorphism that was significant in the 14 YR-16 YR group was not found in the 16 YR-18 YR group. The mean number of spaces per subject was 6 +/- 4.3 for males and 5 +/- 3.7 for females, again displaying diminishing incidence with age and a concomitant absence of the sexual dimorphism in the highest age group. Spaces were found in both arches in 49.5% of the spaced dentitions, and in only the maxillary arch in 34.3%. Incidence of spacing in the mandibular arch alone was only 16.2%. In most subjects the spaces were distributed equally between the two quadrants of each arch. The most common sites and largest space widths in either dental arch were found between cuspids and the first bicuspids and between cuspids and lateral incisors.
