Chorion villus sampling (CVS) is a widely used technique in first trimester prenatal diagnosis. In the early 1990s, two clusters of babies with limb defects following CVS were reported, raising the possibility of a causal association between early CVS and transverse limb deficiency. The evidence for this association is reviewed and shows a diminishing risk for transverse limb deficiency with advancing gestation. The risk of limb deficiency extends through the period of limb morphogenesis and slightly beyond; falling from levels 10-20-fold above background at nine weeks and below, to levels approaching (or only a few-fold above) background at 11 weeks and beyond.

Use of medical expulsive therapy (MET) is common practice in urology for the treatment of symptomatic urolithiasis, despite this its efficacy is debated. Its use in pregnancy is controversial. Our objective was to evaluate the safety and efficacy of Tamsulosin as a MET in pregnant women. We retrospectively identified pregnant patients who presented with renal colic at the CHU de Quebec from 2000 to 2015. We compared patients who received Tamsulosin as MET to a control group without MET. We evaluated efficacy as passage rate of lithiasis and necessity of intervention. We evaluated safety of the treatment according to fetal outcomes (birth weight, APGAR, gestational age). We evaluated 207 pregnant patients presenting renal colic, 69 patients in the MET group were compared to 138 patients in the control group. Of these, 48 (70%) in the Tamsulosin therapy group and 76 (56%) in the control group had proven urolithiasis. No significant difference was found for mean gestational age at birth, birth weight and APGAR. No sudden infant death syndrome was encountered in Tamsulosin group. There was no significant difference for length of hospital stay and need for surgical intervention. The spontaneous passage rate was 58% (25/48) in the MET group compared to 43% (29/76), but this difference was not statistically significant (p = 0.18). Short-term utilisation of Tamsulosin as MET in second and third trimester of pregnancy is not associated with adverse maternal or infant outcomes. Moreover, there was no significant adjunct for the rate of stone passage.

Safety and efficacy of Tamsulosin as medical expulsive therapy in pregnancy

OFCs (orofacial clefts) are among the most frequent congenital defects, but their etiology has yet to be clarified. OFCs affect different structures and functions with social, psychological and economic implications in children and their families. Identifying modifiable risk factors is mandatory to prevent the occurrence of non-syndromic OFCs (NSOFCs). PubMed, Cochrane Library, Scopus and Web of Science were searched from 1 January 2012 to 25 May 2022 and a total of 7668 publications were identified. Studies focusing on the risk factors of NSOFCs were selected, leading to 62 case-control and randomized clinical trials. Risk factors were categorized into non-modifiable and modifiable. The first group includes genetic polymorphisms, gender of the newborn, ethnicity, and familiarity. Within the second group, risk factors that can only be modified before conception (consanguinity, parental age at conception, socio-economical and educational level, area of residency and climate), and risk factors modifiable before and after conception (weight, nutritional state, acute and chronic diseases, psychophysical stress, licit and illicit drugs, alcohol, smoke, pollutants and contaminants) have been distinguished. This study provides a wide overview of the risk factors of NSOFCs, focusing on modifiable ones, to suggest new perspectives in education, prevention, medical interventions and clinical research.

Modifiable Risk Factors of Non-Syndromic Orofacial Clefts: A Systematic Review

Background: A retrospective review of cleft lip and palate sur-geries at our hospital during 2001Y2005 showed an increase inpatients treated per year. The aim of the study was to review theactivities of the clinic, hence to determine the variation in numberof cleft lip and palate cases and surgeries from 2006 to 2009, andalso to identify the commonest type of cleft cases during the sameperiod.Methods: Data for this retrospective study were obtained fromthe outpatient department records of the cleft clinic and operatingtheater. Information collected included demographic features ofthe patients, types of cleft lip/palate, number of attendance, andsurgeries performed. The data were then analyzed using SPSSversion 12.0.Results: A total of 528 patients were operated on for cleft lip/andpalate (ie, 132 surgeries per year). Most patients (54.2%, n = 286)were boys. The age of the patients ranged from 1 to 25 months, witha mean age of 3.7 months. There was also an increase in cleftlip surgeries from 2006 to 2009, whereas cleft palate surgeries de-creased toward the year 2009. In terms of types, cleft lip (73.1%,n = 386) outnumbered cleft palate. Also, unilateral clefts (70.5%,n = 372), in terms of position, were the majority.Conclusions: The number of cleft surgeries was found to increaseperyear,andunilateralcleftlipwasidentiﬁedasthecommonesttypeof cleft cases. Background: To determine the prevalence and characteristics of birth defects in perinatal infants in Hubei Province during 200l-2008. Methods: The prevalence of birth defects in perinatal infants delivered after 28 weeks or more was analyzed in Hubei surveillance hospitals during 200l-2008. Results: The incidence of birth defects in perinatal infants from 200l to 2008 was 120.0 per 10,000 births, and was increased by about 41% from 81. 1 in 2001 to 138.5 per 10,000 births in 2008. The incidence in the first 4 years (20052008) was much higher than the latter four (2001-2004) (χ 2 =77.64 , P 0.05), but that between male and female was significant (χ 2 =5.24 , P 0.05). The two leading birth defects were cleft lip and/or palate and polydactyly, followed by congenital heart disease, hydrocephaly, external ear malformation and neural tube defects. The prevalence of congenital heart disease was rising. Conclusions: Eight years’ birth defects data indicate that the birth defect rate was on the rise and the birth defects prevalence in Hubei province should be valued.

Transverse limb defects are reported in a fetus and an infant born to mothers on treatment for hypertension. One pregnancy resulted in an intrauterine death at 20 weeks, and in addition to the limb defects, there was bilateral cleft lip and palate and renal hypoxic damage. It is proposed that the drugs caused maternal hypotension which led to reduced uteroplacental blood flow, fetal hypotension and hypoxia and that the anomalies seen in the two babies are a consequence of these events.
