The search for highly sensitive and highly specific biomarkers of MS, including neuroimaging biomarkers, continues. One of such biomarkers is the central vein sign detectable on SW and T2-weighted MR images. The sensitivity and specificity of methods used for central vein sign detection vary. This article describes two clinical cases of patients with similar neurological symptoms which required making differential diagnosis between multiple sclerosis and secondary demyelination in the presence of a systemic disorder (systemic lupus erythematosus). In addition to routine MR sequences, we used SWI generated by a 3T scanner. The lesions with the central vein sign were counted; the proportion of perivenular lesions was determined. In the multiple sclerosis case, all the lesions were perivenular; the proportion of lesions with the central vein sign in the patient with secondary demyelination in the presence of systemic lupus erythematosus was 16.7%. The use of SW images improved the informative value of the analysis.

Central vein sign for differential diagnosis of demyelinating diseases of cns

Objective : to determine the specificity and sensitivity of the central vein sign (CVS) for the diagnosis of multiple sclerosis (MS). Material and methods . We analyzed 76 patients. The main group consisted of 40 patients with a confirmed diagnosis of MS according to the McDonald criteria, 2017. The comparison group included 20 patients with migraine and 16 patients with cerebral microangiopathy. All patients underwent a standardized MRI scan of the brain, including 3D FLAIR and SWI modes. The total number of T2/FLAIR hyperintense lesions in the brain and the number of lesions with CVS were calculated. Results . In the group of MS patients, the total and relative number of lesions with CVS was higher than in the comparison group (p&lt;0.001). The 40% lesion rule had a sensitivity of 97.5% and a specificity of 94.4%. The sensitivity of the 3-lesion rule was 92.5% and the specificity 66.7%. The use of the 5-lesion rule had a sensitivity of 75% and a specificity of 83.7%. Conclusion . The data obtained indicate a significant diagnostic role for SCV in MS. The use of the 5-lesion rule provides high sensitivity and specificity with relative ease of use. 

The central vein sign in the diagnosis of multiple sclerosis

Objective : to study the pyramidal tract in patients with highly active multiple sclerosis (HAMS) during treatment switching from first-line MS therapy to second-line therapy due to suboptimal response. Material and methods . 24 patients with HAMS were analyzed. Depending on the severity of pyramidal functional system (PFS) impairment according to EDSS, patients were divided into 2 groups; group 1 — 17 patients with an EDSS score of 0—2.5 points, group 2 — 7 patients with an EDSS score of ≥3 points. All patients underwent MRI of the brain according to the standard protocol. The diffusion tensor images (DTI) were processed using the DTI FiberTrak software. Results . The volume of the pyramidal tract was decreased in patients in group 2 (p&lt;0.001), asymmetry of the indicator was noted, in several patients a decrease in volume on the clinically intact side, which may indicate visually undetectable signs of damage of pyramidal tract. There was a clear tendency for a decrease in fractional anisotropy and a decrease in pyramidal tract length with increasing pyramidal deficit (p&lt;0.001). Negative correlations were found between indicators of neurological deficit and pyramidal tract volume and length (Spearman's Rho=-0.5246; p&lt;0.001) and a direct correlation between duration of MS and apparent diffusion coefficient (ADC) and inversely — with fractional anisotropy (Pearson's R=-0.290; p=0.039). Conclusion . The observed decrease in pyramidal tract volume and length, increase in ADC, asymmetry of these indicators, correlations with the degree of pyramidal insufficiency, EDSS and duration of MS can obviously serve as additional criteria for evaluation of disease dynamics and efficacy of therapy.

Diffusion tensor tomography with tractography in assessment of the pyramidal system in patients with highly active multiple sclerosis

The current diagnosis of multiple sclerosis (MS) is based on confirmation of the disseminated pathological process in space and time in accordance with the McDonald criteria. Despite this, the search continues for specific markers of the disease, including those detected using neuroimaging techniques that have high sensitivity and specificity in the diagnosis of MS. The paper considers the central vein sign, a new promising diagnostic one of MS. This sign refers to a parenchymal vein visualized in the focus of demyelination, by using special magnetic resonance imaging (MRI) modes. Studies show that the central vein sign has high sensitivity and specificity in the diagnosis of MS, allowing it to be differentiated from other demyelinating, vascular, and systemic diseases that have an MRI pattern similar to that of MS. According to various authors, a threshold of 40–50% perivenular lesions allows MS and MS-like diseases to be differentiated with high accuracy.
