Abstract Previous studies have demonstrated the impact of potassium humate (KH) and chitosan (CH) on ameliorating drought effects, but their combined applications in promoting these benefits are still unfound. Therefore, the current study aims to evaluate the efficacy of KH and CH on corn growth, yield, nutrient contents, and water productivity under full and limited irrigation conditions. Under the drip irrigation system, a split-plot experiment was performed with three replications in the second week of February in the seasons of 2021 and 2022. The main plot was equipped with a valve and a flow emitter to control the amount of the targeted irrigation levels (full irrigation and limited irrigation from the development stage onwards), as well as four foliar applications in the subplot (0, CH 500 mg l −1 , KH 3000 mg l −1 , and CH 500 mg l −1 + KH 3000 mg l −1 ). It was found that separate foliar applications of KH or combined foliar applications of KH + CH had a significant impact on the most examined traits. However, compared to the control, adopting limited irrigation and applying combined applications thereof have significantly increased iron, zinc, manganese, oil, protein, yield, and water productivity. In addition, this combination decreased proline, and the maximum reduction was observed for the combined application with adopting full irrigation. In arid regions, the researcher recommends treating stressed plants with combined foliar applications of KH + CH, which could help plants overcome the negative effects of drought and attain the highest yield and water productivity.

Simple Summary The peritoneum, a common metastatic site of colorectal cancer (CRC), has a high incidence and poor prognosis that makes it difficult to diagnose early. Peritoneal metastasis (PM) depends on the synergistic action of multiple molecules and the regulation of various components of the tumor microenvironment. A multidisciplinary combination approach is still recommended for treating the disease currently. Cytoreductive surgery (CRS) combined with intraperitoneal chemotherapy (IPC) may benefit patients with CRC-PM, but further clinical trials and higher-level evidence-based medical evidence are needed. Abstract Peritoneal metastasis (PM) is a common mode of distant metastasis in colorectal cancer (CRC) and has a poorer prognosis compared to other metastatic sites. The formation of PM foci depends on the synergistic effect of multiple molecules and the modulation of various components of the tumor microenvironment. The current treatment of CRC-PM is based on systemic chemotherapy. However, recent developments in local therapeutic modalities, such as cytoreductive surgery (CRS) and intraperitoneal chemotherapy (IPC), have improved the survival of these patients. This article reviews the research progress on the mechanism, characteristics, diagnosis, and treatment strategies of CRC-PM, and discusses the current challenges, so as to deepen the understanding of CRC-PM among clinicians.

Peritoneal Metastasis: A Dilemma and Challenge in the Treatment of Metastatic Colorectal Cancer

Simple Summary Disease outcome of patients with pseudomyxoma peritonei (PMP) and appendiceal neoplasms improved gradually in the past decades, as their pathology is better understood, together with a refined subtypisation. The introduction of cytoreductive surgery and hyperthermic intraperitoneal chemotherapy in the treatment of well-selected patients has led to significantly better outcomes compared to “classical” therapies alone. Due to the fact that PMP is considered a rare malignancy, real-word cross-sectional studies are highly warranted to establish reliable prognostic and predictive factors in large and uniform patient populations. Apart from established prognostic indicators, such as the peritoneal cancer index, the extent of cytoreduction and the plasma level of tumor markers are as well of significance. We aimed to refine and confirm the validity of these factors in our whole patient population and in patients with low-grade histologies. Our study can refine and improve our current understanding of PMP management on a sizeable cohort. Abstract Background: Pseudomyxoma peritonei (PMP) is a rare peritoneal condition where mucus-secreting tumorous cells progressively produce a thick, gelatin-like substance. The prognosis of patients with PMP is determined by the degree of cellularity within the mucin (low-grade (LAMN) vs. high-grade (HAMN) histologic features) and by the extent of the disease. Methods: Prognostic relevance of tumor markers CA19-9 and CEA, gender, Peritoneal Cancer Index (PCI), and completeness of cytoreduction (CC) after cytoreductive surgery were evaluated on 193 consecutive PMP patients, based on a retrospective analysis of prospectively gathered data from a German tertial referral center. Results: We demonstrated that low PCI, CC0 status, low-grade histology, and female gender were independent positive prognostic factors for both overall survival (OS) and progression-free survival (PFS). Furthermore, LAMN patients with achieved CC0 status show significantly better OS and PFS compared to those with CC1 status (p = 0.0353 and p = 0.0026 respectively). In contrast, the duration and drug of hyperthermic intraperitoneal chemotherapy (HIPEC) were not prognostic in any comparison. Increased CA19-9 and CEA levels were significantly associated with HAMN cases, but also predicted recurrence in patients with low-grade histologies. Conclusion: Our study confirmed the prognostic role of tumor markers and emphasized the importance of CC status and PCI in a large cohort of PMP- and LAMN patients.

Prognostic Factors in Pseudomyxoma Peritonei with Emphasis on the Predictive Role of Peritoneal Cancer Index and Tumor Markers

Ventral incisional hernias are common indications for elective repair and frequently complicated by recurrence. Surgical meshes, which may be synthetic, bio-synthetic, or biological, decrease recurrence and, resultingly, their use has become standard. While most patients are greatly benefited, mesh represents a permanently implanted foreign body. Mesh may be implanted within the intra-peritoneal, preperitoneal, retrorectus, inlay, or onlay anatomic positions. Meshes may be associated with complications that may be early or late and range from minor to severe. Long-term complications with intra-peritoneal synthetic mesh (IPSM) in apposition to the viscera are particularly at risk for adhesions and potential enteric fistula formation. The overall rate of such complications is difficult to appreciate due to poor long-term follow-up data, although it behooves surgeons to understand these risks as they are the ones who implant these devices. All surgeons need to be aware that meshes are commercial devices that are delivered into their operating room without scientific evidence of efficacy or even safety due to the unique regulatory practices that distinguish medical devices from medications. Thus, surgeons must continue to advocate for more stringent oversight and improved scientific evaluation to serve our patients properly and protect the patient–surgeon relationship as the only rationale long-term strategy to avoid ongoing complications.

Are Surgeons Going to Be Left Holding the Bag? Incisional Hernia Repair and Intra-Peritoneal Non-Absorbable Mesh Implant Complications

Abstract Identifying predictors for surgical site infections (SSIs) after emergency surgical treatment for traumatic pulmonary haemorrhage (TPH) is crucial for improving patient outcomes. This study aims to ascertain these predictors. In this comprehensive retrospective study, conducted from January 2020 to December 2023 at our institution, 75 patients were analysed, including a case group of 25 patients with SSIs and a control group of 50 without SSIs post‐TPH surgery. Inclusion criteria focused on patients aged 18 and above undergoing thoracotomy or minimally invasive thoracic surgery for TPH. Exclusion criteria included compromised immune systems, chronic pulmonary diseases, prior thoracic surgery or active infections at admission. We assessed several predictors: anaemia; operation time over 2 h; hospital stay over 5 days; intraoperative blood loss exceeding 500 mL; body mass index (BMI) ≥25 kg/m2; age ≥ 50 years; use of surgical drains; the presence of open wounds; diabetes mellitus and non‐prophylactic antibiotic use. Statistical analysis involved univariate and multivariate logistic regression, using SPSS Version 27.0. Univariate analysis revealed significant associations between SSIs and surgical drain placement, diabetes mellitus, open wounds and non‐prophylactic antibiotic use (p < 0.01). Multivariate analysis confirmed these factors as significant predictors of SSIs, with notable odds ratios. Other variables like anaemia, extended hospital stay, excessive intraoperative blood loss, older age and higher BMI did not significantly predict SSIs. Significant predictors for SSIs following TPH surgery include surgical drain placement, diabetes mellitus, open wounds and non‐prophylactic antibiotic use. Identifying and managing these risks is crucial in clinical practice to reduce SSIs incidence and improve patient outcomes.

T HE present investigation was carried out during 2018 and 2019 seasons to investigate the consequences of K - Humate with and without magnetic water irrigation on yield, fruit quality and some mineral composition of Hayany date palm cultivar cultivated in South Sinai Research Station (Ras Sudr), Desert Research Center, South Sinai Governorate, Egypt. Results showed that there was significantly effect by using K – Humate as a soil application and magnetic water irrigation in both seasons. Treatment K – Humate at 300 g/tree with magnetic water irrigation produced higher yield and bunch weight. The same treatment gave the highest fruit weight, flesh weight, fruit size and fruit dimensions in both seasons. Also, data showed that the total soluble solids, reducing sugars and nonreducing sugars content in fruits increased significantly by application of K – Humate and magnetic water irrigation. However results indicated that such treatment gave the highest values of N, P, K, Ca and Mg contents in pinnae of Hayany date palm cultivar in the first and second seasons compared with the control and other treatments.
