Very High Risk of Cancer in Familial Peutz—Jeghers Syndrome

Invasive aspergillosis of the central nervous system is a rare but well-described disease. There have been only a few reported survivors, and mortality exceeds 95% in the immunosuppressed host. We present a 2-year-old boy with acute lymphatic leukemia and multiple Aspergillus brain abscesses who was successfully treated with liposomal amphotericin B, itraconazole, and surgical excision of the abscesses. Liposomal amphotericin B is a new preparation that safely allows the attainment of significantly higher tissue levels with less toxicity than standard amphotericin B. The treatment of patients with invasive central nervous system aspergillosis is reviewed.

BACKGROUND

Undifferentiated embryonal sarcoma of the liver (UESL), a rare tumor that predominantly affects children, generally has been considered an aggressive neoplasm with an unfavorable prognosis. More recent reports have indicated that modern multimodal treatment and supportive care improve the survival of children with UESL. Data regarding the treatment and survival of adults have not been reviewed comprehensively, and only a few adult patients with UESL have been reported in the literature.



METHODS

The authors analyzed demographics, treatment, and actuarial survival of all reported cases of UESL in patients aged ≥15 years (n = 67 patients). In addition, 1 case is presented of a patient with UESL who was treated successfully at the authors' institution.



RESULTS

The median survival of all patients with UESL who were analyzed was 29 months. Patients who underwent complete tumor resection followed by adjuvant chemotherapy survived over a median follow-up of 28.5 months and had significantly better survival compared with patients who underwent surgical treatment alone. Patients who underwent an incomplete tumor resection had a tendency toward poorer outcomes.



CONCLUSIONS

To the authors' knowledge, this is the first report to demonstrate a significant effect on survival for adjuvant chemotherapy after complete surgical resection of UESL in adults. The role of neoadjuvant chemotherapy was not evaluated in this study. In the case study presented herein, combined therapy with surgery and chemotherapy led to a complete, sustained remission that has lasted for >6 years to date. Undifferentiated embryonal sarcoma of the liver in adults.

Intracranial aspergillus granulomas over a 12-year period were studied retrospectively. Three distinctive types of presentations were seen, namely nine patients had the rhinocerebral form with primary involvement of the air sinuses and secondary involvement of the skull base, cranial nerves and brain, three patients with the apparently purely intracranial form presented like any other space occupying lesion and one patient who had a 'stroke-like' presentation had a parasellar lesion. None of these patients were immunocompromised. The clinical features, and the medical and surgical options are discussed together with outcome.

The authors report a case of Scrotal subcutaneous dirofilariasis in man for the first time in Tunisia. The diagnosis was established by pathological examination of a biopsy but the identification of the species was not possible. Surgical exeresis of the node was followed by a complete recovery.

Resume Objectif Etudier les etiologies de l’asthme professionnel et determiner le devenir professionnel des asthmatiques. Methodes Il s’agit d’une etude descriptive retrospective ayant porte sur l’ensemble des cas d’asthme declares dans le secteur prive dans la region du centre tunisien et reconnus au titre de maladie professionnelle durant une periode de neuf ans (2000–2008). Resultats Nous avons recense 219 cas d’asthme professionnel, ce qui a represente 16,8 % de l’ensemble des maladies professionnelles reconnues durant la periode d’etude. L’asthme professionnel a interesse les adultes jeunes (40 ± 8,2 ans) avec une predominance du genre feminin (67,7 %). Le secteur textile etait preponderant (74,9 %). La majorite des salaries etait exposee aux allergenes de haut poids moleculaire (82,3 %) et les poussieres de coton etaient le principal agent incrimine (75,3 %). La perte d’emploi a ete observee chez pres de la moitie des salaries (46,6 %) et a ete associee a un âge inferieur ou egal a 35 ans ( p  = 0,01) et a une anciennete professionnelle inferieure a 15 ans ( p  = 0,03). Conclusion La proportion elevee de l’asthme professionnel dans le secteur textile dans la region du centre justifie le renforcement des mesures preventives dans ce secteur d’activite.
