OBJECTIVE. The objective of this study was to determine if restrictive risk-based mammographic screening could miss breast cancers that population-based screening could detect. MATERIALS AND METHODS. Through a retrospective search of records at a single institution, we identified 552 screen-detected breast cancers in 533 patients. All in situ and invasive breast cancers detected at screening between January 1, 2011, and December 31, 2014, were included. Medical records were reviewed for history, pathology, cancer size, nodal status, breast density, and mammographic findings. Mammograms were interpreted by one of 14 breast imaging radiologists with 3–30 years of experience, all of whom were certified according to the Mammography Quality Standards Act. Harms of Restrictive Risk-Based Mammographic Breast Cancer Screening

L’attitude diagnostique devant une lesion mammaire infraclinique impose encore trop souvent une tumorectomie. Objectif : valider la classification de l’American College of Radiology (ACR) qui utilise une description standardisee permettant d’homogeneiser la conduite a tenir face a une anomalie mammographique afin de reduire le nombre de tumorectomies diagnostiques inutiles. Materiel et methode : etude prospective sur 4 ans conduite dans le service de gynecologie-obstetrique du Centre hospitalier de Libreville (Gabon). Nous avons compare le resultat histologique de 150 lesions mammaires infracliniques, prelevees par tumorectomie, a leur classification ACR dans le but d’evaluer la correlation radio-histologique et de proposer une conduite standard a tenir. Resultats : nous avons releve 90 cas (60 %) de lesions ACR2 (benignite d’emblee), 28 cas (18 %) d’ACR3 (benignite probable, a surveiller), 6 cas (4 %) d’ACR4 (suspect et a biopsier) et 26 cas (17 %) d’ACR5 (malignite d’emblee). L’histologie a ete la suivante : 3 % de lesions malignes parmi les ACR2, 11 % de malignite parmi les ACR3, 67 % parmi les ACR4 et 92 % de malignite dans les ACR5. Conclusion : la classification ACR montre une excellente fiabilite pour les opacites benignes (ACR2) et malignes (ACR5). Elle offre la possibilite d’optimiser leur prise en charge therapeutique. Les lesions ACR3 et ACR4 imposent une attention toute particuliere. L’implication de tous les radiologues africains est a renforcer.

[Nonpalpable breast lesions: correlation of the BI-RADS classification and histologic findings].

BACKGROUND

A risk-based model was developed for recommendations for mammography screening of women in their 40s. We determined retrospectively the proportion of women already diagnosed in their 40s with breast cancer (BC) who would have been recommended for mammography screening by this model.



METHODS

Information was obtained from 404 women who were diagnosed with BC in their 40s from 1990 to 1998 at a large midwestern hospital. Data on BC detection method, mammography history, and BC risk factors existing before diagnosis were obtained from 353 of these women by mailed questionnaires. Data for an additional 51 women who had died were obtained by chart review. Two algorithms, the Exact Age Procedure (EAP) and the Grouped Age Procedure (GAP), were used to calculate the proportion of women who would have been recommended for mammography screening based on age, race, and BC risk factors.



RESULTS

Of women diagnosed with BC in their 40s, 74.5% would have been recommended for screening by the EAP and 70.5% by the GAP. The model would have recommended screening for approximately one half of the women who had no conventional risk factors (54% by EAP, 48% by GAP).



CONCLUSION

Although use of the risk-based model could result in more recommendations for screening than individual recommendations based on patient-physician discussions about the woman's risk factors, one in four women diagnosed with breast cancer in their 40s would not have been recommended for mammography screening. Evaluating a risk-based model for mammographic screening of women in their forties.

But : Etudier le profil clinique et epidemiologique de la pathologie mammaire chez les femmes qui consultent pour depistage du cancer du sein Materiels et Methodes : Il s’agissait d’une etude transversale avec collecte prospective des donnees, realisee a l’Hopital General de Douala du 30 Avril 2007 au 30 Avril 2010. Seules les femmes d’au moins 45 ans etaient recrutees. L’analyse des donnees portait essentiellement sur : l’âge, les antecedents personnels et familiaux, les donnees  cliniques. Resultats : Cent-trois femmes sur 153 ont ete recrutees ; quatre vingt dix femmes (87,37%) etaient âgees entre 45 et 60 ans. Les antecedents de cancer du sein dans la famille etaient retrouves chez 24 femmes (23 ,3%). La clinique ne revelait aucun signe de cancer ou de pathologie benigne; la mastodynie bilaterale etait le symptome retrouve dans 22 cas (21,4%), a chaque fois dans un contexte de syndrome pre-menstruel.    Conclusion : En raison du risque potentiel eleve de cancer du sein dans notre population d’etude,  nous recommandons un suivi clinique et  radiologique regulier des patientes presentant des facteurs de risque eleves.  SUMMARY:  Objective: To study the clinical and epidemiological profiles of breast disease in women consulting for breast cancer screening at the Douala General Hospital. Materials and methods : This was a transversal study from the 30 April 2007 till the 30 April 2010 at the Douala General Hospital. Women of at least 45 years old were recruited. The following data were analyzed: age, personal and family past histories, clinical and mammographic findings. Results: One hundred and three out of 153 women were recruited; ninety women (87.37%) were within 45 – 60 years old. Past histories of breast cancer in the family were found in 23.3% of cases (24 women). There were no suspicious clinical findings of breast cancer. Premenstrual breast pain was the main symptom in 22 cases ( 21.4%) Conclusion: Within the limit of this study, the potential risk of breast cancer is high. We therefore recommend regular clinical and radiological follow-up for patients presenting with high risk factors of breast cancer in our milieu

Profil epidemiologique et clinique de la pathologie mammaire a l’hopital general de douala (cameroun)

Background: Mammography is a particular mammary imaging technique, an important tool for screening, diagnosis and monitoring of breast disease in general and breast cancer in particular, for which incidence and mortality are well established. Several studies have shown a reduction in breast cancer mortality reaching 40% in women screened by mammography. Thus, the prognosis after treatment is better because of early detection. In the DRC, in developing countries, the mortality rate is high because the diagnosis is late and therefore responsible for a bad prognosis. However, no studies on early detection by mammography have been performed to our knowledge. Our goal was to determine the mammography contribution in suspicious lesions detection of breast cancer in Kinshasa. Method: A retrospective and descriptive study of 1738 mammograms at the Gombe Imaging Center from January 2012 to December 2015. The reports were prepared by radiologists. We re-read approximately 230 images under supervision to complete the missing data related to interpretation. Age, clinical information, breast density (BIRADS type), elemental or associated lesions, and ACR radiological diagnosis were the key parameters collected on an epidemiological data questionnaire and analyzed on the SPSS 20 software.Results: 347 screening mammograms or 20% were performed with an average age of 48.6 years ; 25 of these (7.2%) had mammographic lesions suspected of malignancy, among which 23 were considered in situ. In addition, our study showed that high breast density is a radiologically visible risk factor. Conclusion: This study has shown that mammography is a tool to integrate into cancer screening in our country. Because it allowed to detect suspicious lesions in asymptomatic women.
