Aim: Settling (embedment relaxation), which is the main cause for screw loosening, is developed by microroughness between implant and abutment metal surface. The objective of this study was to evaluate and compare the relationship between the level of applied torque and the settling of abutments into implants in external and internal implant–abutment connection.

Material and methods: Five different implant–abutment connections were used (Ext, External butt joint+two-piece abutment; Int-H2, Internal hexagon+two-piece abutment; Int-H1, Internal hexagon+one-piece abutment; Int-O2, Internal octagon+two-piece abutment; Int-O1, Internal octagon+one-piece abutment). All abutments of each group were assembled and tightened with corresponding implants by a digital torque gauge. The total lengths of implant–abutment samples were measured at each torque (5, 10, 30 N cm and repeated 30 N cm with 10-min interval) by an electronic digital micrometer. The settling values were calculated by changes between the total lengths of implant–abutment samples.

Results: All groups developed settling with repeated tightening. The Int-H2 group showed markedly higher settling for all instances of tightening torque and the Ext group was the lowest. Statistically significant differences were found in settling values between the groups and statistically significant increases were observed within each group at different tightening torques (P<0.05). After the second tightening of 30 N cm, repeated tightening showed almost constant settling values.

Conclusions: Results from the present study suggested that to minimize the settling effect, abutment screws should be retightened at least twice at 30 N cm torque at a 10-min interval in all laboratory and clinical procedures.

Variation in the total lengths of abutment/implant assemblies generated with a function of applied tightening torque in external and internal implant–abutment connection

Purpose This clinical report describes an immediate tooth extraction, followed by placement and provisional restoration of a dental implant in the prepared socket of a right maxillary central incisor. Materials and methods The tooth was extracted with minimal hard and soft tissue trauma and without flap reflection. A flapless, transmucosal surgical approach was used to prepare the socket and insert a tapered implant. The implant was immediately restored with a provisional abutment and crown without occlusal contacts. An impression was made 22 days after implant insertion, and a definitive, all-ceramic restoration was placed 3 days later. Results During the period of provisional progressive loading, no significant soft tissue contraction was observed related to noninvasive operating techniques and the immediate insertion of the provisional restoration. The patient exhibited no clinical or radiologic complications through 8 months of clinical monitoring after loading. Conclusion The Tapered Screw-Vent implant and all-ceramic restoration provided the patient with immediate esthetics, function, and comfort without any complications during the postloading follow-up period.

Immediate tooth extraction, placement of a Tapered Screw-Vent implant, and provisionalization in the esthetic zone: a case report.

Cemented crowns are increasingly being used on dental implants instead of on screw-retained prostheses because of the reliability of internal Morse taper implant-abutment connections. However, there is a lack of information on the fit of metal ceramic and premachined alumina infrastructures. Therefore, the aim of this study was to evaluate the marginal and internal fits of different metal and alumina infrastructures cemented on universal post abutments. A total of 45 abutments (6 mm in height and 3.3 mm in diameter) were divided into five groups on the basis of their infrastructure material: cobalt-chromium (CoCr), nickel-chromium (NiCr), nickel-chromium-molybdenum-titanium (NiCrMoTi), gold (Au), and premachined alumina. The alumina group showed marginal overextension, and the Au group showed the highest discrepancy in marginal fit among the metal alloys. The CoCr and alumina groups showed the lowest discrepancies in internal fit. In conclusion, the alumina cylinders exhibited the best internal fit, despite their horizontal overextension. Among the metal alloys, CoCr exhibited the best fit at critical regions, such as the cervical and occlusal areas.

Discrepancies in marginal and internal fits for different metal and alumina infrastructures cemented on implant abutments

OBJECTIVES Caries, periodontal disease, and peri-implant inflammation caused by deficient marginal adaptation of fixed prostheses are reasons for clinical failure of combined tooth-implant-supported fixed dental prostheses (TISFDP). This in vitro study examined the marginal accuracy in TISFDP after simulated stress in an artificial oral environment. MATERIALS AND METHODS Twelve three-unit TISFDPs were fabricated using a high noble alloy on models containing a human premolar with an artificial periodontium and an implant. Four three-unit tooth-supported prostheses (TSFDP) represented the control group. The experimental TISFDPs (four per group) were luted with three different cements: group 1, zinc phosphate; group 2, glass ionomer; group 3, self-adhesive resin. The specimens were mechanically loaded (1.2 million cycles/50 N) and thermally cycled (8000 cycles with 5/55 degrees C). The vertical marginal gap was measured before and after cementation, after chewing simulation and after thermal cycling by light microscopy (x 560). The results were subjected to statistical analysis (t-test/one-way analysis of variance/Bonferroni). RESULTS Significant increase (P< or =0.05) in the marginal gap was found after cementation within the experimental TISFDPs (implants, 11.7-18.7 microm; teeth, 13.4-24.2 microm) and the control TSFDPs (28.5 microm). Comparison of groups 1 and 2 revealed significant differences for the teeth while comparison of the implants showed significant differences among all groups. Chewing simulation and thermal cycling caused statistically insignificant changes in the marginal gaps of the experimental as well as the control TSFDPs. CONCLUSION The cementation of the TISFDPs with different luting materials caused a specific enlargement of the marginal gap in teeth and implants. Subsequent to simulated oral stress in an artificial oral environment, no significant changes of the marginal accuracy could be found.

Marginal accuracy of combined tooth–implant‐supported fixed dental prostheses after in vitro stress simulation

The module was developed as an elective to give motivated senior dental students an opportunity to expand their horizons in planning oral rehabilitation. It comprised one tutor and 12 students, from five universities world-wide, communicating on the World Wide Web (WWW), to develop oral rehabilitation plans for simulated patients. Trigger material came from one of two Case Profiles and consisted of diagnostic casts and details of the clinical and radiographic examination in WWW/CD-ROM form. No background material was supplied as to the “patient’s” age, sex, history or main concern(s). Students worked in groups of three, each student from a different location. Individual students were given a role within the group: ”Patient”, who developed a “personal background” belonging to the trigger examination material, “Academic” who identified state-of-the-art treatment options available for the dental treatment needs identified by the group and “General Practitioner” who tailored these options to the “patient’s” needs and wants. Student feedback focused on their perception of their experience with the program in response to a questionnaire comprising 11 structured and four “open” questions. All students felt that the program increased their confidence in planning oral rehabilitation. Ten students felt that the “best thing about the program” was the interaction with students from other universities and the exposure to different philosophies from the different schools. Eight students mentioned their increased awareness of the importance of patient input into holistic planning. Under the heading "What was the worst thing”, students cited some technical hitches and the snowball effect of two sluggish students who were not identified early enough and thus impacted negatively on the working of their groups. Student feedback showed that the module succeeded in its aims but needed modification to improve the logistics of working with an extended campus. Negotiating the Maze: Case based, Collaborative Distance Learning in Dentistry.

PURPOSE The purpose of this investigation was to determine the mean marginal discrepancy of metal-ceramic crowns fabricated with gold cylinders and cemented on implant abutments. These discrepancies were then compared with those measured previously for implant-supported ceramic crowns. MATERIALS AND METHODS Fifteen Nobel BioCare CeraOne abutments were connected to implant fixtures embedded in acrylic resin blocks. Marginal discrepancies were determined for gold cylinders, gold cylinders plus ceramic alloy (metal frameworks), completed metal-ceramic crowns, and cemented metal-ceramic crowns using a stereomicroscope equipped with a video camera linked to a computer. A Hotelling's T2 test (p < or = .05) was used to evaluate potential differences in mean marginal discrepancies among groups. RESULTS The mean marginal discrepancies were: 1) gold cylinders, 7.56 +/- 2.73 microns; 2) metal frameworks, 6.21 +/- 1.34 microns; 3) metal-ceramic crowns, 11.06 +/- 3.21 microns; and 4) zinc-phosphate cemented crowns, 31.47 +/- 6.65 microns. No significant difference between gold cylinders and metal frameworks was found. Mean marginal discrepancies for metal-ceramic crowns were significantly greater than discrepancies for cast gold cylinders. Cemented-crown mean marginal discrepancy was significantly greater than all other means. CONCLUSIONS Cemented metal-ceramic crowns fabricated using proprietary gold cylinders exhibited well-fitting margins (31.47 microns).
