PURPOSE: To assess the value of magnetic resonance (MR) imaging in the diagnosis of pelvic inflammatory disease (PID) and to compare MR imaging with transvaginal ultrasonography (US) and laparoscopy. MATERIALS AND METHODS: Thirty consecutive patients hospitalized because they were clinically suspected of having PID underwent transvaginal US and T1-weighted spin-echo, T2-weighted turbo spin-echo, and inversion-recovery MR imaging at 1.5 T. All patients underwent laparoscopy after MR imaging. RESULTS: PID was laparoscopically proved in 21 (70%) patients. The MR imaging diagnosis agreed with that obtained with laparoscopy in 20 (95%) of the 21 patients with PID. The imaging findings for PID were as follows: fluid-filled tube, pyosalpinx, tubo-ovarian abscess, or polycystic-like ovaries and free pelvic fluid. MR Imaging in Pelvic Inflammatory Disease: Comparison with Laparoscopy and US

The differentiation of a uterine leiomyoma from other solid pelvic masses on sonography is usually straightforward. Occasionally, the sonographic appearance of a pedunculated uterine leiomyoma may simulate that of a solid adnexal mass. The purpose of this study was to determine if MR imaging adds specificity to the diagnosis of indeterminate solid pelvic masses visualized with sonography. Nineteen patients were imaged with MR after sonography revealed the presence of a solid pelvic mass adjacent to the uterus but could not be used to determine whether the mass was a leiomyoma or some other type of tumor. The diagnostic criteria for a leiomyoma on MR imaging included (1) the presence of a mass adjacent to the uterus and (2) a mass that was predominantly low signal intensity or isointense compared with normal myometrium on T1-weighted images and predominantly low signal intensity on T2-weighted images. In 11 of 13 patients, the masses that met these MR criteria for leiomyoma were proved to be uterine leiomyomas at surgery. Another mass that met the criteria was shown to be a leiomyoma in the broad ligament; the other was an ovarian fibroma. Of the six cases that did not meet the MR criteria for the diagnosis of leiomyoma, three were proved to be degenerated fibroids, one was squamous cell carcinoma of the cervix, and two were ovarian malignant tumors. Because leiomyomas often have an MR appearance that is distinct from that of solid pelvic malignant tumors, MR imaging can be useful for the diagnosis of some indeterminate solid pelvic masses.

The value of MR imaging in distinguishing leiomyomas from other solid pelvic masses when sonography is indeterminate.

Purpose: Our goal was to test the hypothesis, as previously reported in other studies, that apparent diffusion coefficients (ADCs) provide specific information to diagnose ovarian tumors, especially to discriminate between benign and malignant lesions. Method: Tl- and T2-weighted spin echo imaging and diffusion-weighted echo planar imaging were performed in 31 women with 61 cystic components of ovarian tumors. Results: The lesions that showed typical watery intensity, hypointensity in Ti-weighted imaging, and hyperintensity in T2-weighted imaging had similar ADCs, ranging from 1.54 to 1.84 x 10 -3 mm 2 /s. The lesions that showed signal intensity different from typical watery intensity in conventional MRI tended to have low ADCs. In endometrial cysts, the mean ADC of the subgroup that showed typical watery intensity was higher than that of other subgroups. Conclusion: With conventional MRI, a tendency of ADCs could be predicted. ADCs may not provide additional information, especially to discriminate benign from malignant lesions.

Diffusion-weighted echo planar imaging of ovarian tumors: is it useful to measure apparent diffusion coefficients?

Diffusion-weighted echoplanar imaging (EPI) was performed to evaluate the contents of cystic ovarian lesions [33 endometrial cysts, 16 ovarian cysts, 5 serous cystadenomas, 6 mucinous cystadenomas, 13 malignant cystic ovarian tumors, and 3 benign cystic lesions mimicking malignant cystic ovarian tumor (BMMs)]. Apparent diffusion coefficient (ADC) values were calculated for the cystic contents. Many lesions showed unrealistically high ADC greater than water at body temperature (0.00324 mm(2)/sec), especially in lesions more than 12 cm in diameter. The higher ADC values were attributed to the sloshing effect: intermittent compression of large ovarian lesions by abdominal breathing before the breath-hold scan. When limited to lesions with ADC less than 0. 00324 mm(2)/sec, endometrial cysts and malignant cystic ovarian tumors showed lower ADC values than ovarian cysts and serous cystadenomas (P < 0.03). When further limited to lesions less than 12 cm in axial diameter, an additional ADC difference between malignant cystic ovarian BMMs was demonstrated (P < 0.03). Ultrasonography and magnetic resonance imaging of uterine fibroids

Magnetic resonance (MR) imaging, performed on 31 women with 40 surgically proven adnexal masses, was reviewed to determine whether MR can distinguish hemorrhagic from nonhemorrhagic masses. Fourteen masses proved to be hemorrhagic at surgery including functional ovarian cysts (seven cases), cystadenoma (one case), endometriomas (three cases), hematosalpinx (one case), ectopic pregnancy (one case), and parametrial extension from cervical carcinoma (one case). A high signal intensity (compared with adjacent pelvic fat) on a Tl-weighted spin echo sequence was found to be a reliable indicator of hemorrhage and was demonstrated in all 14 hemorrhagic masses. The “hematocrit” effect was seen as an area of high signal intensity layering in the dependent portion of six hemorrhagic cysts. Only four of 26 nonhemorrhagic masses demonstrated high signal intensity on Tl-weighted sequences and, of these, three proved to be fat-containing dermoid cysts and one was a simple cyst with adherent mesenteric fat. We conclude that high signal intensity on a Tl-weighted spin echo pulse sequence is evidence for a hemorrhagic mass and that a hematocrit effect appears to be a specific sign for a hemorrhagic cyst. The clinical significance of hemorrhagic adnexal masses is discussed.
