Title. The efficacy of reflexology: systematic review.

Aim.  This paper is a report of a systematic review to evaluate the efficacy of reflexology in any condition.

Background.  Anecdotal evidence has shown potential benefits of reflexology in a variety of health conditions. However, the efficacy of reflexology has yet to be determined.

Data sources.  Cochrane library, PubMed, MEDLINE, EBM review, ProQuest Medical Bundle and SCOPUS databases were searched using the following medical subject headings or key words: reflexology, foot reflexotherapy, reflexological treatment, foot massage and zone therapy. Chinese articles were searched through the Chinese electronic periodical services and Wangfane database. The publication date was limited from 1996 to 2007.

Review methods.  Studies were selected if they were written in English or Chinese, used a controlled clinical trial design, used reflexology as a stand-alone modality, and reported such outcomes as symptoms relief, quality of life and patients’ perceptions of reflexology. Study quality was reviewed based on the evidence rating system of the United States Preventive Services Task Force, and studies with the evidence rating of II-2 fair or above were included in this review.

Results.  Among the five studies suitable for review, there was only one report of a statistically significant treatment effect. Among the 12 outcome variables examined, the treatment effect size for urinary symptoms was large, whereas the effect size for other conditions was negligible.

Conclusion.  There is no evidence for any specific effect of reflexology in any conditions, with the exception of urinary symptoms associated with multiple sclerosis. Routine provision of reflexology is therefore not recommended.

Bioactive Essential Oils and Cancer

This systematic review synthesizes knowledge about the use of complementary and alternative medicine (CAM) among advanced cancer patients. EBSCO and Ovid databases were searched using core concepts, including advanced cancer, CAM, integrative medicine, and decision-making. Articles included in the final review were analyzed using narrative synthesis methods, including thematic analysis, concept mapping, and critical reflection on the synthesis process. Results demonstrate that advanced cancer patients who are younger, female, more educated, have longer duration of disease, and have previously used CAM are more likely to use CAM during this stage of illness. Key themes identified include patterns of and reasons for use; and barriers and facilitators to informed CAM decision-making. Knowledge regarding the use of CAM in advanced cancer remains in its nascent stages. Findings suggest a need for more research on understanding the dynamic process of CAM decision-making in the advanced cancer population from the patients' perspective.

Use of non-pharmacological interventions for comforting patients in palliative care: a scoping review.

Kaffir lime essential oil was obtained from fresh peels of Citrus hystrix (Rutaceae) by hydrodistillation and analysed by GC and GC–MS. The effects of kaffir lime oil on human autonomic and behavioural parameters after massage were investigated in this study. Forty healthy volunteers participated in the experiments. Autonomic parameters recorded were skin temperature, pulse rate, breathing rate and blood pressure. Behavioural parameters were assessed by means of visual analogue scales (VAS). The kaffir lime oil caused a significant increase in blood pressure and a significant decrease in skin temperature. Regarding the behavioural parameters, subjects in the kaffir lime oil group rated themselves more alert, attentive, cheerful and vigorous than subjects in the control group. These findings are likely to represent stimulating/activating effects of the kaffir lime oil and provide some evidence for the use of kaffir lime oil in aromatherapy, such as causing relief from depression and stress in humans. Chemical composition and stimulating effect of Citrus hystrix oil on humans

Many patients suffering from cancer make use of complementary therapies, with aromatherapy being a popular choice. Quantitative studies, using questionnaire data, have shown that aromatherapy may reduce psychological distress and enhance symptom control in cancer patients. However, little is known about the personal meanings patients associate with the therapy. This study explored the patients' experiences of aromatherapy using of a focus group interview. Eight themes emerged from the analysis, six of which have been acknowledged to some extent by previous authors: de-stressing effects of aromatherapy, the counselling role of the aromatherapist, aromatherapy as a reward, patient empowerment, communication through touch, and negative aspects of the service. Two apparently new themes emerged concerned with security of context (where the aromatherapy took place) and preconceived perceptions of the value of aromatherapy as a treatment of cancer patients. The implications of the findings are discussed in relation to the perceived role of counselling, collaborative practice and training in complementary therapies.
