TL;DR: The release of miR133a, as measured by its transcoronary concentration gradient, is associated with a higher incidence of MACE in patients with CAD, but it does not add significant prognostic information compared with traditional prognostic biomarkers, therefore limiting its potential usefulness in the clinical practice.

TL;DR: In this paper, a review focused on available data informing evidence-based interventional and pharmacological approaches for older adults with acute coronary syndromes, including guideline-directed management, showed that an invasive approach appears to demonstrate a better benefit-risk ratio compared to a conservative one across the ACS spectrum, even considering patients' clinical complexity and multiple comorbidities.
