Background:Ketamine is anN-methyl-D-aspartate receptor antagonist that has been shown to be useful in the reduction of acute postoperative pain and analgesic consumption in a variety of surgical interventions with variable routes of administration. Little is known regarding its efficacy in opiatedependent patients with a history of chronic pain. We hypothesized that ketamine would reduce postoperative opiate consumption in this patient population. Methods: This was a randomized, prospective, doubleblinded, and placebo-controlled trial involving opiate-dependentpatientsundergoingmajorlumbarspinesurgery.Fifty-two patients in the treatment group were administered 0.5 mg/kg intravenous ketamine on induction of anesthesia, and a continuousinfusionat10gkg 1 min 1 wasbegunoninductionand terminatedatwoundclosure.Fiftypatientsintheplacebogroup received saline of equivalent volume. Patients were observed for 48 h postoperatively and followed up at 6 weeks. The primary outcome was 48-h morphine consumption. Results: Total morphine consumption (morphine equivalents) was significantly reduced in the treatment group 48 h after the procedure. It was also reduced at 24 h and at 6 weeks. The average reported pain intensity was significantly reduced in the postanesthesia care unit and at 6 weeks. The groups had no differences in known ketamine- or opiaterelated side effects. Conclusions: Intraoperative ketamine reduces opiate consumption in the 48-h postoperative period in opiate-dependent patients with chronic pain. Ketamine may also reduce opioid consumption and pain intensity throughout the postoperative period in this patient population. This benefit is without an increase in side effects.

Evidence is accumulating that nurses completing pre-licensure programs are not equipped with the essential knowledge and skills for today’s nursing practice, nor prepared to continue learning for tomorrow’s nursing. Citing the need to improve quality and increase capacity, this paper offers three recommendations for transforming nursing education: (1) Create new nursing education systems which use existing resources in community colleges and universities and which provide for common prerequisites and a shared competency-based nursing curriculum and instructional materials. (2) Convene one or more expert panels to develop model pre-licensure curricula which: (a) can be used as a framework by faculty in community college-university partnerships for development of their local curriculum; (b) are based on emerging health care needs and widely accepted nursing competencies as interpreted for new care delivery models; (c) incorporate best practices in teaching and learning. (3) Invest in a national initiative to develop and evaluate new approaches to pre-licensure clinical education, including a required post-graduate residency under a restricted license. The author notes that these changes will require significant investment in the reforms, as well as in nursing education research and faculty development. The return on investment would be improved educational capacity and a better prepared nursing workforce, responsive to emerging health care needs and rapidly changing health care delivery systems. Citing the shift of significant responsibility to nurses for managing complex medical regimens, as well as increasing complexity of community based practices, Benner and colleagues concluded that nurses entering the field are not equipped with the essential knowledge and skills for today’s practice nor prepared to continue learning for tomorrow’s nursing (p. 31). Pain intensity and postoperative pulmonary complications among the elderly after abdominal surgery

The Institute of Medicine has called for more interprofessional collaboration between physicians and nurses. The purpose of this research was to compare the outcomes in affective and communication domains using a traditional (roundtable) model versus simulation in nursing and medical students. A prospective, descriptive survey design was used to collect data on 41 senior bachelor of science in nursing students, and 19 second-year medical students. The simulation strategy resulted in statistically higher levels of stress as identified by participants. In addition, nearly all participants reported having a better sense of the clinical role, and with 55 percent of participants stating that the experience changed their view of the role of the clinical team. This initial study indicates that interprofessional communication may be enhanced using simulation.

COMPARISON of Communication Outcomes in Traditional VERSUS Simulation Strategies in Nursing and Medical Students

Health promotion skills are a key component of most nursing education programs. While many curricula center around a singular health promotion project contained in one course, this nursing program saw the opportunity to use service-learning as a vehicle for developing a range of both health promotion and research skills. This report details a service-learning research program involving second and third year nursing students. Students conducted a community needs assessment, and designed, implemented, and evaluated health promotion programs that were developed as a result of the community needs assessment. Students collected data on the effectiveness of their programs through pre- and post-test design. Once in their statistics and research courses, students analyzed the data and prepared the data for presentation, developing their research skills. Results from the community on pre- and post-tests indicated significant gains in knowledge regarding hypertension and diabetes risk factors, prevention strategies, and intent to change behavior. Student outcome data was also collected and showed students perceived they had increases in the following skill sets: health promotion, assessment, civic engagement, and research.
