In this review the processes occurring on the treatment of triphenylphosphane with tetrachloromethane are first reported; a key role is played here by (trichloromethyl)phosphonium chloride which reacts with a further amount of the phosphane to give the stable salt [chloro(triphenylphos-phoranediyl)methyl]triphenylphosphonium chloride by way of dichlorophosphorane and (dichloromethylene)phosphorane that is detectable only as an intermediate. The preparative application of the two-component system as a chlorinating, dehydrating, and P—N-linking reagent is ascribable to “phosphorylation” of the substrate in question through several reactive species. In these applications the reactions with the substrate compete with reactions of the two-component system with itself, so that the overall course of the reaction is in nearly all cases much more complex than was previously assumed. Nevertheless. very good results can be achieved by the use of this reagent, high yields and mild reaction conditions being characteristic.

The present invention provides systems and methods for reducing the incidence of post-surgical tissue adhesions due to surgical incisions or perforations by applying heat, coolant, and/or sealant to or around the edges of the injured tissues, particularly to those in the peritoneum. One aspect of the invention is RF energy delivery systems (200), and control methods thereof. The RF delivery system is typically arranged to be coupled to a conventional surgical tool such as a trocar (70), and may include a surgical sheet (400), with one or more electrodes (401) for delivering the RF energy to the injured tissue resulting from conventional incisions into the abdominal wall. The treatment dosage of RF heat to the injured tissue may be controlled by monitoring parameters such as treatment time, change in tissue temperature, change in tissue dimension, and change in tissue impedance.

Systems and methods for reducing post-surgical complications

A hand-held applicator delivers liquid or semi-liquid compositions, such as adhesives, to an internal tissue or organ in an even and controlled manner. The applicator includes a pump, a tubular extension that is thin enough to pass through an endoscopic pathway or trocar, a proximal end of the tubular extension being sealingly connected to the pump, and an applicator tip that attaches to the distal end of the tubular extension. The tip includes the liquid and the pump pumps the liquid to the internal tissue in an even and controlled manner without contact of the liquid by the pump. The pump is preferably a micropipetter that includes a hand-held portion having a hand-actuatable plunger that does not come in direct, physical contact with the liquid to be dispensed. The composition may be an adhesive such as an alpha-cyanoacrylate. The device may further include a wound closure device including at least two closure pins extending from the distal end of the tubular extension. The wound closure device retracts from the applicator device and orients and aligns a wound or incision target tissue site immediately prior to application of the medical composition. The device provides improved control, ergonomics, and convenience. The device is particularly useful in the field of surgery, especially in the field of laparoscopic or endoscopic surgery, where an even and controlled amount of a medically-acceptable adhesive is to be applied to a target site on an internal tissue or organ.
