Silicone elastomer sheeting is a medical device used to prevent the development of and improve the appearance and feel of hypertrophic and keloid scars. The precise mechanism of action of silicone elastomer sheeting has not been defined, but clinical trials report that this device is safe and effective for the treatment and prevention of hypertrophic and keloid scars if worn over the scar for 12 to 24 hours per day for at least 2 to 3 months. Some of the silicone elastomer sheeting products currently on the market are durable and adhere well to the skin. These products are an attractive treatment option because of their ease of use and low risk of adverse effects compared to other treatments, such as surgical excision, intralesional corticosteroid injections, pressure therapy, radiation, laser treatment, and cryotherapy. Additional controlled clinical trials with large patient populations may provide further evidence for the efficacy of silicone elastomer sheeting in the treatment and prevention of hypertrophic and keloid scars. The purpose of this article is to review the literature on silicone elastomer sheeting products and to discuss their clinical application in the treatment and prevention of hypertrophic and keloid scars.

Significance: The physical and psychological sequalae of burn injuries account for 10 million disability-adjusted life years lost annually. Hypertrophic scarring (HSc) after burn injury results in reduced mobility, contracture, pain, itching, and aesthetic changes for burn survivors. Despite the prevalence of scarring and the number of scar therapies available, none are highly effective at preventing HSc after burn injury. Recent Advances: Recent studies modulating the mechanical environment surrounding incisional and excisional wounds have shown off-loading of tension to be a powerful strategy to prevent scar formation. Preclinical studies applying force perpendicular to the surface of the skin or using a combination of pressure both circumferentially and perpendicularly have shown substantial reductions in scar thickness and contraction after burn injury. Critical Issues: Though pressure therapy is highly effective in preclinical studies, outcomes in clinical studies have been variable and may be a result of differing therapy protocols and garment material fatigue. A recent adult clinical study reported a significant reduction in pressure after 1 month of use and significant reduction between 1 and 2 months of use, resulting in below therapeutic doses of pressure applied after only 1 month of use. Future Directions: To enhance efficacy of pressure garments, new low-fatigue materials must be developed for use in standard garments or garments must be redesigned to allow for adjustment to compensate for the loss of pressure with time. Additionally, measurements of applied pressure should be performed routinely during clinic visits to ensure that therapeutic doses of pressure are being delivered.

Bei ihrer Versorgung gilt das Prinzip: von innen nach ausen. Zunachst werden knocherne Verletzungen, anschliesend Schleimhaut und Muskulatur und erst abschliesend die Haut versorgt. Bissverletzungen konnen in der Regel nach den gleichen Kriterien behandelt werden. Narben verursachen insbesondere um Mund, Augen und Nase erhebliche funktionelle Storungen. In der Regel erfolgt die Korrektur fruhestens nach 6 Monaten. Die Behandlung von Keloiden ist problematisch.

Introducao:There is no standardized approach to immediate post-procedure care for epidermal ablation; the conscientious choice of products can provide greater comfort to the patient, reduced risks and complications, and shorter recovery times. Objective: To analyze the safety and effectiveness of silicone spray in tissular restoration after two epidermal ablation procedures ‐ fractional ablative 2,790 nm YSGG laser and dermabrasion. Methods: Patients with photoaging or acne scars (n = 20) applied silicone spray on one side of the face and liquid Vaseline (control) on the other side of the face after the procedures.Transepidermal loss of water was measured in all patients. Results: Erythema and burning sensation were the most common findings, and were significantly more frequent in the areas where Vaseline was applied. Conclusions: Silicone spray is safe and effective for use after epidermal ablative procedures, and is significantly superior to Vaseline in controlling burning sensations and erythema.

Silicone thermoplastic sheeting has been used successfully in the management of hypertrophic and keloid scars resulting from thermal burn injuries. A technique is described that incorporates silicone thermoplastic sheeting for fabrication of a compression face mask. This technique combines the moldability of thermoplastic splinting materials with the therapeutic surface of silicone, yielding the advantages of both in a one-step process.
