Three hundred and fifty consecutive patients (322 non-obstetric, 28 obstetric; 185 female) were assessed before operation using the modified Mallampati test and by measuring thyromental and sternomental distances, forward protrusion of the mandible and interincisor gap with the mouth fully open. Tracheal intubation was difficult in 17(4.9%) patients, of whom four (1.14%) had a grade III or IV view on laryngoscopy. A sternomental distance of 12.5 cm or less with the head fully extended on the neck and the mouth closed predicted 14 of the 17 patients in whom tracheal intubation was difficult. As a screening test, sternomental distance appeared to be more sensitive (82.4%) and more specific (88.6%) than thyromental distance (64.7% and 81.4%, respectively), the modified Mallampati test (64.7% and 66.1%, respectively) and forward protrusion of the mandible (29.4% and 85.0%, respectively). The interincisor gap ranged from 2.0 to 5.0 cm in all patients except one. There was no correlation between the interincisor gap and the view on laryngoscopy ( P > 0.05, one-way ANOVA). There was also no difference in mean interincisor gap between those patients who presented no difficulty with tracheal intubation and those who did ( P = 0.7–0.8, two sample t test). Sternomental distance may be a useful bedside screening test for preoperative prediction of difficult tracheal intubation.

Modified closed cap splint: Conservative method for minimally displaced pediatric mandibular fracture.

Background Children with microtia may experience difficult laryngoscopy because the ear and the mandible develop from the first and second bronchial arches and clefts. Methods In this prospective observational study, we enrolled 166 patients (6-14 years old) with microtia scheduled for auricular reconstruction with autologous costal cartilage framework. Airway of the subjects was assessed preoperatively with the Modified Mallampati classification, Thyromental distance, Interincisor gap, Forward protrusion of the mandible and Horizontal length of the mandible. Anesthesiologist performed the direct laryngoscopy. Difficult laryngoscopy was classified as grade III or IV based on the Cormack-Lehane classification. Results A total of 166 patients completed the study, including 158 (95%) isolated microtia patients and 8 (5%) hemifacial microsomia patients. The incidence of difficult laryngoscopy was 34/166 (20.5%) in all patients, 3/8 (37.5%) in hemifacial microsomia patients, 31/158 (19.6%) in isolated microtia patients (P=0.35). Multivariate logistic regression shows that Thyromental distance, Interincisor gap, Forward protrusion of the mandible are all independent predictors of difficult laryngoscopy. The sensitivity and specificity of each predictor were: Thyromental distance (82.35%, 89.39%), Interincisor gap (58.82%, 86.36%), and Forward protrusion (17.65%, 97.73%), respectively. The cutoff values of Thyromental distance and Interincisor gap for prediction of difficult laryngoscopy were 4.0 cm and 3.3 cm. The best combination of predictors was Thyromental distance/Interincisor gap/Forward protrusion of the mandible with a sensitivity of 94.12% and specificity of 86.36%. Conclusions The combination of Thyromental distance, Interincisor gap, and Forward protrusion of the mandible is the optimal assessment to predict difficult laryngoscopy in school-aged patients with microtia.

Mandibular fractures are relatively less frequent in children when compared to adults, which may be due to the child's protected anatomic features and infrequent exposure of children to alcohol related traffic accidents. Treatment principles of mandibular fractures differ from that of adults due to concerns regarding mandibular growth and development of dentition. A case of a 7 year old boy with fractured left body of mandible managed by closed reduction using open occlusal acrylic splint and circummandibular wiring is presented. Patient was reviewed on regular intervals, the circum-mandibular wiring and splint was removed after 15 days under local anesthesia. No mobility was present at the fracture site. Postoperative recovery was uneventful and satisfactory occlusion was achieved. Patient had good masticatory efficiency.

Management of Mandibular Body Fractures in Pediatric Patients: A Case Report with Review of Literature

India is the second-highest contributor to the post-2000 global greening. With satellite data, here we show that this 18.51% increase in Leaf Area Index (LAI) during 2001-2019 fails to translate into increased carbon uptake due to warming constraints. Our analysis further shows 6.19% decrease in Net Primary Productivity (NPP) during 2001-2019 over the temporally consistent forests in India despite 6.75% increase in LAI. We identify hotspots of statistically significant decreasing trends in NPP over the key forested regions of Northeast India, Peninsular India, and the Western Ghats. Together, these areas contribute to 31% of the NPP of India (1274.8 TgC.year -1). These regions are the warming hotspots in India. Decreasing photosynthesis and stable respiration, above a threshold temperature, are the key reasons behind the declining NPP. Warming has already started affecting carbon uptake in Indian forests and calls for improved climate resilient forest management practices in a warming world.

Ameloblastic fibroma (AF) is a rare tumour of mixed odontogenic origin that can occur either in mandible or maxilla but is most frequently found in the posterior region of mandible. Age of occurrence is generally between first and second decades of life. It is often mistaken for a den tigerous cyst due to presence of an impacted tooth. The diagnosis of AF usually occurs accidentally by routine radiographic examination for an impacted tooth. Histologically it consists of odontogenic ectomesenchyme resembling the dental papilla, epithelium resembling dental lamina and enamel organ without dental hard tissues. There is controversy in the literature as to whether the treatment should be conservative or a radical resection should be done. A conservative treatment strategy, such as enucleation and curettage, is usually sufficient. We describe a case of massive ameloblastic fibroma in a 5-year-old child with an unusual position in maxillary posterior region and without any impacted tooth. Surgical resection of the tumor through Weber Ferguson approach was done under GA with 2 years of follow-up without any recurrence.

Objective: To determine a clinically useful variable for predicting difficult tracheal intubation in patients with seemingly normal airways. Material and Methods: In our study we had included 68 patients from September 2011 to September 2013 who needed tracheal intubation for elective maxillofacial surgery. An airway assessment test was conducted on each patient prior to general anaesthesia, with respect to mouth opening, sternomental distance, thyromental distance, oropharyngeal (Mallampati) classification and ability to protrude the mandible. After induction of anaesthesia, the laryngeal view during laryngoscopy was graded and then the ability to intubate was assessed. Results: Incidence of difficult intubation occurred in 8 (11.76%) cases out of 68 patients. Airway test that was significant for predicting difficult tracheal intubation was SMD of less than 12.5 cm, TMD of less than 6 cm, a score according to Mallampati et al. of greater than III, protrusion of mandible position B and position C and IIG less than 3 cm with sensitivity of 87.5%, 62.5%, 62.5%, 25% and 50%, respectively.
