EGFR signaling pathway occupies an important position in cancer-related downstream signaling pathways of Pyk2.

Objective: Gallbladder cancer (GBC) is the most common biliary tract malignancy found worldwide with very high incidence in North India especially Delhi region. It is characterized by poor prognosis and ineffective treatment especially in advanced stage. The aim of this study was to evaluate EGFR and HER2/neu immunoexpression in cancer patients and to correlate it with the clinicopathological parameters so as to identify GBC patients who can benefit from targeted therapy.Methods: Present study was conducted in the Department of Pathology, Hamdard Institute of Medical Sciences & Research, New Delhi. A total of 40 cases of Gallbladder carcinoma (GBC) were evaluated for Immunohistochemical expression of EGFR and HER2/neu. Clinicopathological parameters of GBC were studied and correlated with immunoexpression of EGFR and HER2 /neu. Result: The mean age of the GBC patients was 55.9 years with 90% being females. On histopathology, 34(85%) cases were conventional adenocarcinoma. The EGFR expression was positive in 29/40 cases (72.5%). It was significantly more positive in poorly differentiated grade and advanced stages of gall bladder carcinoma (P<0.05). The expression of HER2/neu was positive in 13/40 cases (32.5%). It was significantly more positive in well differentiated gall bladder carcinoma (P<0.05). Immunoexpression of EGFR was inversely related with HER2/neu expression and this association was statistically significant.Conclusion: Among GBC patients, EGFR expression and HER2/neu expression was 72.5% and 32.5%, respectively. Significant EGFR expression was seen in poorly differentiated and advanced stage cancers while significant HER2/neu expression was seen in well differentiated gall bladder carcinomas. To conclude, these two markers HER2/neu and EGFR can be used as predictive and prognostic markers respectively, with rationale to further explore the use of anti-HER2 and anti- EGFR therapy in gall bladder cancer.

EGFR and HER2neu Expression in Gall Bladder Carcinoma and Its Association with Clinicopathological Parameters - An Institutional Experience from North India

High-altitude exposure and cerebral venous thrombosis: an updated systematic review. High Alt Med Biol. 24:167–174, 2023. Background: High altitude (HA) may increase the risk of cerebral venous thrombosis (CVT). Differentiating it from other HA illnesses is crucial for prompt treatment and better outcomes. We aimed to summarize the clinical data, etiology, and risk factors of this poorly understood entity at an HA. Materials and Methods: A systematic literature search of various databases, including PubMed, Embase, and Google Scholar, was done using relevant keywords; cerebral venous thrombosis; HA, up to May 1, 2022. Results: A total of nine studies, including 75 cases of CVT at HA (3,000–8,848 m), with 66 males and 9 females, were included in this review. Headache and seizure were the most common clinical presentations. Smoking, drinking habits, and the use of oral contraceptive pills (OCP) were the most common risk factors for the development of CVT. Similarly, various underlying hypercoagulable states were also present among cases of CVT associated with HA exposure. Conclusion: Our review concludes that HA exposure can predispose individuals with risk factors such as preexisting hypercoagulable states, smoking, drinking habits, and use of OCP to an increased risk of CVT. 

High-Altitude Exposure and Cerebral Venous Thrombosis: An Updated Systematic Review.

Introduction: Soft tissue tumors are tumors originating from non-epithelial tissues of the body and are seen in extra skeletal location. They do not include glia, reticulo-endothelial system and other supporting tissues of different parenchymal organs. Classification of soft tissue  tumors has histogenetic basis, based on the adult tissue to which they bear a resemblance.
Objective: To evaluate the histomorphological features of benign soft tissue tumors in the different age groups and gender as seen in a medical college of North India, giving clue about prevalence of disease morphology in particular region.
Materials and Methods: This study was retrospective and was done in the department of Pathology, T. S. Misra Medical College and Hospital, Lucknow, Uttar Pradesh. All biopsies diagnosed as benign soft tissue tumors in our Pathology department during study period of two years and eight months were reviewed retrospectively in the present study.
Results and Conclusion: Benign soft tissue tumors in our region have the peak age of incidence in the fourth decade of life. They are slightly more common in males with male: female ratio of 1.6:1. Commonest benign soft tissue tumor is lipomatous tumor. Extremities are the commonest site of these soft tissue tumors.

Histomorphological features of benign soft tissue tumors in a medical college of North India

Hypoxia and subsequent sojourn at high altitude have long been suggested as risk factors for thrombosis. A few small studies and case reports supporting this view have been earlier reviewed by Gupta & Ashraf. Besides increased blood viscosity, a reduced fibrinolytic capacity, a procoagulatory state and an inflammatory activation of the endothelium have been proposed as possible reasons for high altitude induced thrombosis. 6 Recently the long noncoding RNAs LINC00659 and UXT-AS1 have been identified as possible molecular modulators of high altitude induced thrombotic events. In this issue of The Lancet Regional Health Southeast Asia, Velu Nair and colleagues report the results of a challenging, large prospective longitudinal study that investigated thrombotic events in a cohort of 960 acclimatized lowlanders at high altitude. Of these 960 male soldiers, 750 ascended to altitudes above 15.000ft (4472 m). Clinical screening and determination of markers of coagulation and fibrinolysis were performed in blood samples obtained from these individuals once at sea level and twice at high altitude (at 12 15,000ft and at >15,0000ft, respectively). The authors reported 15 thrombotic events in their cohort, of which twelve were venous and three were arterial. The affected individuals showed significantly higher levels of procoagulant Factor VIIa and Xa, and lower levels of the anticoagulant modulators thrombomodulin and tissue factor pathway inhibitor (TFPI) compared to healthy controls. Epidemiology and pathophysiology of vascular thrombosis in acclimatized lowlanders at high altitude: A prospective longitudinal study

Aim: Gallbladder cancer (GBC) is an aggressive disease with poor prognosis and complete surgical resection offering the only cure. Increased epidermal growth factor receptor (EGFR) expression has been noted in various cancers including GBC. Several studies across the world have examined the expression of EGFR in GBC. This study has been done to see the EGFR expression in GBC in Indian context. Materials and Methods: Fifty cases of GBC were evaluated histopathologically using hematoxylin and eosin stained sections. Immunohistochemical assessment of EGFR expression was done, and scoring was done as per Kaufman et al. Data were collected, tabulated, and analyzed statistically by SPSS 16.0 version (Chicago, Inc., USA) software. Results: Of 50 cases, 44 revealed EGFR over-expression while 6 were negative. Of the 44 cases, 10 had weak EGFR immunostaining intensity (1+), 26 had moderate (2+), and 8 showed strong EGFR immunostaining (3+). We found that most of the cases showing weak EGFR immunostaining intensity (1+) were well-differentiated tumor (70%) and cases with a strong EGFR immunostaining intensity (3+) were poorly differentiated cases of adenocarcinoma (75%). Moderately differentiated adenocarcinoma showed moderate EGFR immunostaining intensity (2+) in most of the cases (53.8%). Conclusion: EGFR is expressed in most of the cases of GBC. In well-differentiated adenocarcinoma, the EGFR expression is less compared to EGFR expression in poorly differentiated tumor, leading to the conclusion that the differentiation of the tumor and EGFR expression is inversely related. Thus, intensity of EGFR expression may correlate with aggressiveness of disease.

Epidermal growth factor receptor expression in carcinoma gallbladder: A prospective study in Indian scenario.

INTRODUCTION Head and neck squamous cell carcinoma (HNSCC) is sixth most common neoplasm worldwide and despite the progress made in the diagnosis and treatment, the survivalof these patients has not improved significantly [1] . Therefore a vigilant search for newer diagnostic and prognostic markers along with fresh molecular targets is required for the prevention & cure of HNSCCs and related tumors. Epidermal growth factor receptor (EGFR) and Ki 67 labelling index are reflective of continued cell growth and multiplication, thereby inducing tumorigenesis [2,3] . They are associated with disease progression, which in turn is a pointer towards tumor aggressiveness The importance of studying EGFR expression status lies in the fact that targeted therapy against this protein is available and has great potential [4] . However, extensive online search in indexed journals yielded very few results regarding the prognostic significance of Ki67 in HNSCC. In this study, wecompared the immunohistochemical expression status of EGFR and KI-67 LI with various clinicopathological parameters.
