Illustrated manual of pediatric dermatology: diagnosis and management

Background: Cutaneous manifestations are common in neonates. Transient and pathological neonatal dermatoses should be differentiated to avoid unnecessary treatment and thus considering the variable nature and severity of neonatal skin lesions, it is important to be aware of the transient skin lesions in newborn and to differentiate these from other serious conditions which will avoid unnecessary therapy to the neonates and the parents can be assured of good prognosis of these skin manifestations. The present study has been carried out to study the clinical pattern of cutaneous lesion in neonatal period. Methods: A total of 255 neonates from Department of Dermatology in collaboration of Department of Pediatrics, Era’s Lucknow Medical College and Hospital were evaluated for cutaneous manifestations. A detailed assessment regarding history, clinical examination and investigations were recorded and analyzed. Results: Out of 255 neonates, 138 (54.1%) were males and 117 (45.9%) were females. The sex ratio (M:F) was 1.18. The most common dermatoses were physiological desquamation (54.1%) and mongolian spots (37.6%) followed by milia (19.6%), miliaria (14%), epstein pearls (10.2%). Conclusions: The physiological and transient cutaneous lesions are common are in neonates. Physiological cutaneous manifestations were quite frequent apart from birthmarks/congenital cutaneous manifestations. An understanding of these manifestations helps in managing and deciding the appropriate manifestation.

A clinical study of various cutaneous manifestations in neonates

Introduction: Skin manifestations are common in neonates. The prevalence of skin changes and their association with neonatal and maternal factors are not adequately documented in Nepal. It is important to differentiate common benign skin lesions from infrequent skin changes that cause morbidity, requiring medical management. The objectives of this study were to determine the pattern of skin lesions in new-borns and their association with maternal and neonatal variables.Material and Methods: This was a prospective, cross-sectional study conducted in obstetric unit of the hospital from December 2016 to May 2017. Outborn babies and babies shifted to NICU were excluded.Results: A total of 935 new-borns were examined. The commonest skin lesions were Mongolian spot (66.7%), Erythema toxicum (46.6%), Milia (44.4%), Epstein pearls (26.4%) and Salmon patch (11.9%). Mothers aged 35 years or older (63.6%) had statistically significant association with Erythema toxicum (p=0.01). Erythema toxicum was more in primiparity, term pregnancy and caesarean section delivery. Milia was associated with male babies. Salmon patch was seen more in term pregnancy. Congenital melanocytic nevus, haemangioma and port-wine stain were noted in three, two and one new-borns respectively, which alter morbidity and hence are important to diagnose in time.Conclusion: We found that 93.8% of neonates had at one or more skin lesions. The majority of skin lesions were benign and transient. Erythema toxicum was seen commonly in mothers aged 35 and more. Salmon patch was seen more in term pregnancy.

Variations of Dermatological Findings in New-Borns of a Community Hospital in Nepal

Background: A wide range of disorders can manifest as skin lesions due to the great difficulty the skin encounters while transitioning from an aqueous to a dry environment in a neonate. Among the skin conditions commonly seen in neonates are temporary benign lesions, atopic dermatitis (and associated disorders), bullous dermatoses. Aim was to study the various patterns of skin lesions in neonates and to estimate the prevalence of physiological and pathological skin lesions. Methods: This prospective observational study was conducted in the department of pediatrics at Acharya Shri Chander College of Medical Sciences and Hospital (ASCOMS) a tertiary care hospital in Jammu. A total of 300 neonates (day 1 to day 28 of life) and having any skin lesions or manifestations born at the hospital or visiting out patient’s department were included in the study. The data collected was compiled on excel sheet and was analyzed statistically by using statistical package for the social sciences (SPSS) software version 21.0. Results: It was found that the most of the skin manifestations were observed in neonates born to multiparous mothers at term gestation and having average weight. The male to female ratio was 1.3:1. The frequency of the cutaneous manifestations were seen more in neonates born via the caesarean section. Conclusions: It is concluded that the most common skin lesion was erythema toxicum followed by icterus neonatorum. 

Cutaneous findings in neonates and their socio-demographic profile: a prospective observational study

INTRODUCTION: Maturation of the skin starts at birth with the adaptation to the external environment. To deliver proper skin care it is necessary to know about the characteristics of neonate’s skin and conditions arising from or involving it secondarily. The skin of the preterm neonate is immature, has weaker dermoepidermal attachment which place them at higher risk of systemic dissemination from cutaneous
infections.
AIM OF THE STUDY: 
To determine the prevalence of various dermatoses and to detect association between sex, birth weight, gestational age, mode of delivery, consanguinity and dermatoses.
METHOD: 
A cross sectional study of 100 preterm neonates with cutaneous changes born during one year period was examined.
RESULTS: 
79% of the neonates had physiological changes, 15% had pathological conditions and 6% had both. Among the physiological conditions milia was the most common (77%) followed by epstein pearl (66%), mongolian spot (63%) and others. Pathological conditions were congenital anomalies (5%), ichthyotic disorders, nevi, etc. Lanugo hair, physiological icterus and epstein pearl were more frequent in neonates with ≤ 34 weeks of gestational age. No significant relationship was found between the mode of delivery, consanguinity, parity and any of the common dermatoses observed in our study.
CONCLUSION: 
Although skin changes are common in neonatal period, most of them are benign and transient thus requires no treatment. We should be aware of these dermatoses in order to distinguish them from serious conditions and to assuage parental anxiety.

A Clinico Epidemiological Study on Dermatoses in Preterm Neonates

Background: Skin being the major organ of human body, any lesion on the newborn can cause concern and
 anxiety to parents. Many such lesions can be physiological and transient. There are very few reports in the literature
 regarding the cutaneous manifestations in newborns in India.
Aim: This study is undertaken to determine the prevalence and pattern of various skin lesions in neonates and to
 correlate the relationship between new born skin lesions to gestational age, birth weight, mode of delivery, sex and
 maternal factors.
Materials and methods: All new born delivered at a referral hospital between 2013-2015 were included for the
 study. Data regarding maternal age, illness, drugs, gestational age, mode of delivery, birth weight, sex of new born
 were noted. All the new born were clinically examined for skin lesions during their stay in hospital.
Results: Total of 1427 live births was included. Out of this 92.1% of newborns had skin lesions and 7.9% had no
 skin lesions. The incidence of single lesion was 25.4% and multiple in 66.5%. Common manifestations included
 mangolian spots, milia, ebstein pearls, erythema toxicum neonatorum. Our study showed increased incidence of
 mangolian spots, lanugo hairs and cutis marmorata in new-borns of mothers having illness and in preterms. Birth
 injuries were more in instrument assisted deliveries. The skin peeling and birth marks were more in posterms.
 However there was no difference in the manifestations between male and female babies.
Conclusion: Majority of the skin lesions are transient and physiological which does not require any treatment.
 Recognizing these transient lesions in new born will avoid unnecessary therapy to the new-borns and it will enable
 the paediatrician to allay parental concern and anxiety.
