Hypothyroidism presents a large epidemiological burden in India As a result of subtle and nonspecific clinical symptoms and signs, the condition often goes undiagnosed and is not adequately treated when it is detected There is heterogeneity in the diagnostic and treatment approaches to hypothyroidism As a result of the physiological changes in thyroid hormones with age and
 illness, it is important to tailor the diagnosis and management of this condition in specific populations including pregnant women, infants, children, geriatric patients, and those with comorbid conditions Enhanced understanding and education of physicians and patients can help to improve the outcomes of treatment in hypothyroidism which should be focused on patient-centered care Policies and reforms should be crafted and implemented at the national level to curb public health challenges of hypothyroidism This publication summarizes the recommendations of a national advisory board meeting to identify and bridge the gaps in understanding of the diagnosis and treatment of hypothyroidism in India As a complement to clinical judgment, these recommendations will foster the diagnosis and management of hypothyroidism in the community and clinics for the benefit of the patients Merck Ltd, India

Study of Thyroid Profiles in the Elderly

Background: Thyroid gland diseases are common in India, particularly hilly and tribal areas. The incidence of thyroid conditions, exact figure is not available but about 5 lakh cases come for Medical Treatment all over India. To study the prevalence of common thyroid diseases in the Sangareddy district area, which is near Hyderabad, particularly age & sex specific incidence.Methods: This is a retrospective study conducted at the Maheshwara Medical College and Hospital, Chitkul, Sangareddy district of Telangana State. The data was collected from the registers of General Surgery outpatient department, operation theatre, medical record section and cytology, histopathology registers of pathology department. A total number of 64 patients were included in this study who was admitted at this hospital during the period of 2 years, from January 2017 to December 2018. The data was analysed by proper statistical methods.Results: The study had shown that most common conditions in this area are benign diseases. The age group ranges 21-30 years, female are most vulnerable (37.51%). The pathological benign condition most common is nodular goiter 48.43%.Conclusions: The observations in this study made may be useful in future to diagnose the cases and advice regarding prevention of the disease.

Incidence of thyroid diseases in a tertiary hospital: a retrospective study

Thyroid disorders are associated with the abnormalities occur in thyroid hormone concentration and imbalancein their regulation may causes life threatening diseases. Thus, the present study was planned to establish theprevalence of thyroid defects in Diyala, Iraq due to paucity of information is available in our region. Total67 subjects in the age range of 20-80 years were recruited in Al - Shafa hospital, Diyala, Iraq, sufferingfrom thyroid disorders or suspected with the symptoms favoring thyroid diseases. Thyroid function test andphysical examination were performed. Those who were found alteration in thyroid hormones and presenceof nodules, further investigation like FNAC of neck was performed for the diagnosis. Once confirmation ofthyroid defect, total thyroidectomy was performed as a first line of treatment and malignancy was confirmedusing histopathology analysis. The incidence of thyroid diseases in female (86.57%) was found to be morethan male (13.43%) and there ratio was 19.3:3. The thyroid defects incident rate was highest in mean 49years age. Almost 61.2% patients were found affected by thyroid disorders in this age group. The largestnumber of patients (64.18%) had shown MNG, then other thyroid defects out of which 4 female patientspresented very big thyroid multinodular goiter (MNG). Among 43 MNG patients, 86% patients were femaleand 4% were male. Of the 67 cytologically diagnosed cases, 79.10% cases were found benign cases whichare highest among all other types, out of which 91% cases were female. Among the thyroid disorder burdenof thyroid carcinoma is very high and common in the enrolled patients in Diyala, further longitudinal studiesare required to explore the prognosis and causes of these thyroid defects in the Iraq.

Prevalence of Thyroid Defects in Diyala, Iraq

Background: Autoimmune thyroid disease results from a complex interaction between genetic and enviromental factors. The aim of this study was to find association of anti-thyroid peroxidase antibody with abnormal thyroid function tests. Materials and Methods: This is a retrospective  study, conducted in 160 patients who showed abnormal thyroid function tests  in grande city clinic and hospital services and Kantipur dental college  for  duration  of 18 months since Baisakh 2072. Results: Among 160 individuals , 126(78.8%) were female and 34(21.2%) were male. Subclinical hypothyroidism (56.3%) were most common than overthyperthyroidism (18.0%), overthypothyroidism (16.9%) and subclinical hyperthyroidism (8.8%). Anti thyroid peroxidase antibodies test  was shown positive in  102/160 (63.8% ) cases. Conclusion: According to the present study, anti- thyroid peroxidase antibody has been significantly associated with thyroid dysfunction, it can be used as diagnostic marker for thyroid autoimmune diathesis .

Relationship of thyroid peroxidase antibody test with abnormal thyroid function tests

Aims and objectives: the study was undertaken with an objective to study the spectrum of thyroid dysfunction in elderly and to correlate clinical symptoms with abnormal thyroid function. Methods: a total of 100 subjects aged more than 60 years, admitted to General Medicine IPD of MVJMC & RH who were suspected to be suffering from thyroid disorders or subjects with vague symptoms like generalized weakness, easy fatigability, lethargy, disinterest in daily activities, to be suspicious of thyroid disorder, were subjected to detailed clinical examination & thyroid function testing by biochemical means. Those who were found to have altered thyroid functions, thyroid antibody (TPO) test were done. When thyroid nodule was present, further relevant investigations like USG/FNAC neck was performed to make appropriate diagnosis. Results: a total of 100 patients were included in the study. Thyroid disorders were present in 25%. Overt hypothyroidism in 11%, subclinical hypothyroidism in 9% cases, hyperthyroidism in 2% and subclinical hyperthyroidism in 3% patients was noted. In this study 38 patients were males and 62 were females. Females (18%) had high incidence of thyroid disorders than males (7%). As the age advanced the incidence of thyroid dysfunction increased which was statistically significant. Overt hypothyroidism was more common among elderly (11%). Classical clinical features of hypothyroidism and hyperthyroidism were present in 9 and 2 patients respectively. All patients with hypothyroidism had positive TPO AB.
