Background and Objectives: This study aimed to perform a meta-analysis comparing the effects of corticotomy and flapless piezocision on accelerated tooth movement. Materials and Methods: A comprehensive search using a combination of controlled vocabulary (MeSH) and free-text terms was undertaken by two reviewers to identify published systematic reviews. Three major electronic databases (Medline via PubMed, Cochrane Database, and Embase) were searched up to 2 June 2023. Results: The results of the meta-analysis showed that the pooled standardized mean difference values of accumulative movement distances for flapless piezocision were 1.43 (95% CI, 0.38 to 2.48; p < 0.01), 1.09 (95% CI, −0.08 to 2.26; p = 0.07), and 0.73 (95% CI, −0.58 to 4.02; p = 0.14). The results of the meta-analysis demonstrated that the pooled SMD values of accumulative movement distances for the corticotomy were 2.76 (95% CI, 0.18 to 5.34; p = 0.04), 1.43 (95% CI, −1.10 to 3.96; p = 0.27), and 4.78 (95% CI, −4.54 to 14.10; p = 0.32). Although the test for overall effectiveness was significant for piezocision and corticotomy, there were no significant differences between piezocision and corticotomy. Conclusions: The study determined that both conventional corticotomy and flapless piezosurgery are effective as adjuncts to orthodontic treatment. Moreover, no significant difference was observed in the short-term effectiveness of canine retraction acceleration between conventional corticotomy and flapless piezocision. While piezocision may be a favorable option for orthodontic treatment, corticotomy can be considered in cases requiring additional procedures such as bone grafting.

<h2>Abstract</h2> Piezocision™ assisted orthodontics as a technique was first published 14 years ago. Technical advances in digital dentistry over the past 10 years have brought some important innovations to Piezocision™ assisted orthodontic surgery. Dynamic Navigated Piezocision™ allows the surgeon to see the corticotomies in REAL TIME on the monitor screen, thus avoiding roots, anatomical structures and getting to proper depth. Piezocision™ allows for Precision orthodontics. In the past 10 years it has helped in distalization mechanics, bite opening, severe torque issues, treatment of posterior crossbites in adults, posterior open bites, anterior open bites, alveolar insufficiencies and impacted teeth eruption.

Periodontitis as a comorbidity in systemic lupus erythematosus (SLE) is still not well recognized in the dental and rheumatology communities. A meta-analysis and network meta-analysis were thus performed to compare the (i) prevalence of periodontitis in SLE patients compared to those with rheumatoid arthritis (RA) and (ii) odds of developing periodontitis in controls, RA, and SLE.Pooled prevalence of and odds ratio (OR) for periodontitis were compared using meta-analysis and network meta-analysis (NMA).Forty-three observational studies involving 7,800 SLE patients, 49,388 RA patients, and 766,323 controls were included in this meta-analysis. The pooled prevalence of periodontitis in SLE patients (67.0%, 95% confidence interval [CI] 57.0-77.0%) was comparable to that of RA (65%, 95% CI 55.0-75.0%) (p>0.05). Compared to controls, patients with SLE (OR=2.64, 95% CI 1.24-5.62, p<0.01) and RA (OR=1.81, 95% CI 1.25-2.64, p<0.01) were more likely to have periodontitis. Indirect comparisons through the NMA demonstrated that the odds of having periodontitis in SLE was 1.49 times higher compared to RA (OR=1.49, 95% CI 1.09-2.05, p<0.05).Given that RA is the autoimmune disease classically associated with periodontal disease, the higher odds of having periodontitis in SLE are striking. Higher odds of periodontitis in systemic lupus erythematosus compared to controls and rheumatoid arthritis: a systematic review, meta-analysis and network meta-analysis

OBJECTIVES
To assess the effect of piezocision on periodontal tissues and alveolar bone height and to detect lower second molar root resorption in piezocision-assisted mandibular second molar protraction compared to no-piezocision molar protraction.


MATERIALS AND METHODS
Twenty-one subjects (four males, 17 females, aged 22.43 ± 2.83 years) who presented with bilateral extraction of lower first molars were included. The patients were divided into two groups; Group 1: Piezocision-assisted molar protraction (right or left side of subjects) in which piezocision was performed immediately before lower second molar protraction and, Group 2: No-piezocision molar protraction in which lower second molar protraction was not surgically assisted. Plaque index (PI), gingival index (GI), periodontal pocket depth (PPD), width of keratinized gingiva (WKG), gingival recession (GR), lower second molar mesial root resorption, alveolar bone height, and mandibular bone height were recorded at T1 (immediately before molar protraction) and at T2 (after second molar space closure).


RESULTS
In the piezocision-assisted molar protraction group, significant changes were detected in the WKG (P < .001), GR (P < .05), and the mandibular bone height (P < .001). Compared to the no-piezocision group, piezocision-assisted molar protraction resulted in an increased WKG (P < .001) and less second molar mesial root resorption (P < .01).


CONCLUSIONS
Piezocision does not have any detrimental effect on the periodontium and produces less root resorption.

Effect of piezocision-assisted lower second molar protraction on periodontal tissues, alveolar bone height, and lower second molar root resorption.

Rheumatoid arthritis (RA) patients may have a higher risk to develop periodontitis (PD) than healthy controls, but the results are inconsistent. The aim of this study was to assess the prevalence and severity of periodontal disease among Jordanian patients with RA in comparison with RA free control group. A case control study from Jordan included 202 subjects, 102 with RA and 100 without RA (controls). Patients were enrolled from rheumatology clinic and emergency clinic at King Abdullah University Hospital in Jordan during a 6-month period. The plaque index (PI), gingival index (GI), probing pocket depth (PPD), bleeding on probing (BOP), and clinical attachment loss (CAL) were performed. In addition, the number of decayed, missing, and filled teeth (DMFT) was measured. A questionnaire focusing on potential risk factors for periodontal disease such as smoking, education, alcohol consumption, and body mass index (BMI), as well as chronic diseases associated with RA and periodontal disease was assessed. Findings of RA patients and control group were analyzed using one-way ANOVA, post hoc tests, logistic regression analysis and chi-squared tests. About 63.7% of RA patients and 30.0% of non-RA group had PD (p < 0.005). In multivariate analysis, after adjustment of age, gender and number of missing teeth, subjects with RA (OR = 7.14, 95% CI: 3.38, 15.15) were found to have a significantly higher odds of PD compared to control group. The severity and extent of periodontal disease were found to be significantly higher in RA patients compared to controls (P < 0.05) when measured by percentage of sites with PPD ≥ 3 mm, PPD ≥ 4 mm, CAL ≥ 3, CAL ≥ 4, and bleeding on probing. In addition, DMFT was higher in subjects with RA as compared to non-RA (p = 0.048). RA subjects had increased prevalence of having periodontitis compared to controls. Therefore, interventions to prevent, minimize or treat periodontitis may improve the health status of RA patients.
