BACTERIUM alcaligines (alcaligines faecalis) is usually a harmless saprophyte in the human intestinal tract. On occasion, however, it may invade the tissues of man and produce a wide variety of clinical syndromes; this paper is concerned with an infection of this type that occurred in a patient who had infectious mononucleosis, and with a review of the literature on this subject. Bact. alcaligines is a gram-negative rod that shows considerable variation in cell size but is usually longer and thinner than Escherichia coli. Alcaligenes faecalis (Bacterium alcaligenes) is usually considered a harmless saprophyte in the human intestinal tract. It seems to be well established, however, that the organism can be pathogenic.1,2Human infection in a number of cases with the organism has been recorded, and the clinical picture has varied, depending upon the organ involved.3-9,12-40While A. faecalis has not been frequently encountered as a cause of disease in the newborn, there have been reports of meningitis in a 9-day-old premature twin who was treated with chloramphenicol and recovered4; three cases of fatal meningitis, two of which were associated with bacteremia in newborn infants5,6; three children with diarrhea and grossly bloody stools, one of whom died,7and one case of bilateral conjunctivitis in a 2-week-old infant.8 Infection with A. faecalis in infants is sufficiently rare to warrant reporting its occurrence; its isolation as the causative organism in

INTRODUCTION With the development in recent years of a variety of sulfonamide compounds1and specific bactericidal and antitoxic serums2for the treatment of infections of the eye, the etiologic diagnosis of diseases of the conjunctiva and cornea has become an essential and practical procedure. hole into the sub-retinal space, as it was uninterruptedly doing before the operation was performed. Gonin's operation, therefore, seems to nme to achieve success by excluding \" the hole in the retina, to use the term which we apply to pupillary adhesions, not to \" occlusion.\" In the cured case the position of the retinal hole will correspond with the organized choroidal scar which can be recognized with the ophthalmoscope. It would appear to be necessary to determine as accurately as possible before operating in each case to what part of the choroid the retinal hole will correspond when the sub-retinal fluid flows out, and to puncture with the cautery at this point in the overlying sclerotic; the height of the detachment in the region of the hole will presumably be a factor in determining the spot selected for the puncture; the rather free use of a large cautery would increase the chance of success. A clear apprehension of the modus operandi of Gonin's operation is desirable, and clear thinking may promote further advance in this method of treatment of detachment of retina, which has already achieved so much success. The expression \" sealing the hole \" seems inappropriate and misleading: \" exclusion of the hole \" by ring synechia between retina and choroid is perhaps the phrase which will best convey the state of affairs to the ophthalmic surgeon; in using the term he will not conceive of the annular attachment as merely linear at the extreme margins of the hole; a broader area of the circumference is no doubt adherent to the choroidal scar; the surgeon will not need to be reminded that in the presence of the detachment the direction of the current of fluid is from the intra-ocular to the sub-retinal space.

An epidemic of conjunctivitis due to infection with bacillus faecalis alkaligenes.

PRIMARY melano-carcinoma of the ciliary body is a very rare disease, and hitherto only a few cases have been reported in the literature. On January 16, 1931, H.S., a blacksmith, aged 61 years, presented himself for examination on account of defective right vision. For a month he had noticed that the right vision was hazy, and had become steadily worse. Two years ago his visual acuity was recorded as follows:-Right vision with + 0 5 sph. and + 0 5 cvl. 180° = 6/6, left vision with + 10 sph. and + 1.0 cyl. + 180Q = 6/9. The fundi were normal at this time. Fig. 1 is a diagram representing the clinical appearance of a swelling situated on the iris and in the filtration angle at six o'clock. The summit of the swelling impinged upon the posterior aspect of the cornea, and the greater part of it was of a greyish brown colour, except in the filtration angle where it had a \" pearly \" appearance with two small vessels running over its anterior surface. Some of the anterior ciliary vessels at six o'clock were dilated and engorged with blood. The swelling did not extend to the free margin of the iris. The reaction of the pupil to direct light was sluggish, but the consensual reaction brisk. The pupil dilated well with homatropine, and it was impossible to detect by clinical methods any neoplasm in the ciliary body. The lens was opaque in the vicinity of the neoplasm, and the opacity had spread upwards to the pupillary area. The intra-ocular pressure was normal and there was no evidence of an inflammatory lesion in the eye. A general examination failed to reveal a neoplasm elsewhere in the body. The patient was in good health and had no syinptoms or loss of weight. The Wassermann reaction was negative. He was admitted to the Moorfields Eye Hospital, and a clinical diagnosis of sarcoma of the ciliary body and iris was agreed upon. Excision was advised and performed.
