Many types of action can be taken in favour of the conservation and sustainable use of medicinal plants. Some of these are undertaken directly at the places where the plants are found, while others are less direct, such as some of those relating to commercial systems, ex situ conservation and bioprospecting. In the latter cases, actions taken will not lead to in situ conservation unless they feedback to improvements in the field. Probably the single most important role for medicinal plants in biological conservation is their ‘use’ to achieve conservation of natural habitats more generally. This stems from the special meanings that medicinal plants have to people, related to the major contributions that they make to many people's lives in terms of health support, financial income, cultural identity and livelihood security. Problems associated with biopiracy or (the other side of the coin) excessive restrictions on research have come to assume policy prominence in the general thematic area of ‘medicinal plant conservation and use’. The fair and equitable sharing of benefits from bioprospecting is required under the Convention on Biological Diversity, but it is not always easy to achieve these ideals in practice. While experience is accumulated in how this may practically be achieved, it is important, at the present time, that controls imposed on scientific research to prevent biopiracy or theft of local and indigenous intellectual property do not unduly restrict research that has little or nothing to do with these matters.

Medicinal plants, conservation and livelihoods

Plant species have long been used as principal ingredients of traditional medicine in far-west Nepal. The medicinal plants with ethnomedicinal values are currently being screened for their therapeutic potential but their data and information are inadequately compared and analyzed with the Ayurveda and the phytochemical findings. The present study evaluated ethnomedicinal plants and their uses following literature review, comparison, field observations, and analysis. Comparison was made against earlier standard literature of medicinal plants and ethnomedicine of the same area, the common uses of the Ayurveda and the latest common phytochemical findings. The field study for primary data collection was carried out from 2006-2008. The herbal medicine in far-west Nepal is the basis of treatment of most illness through traditional knowledge. The medicine is made available via ancient, natural health care practices such as tribal lore, home herbal remedy, and the Baidhya, Ayurveda and Amchi systems. The traditional herbal medicine has not only survived but also thrived in the trans-cultural environment with its intermixture of ethnic traditions and beliefs. The present assessment showed that traditional herbal medicine has flourished in rural areas where modern medicine is parsimoniously accessed because of the high cost and long travel time to health center. Of the 48 Nepalese medicinal plants assessed in the present communication, about half of the species showed affinity with the common uses of the Ayurveda, earlier studies and the latest phytochemical findings. The folk uses of Acacia catechu for cold and cough, Aconitum spicatum as an analgesic, Aesculus indica for joint pain, Andrographis paniculata for fever, Anisomeles indica for urinary affections, Azadirachta indica for fever, Euphorbia hirta for asthma, Taxus wallichiana for tumor control, and Tinospora sinensis for diabetes are consistent with the latest pharmacological findings, common Ayurvedic and earlier uses. Although traditional herbal medicine is only a primary means of health care in far-west Nepal, the medicine has been pursued indigenously with complementing pharmacology and the Ayurveda. Therefore, further pharmacological evaluation of traditional herbal medicine deserves more attention.

Ethnobotanical study of medicinal plants from the Humla district of western Nepal.

Ethnopharmacological knowledge is common and import among tribal populations but much of the information is empirical at best lacking scientific validation. Despite widespread use of plant resources in traditional medicines, bioassay analysis of very few plant species have been conducted to investigate their medicinal properties, and to ascertain safety and efficacy of traditional remedies. The present study analyses indigenous uses of medicinal plants of far-west Nepal and compares with earlier ayurveda studies, phytochemical assessments and pharmacological actions. A field study was carried out in Baitadi and Darchula districts of far-west Nepal. Group discussions, informal meetings, questionnaire surveys and field observations were employed for primary data collection. Voucher specimens were collected with field notes and codes and deposited at Tribhuvan University Central Herbarium (TUCH), Kathmandu. Only 50% of species surveyed shared common uses with ayurvedic medicine. This implies that these herbal remedies are part of an independent health care system in the Nepal Himalaya, which is indigenous and influenced by ayurveda. The folk uses of some of the species were contradicting to those of ayurveda and phytochemical bioassays. A detailed phytochemical study on those species would be an important line of research.

Indigenous Use and Ethnopharmacology of Medicinal Plants in Far-West Nepal

Traditional plant use in Nepal has been documented for millennia. The importance of plants as medicine has not diminished in any way in recent times, and traditional medicines are still the most important health care source for the vast majority of the population. This paper examines the ethnobotany and traditional use of plants extracted from the vulnerable alpine zone in the Dolpa, Humla, Jumla and Mustang districts of Nepal. The results of this ethnobotanical study indicate that a very large number of plant species is used as traditional medicines. There were 107, 59, 44 and 166 species of ethnomedicinal importance in surveyed areas of Dolpa, Humla, Jumla and Mustang district respectively. Of these, 84 common species, used at least in two districts, were selected to enumerate their ethnomedicinal properties. The 84 species belonged to 75 genera and 39 families. The commonest species in this pharmacopoeia were: Allium wallichii, Cordyceps sinensis, Dactylorhiza hatagirea, and Rheum australe. A total of 21 species were most common in three districts and 59 in two districts. The genera Aconitum, Allium, Arisaema, Berberis, Corydalis, Gentiana, Hippophae, Juniperus and Rhododendron each possessed two species with ethnomedicinal use. Labiatae was the most medicinally important family with five species used, followed by Araceae, Compositae, Liliaceae, Polygonaceae, Ranunculaceae, Scrophulariaceae and Umbelliferae, each contributing four species.

Background . Chronic low back pain (CLBP) is a clinically common and expensive disease. Patients frequently take sick leaves because of pain and dysfunction, and their unpleasant life and work experiences cause psychological depression and anxiety and affect their quality of life. Sleep disturbance is a common problem among patients with low back pain (LBP) with more than 50% complaining about poor sleep quality. This study aimed to explore the correlations between anxiety, sleep quality, and pressure-pain threshold (PPT) and their differences between patients with CLBP and asymptomatic people. Methods . Forty patients with CLBP and 40 asymptomatic people were recruited. Relevant data, including State-Trait Anxiety Inventory, Pittsburgh Sleep Quality Index, and PPT, were individually and independently collected by blinded physiotherapists with a practicing certificate and then statistically analyzed. An independent sample t -test was used to determine the intergroup differences between patients with CLBP and asymptomatic populations. Pearson correlation coefficient was employed for correlation analysis. Results . The CLBP group had significantly higher anxiety scores (41.64 ± 9.88 vs. 36.69 ± 8.31; t (cid:31) − 2.496, p (cid:31) 0 . 015) than the asymptomatic group. A significant difference was found in the total score of the Pittsburgh Sleep Quality Index (6.41 ± 2.43 vs. 5.09 ± 2.18; t (cid:31) − 2.628, p (cid:31) 0 . 010) but not in the trait anxiety (44.00 ± 7.83 vs. 42.67 ± 9.51; t (cid:31) − 0.695, p (cid:31) 0 . 489) of the two groups. State − Trait Anxiety Inventory showed a low to moderate negative correlation with PPT. No remarkable correlation was observed between Pittsburgh Sleep Quality Index and PPT. Conclusions . Patients with CLBP showed considerably worse state anxiety and sleep quality than asymptomatic people; however, no substantial difference in PPT was found between the two groups. The results suggest that in clinical practice, the focus should include pain and related social and psychological factors. CLBP treatment could be considered from multiple perspectives and disciplines.This trial is registered with Chinese Clinical Trial Registry (Trial registration: ChiCTR-TRC-13003701).
