Febrile urinary tract infections: pyelonephritis and urosepsis.

Introduction: Emphysematous pyelonephritis (EPN) is associated with high mortality rate, up to 25%. There is still conflicting reports regarding the most appropriate management, conservative versus nephrectomy.
Objective: To describe the outcome of patients with EPN.
Methods: We retrospectively reviewed the medical records of patients diagnosed with EPN by computed tomography from three tertiary institutes in Jeddah, Saudi Arabia. Type of management was classified as conservative and surgical. The conservative includes medical and minimally invasive procedures, such as percutaneous drainage and nephrostomy. The surgical which is nephrectomy. The outcome observed was preservation of the kidney function or patient's life.
Results: A total of 10 patients were included (9 females and 1 male), median age was 55 years and 63% were diabetic. The most common presentation was flank pain (100%), fever (75%), and vomiting (63%). The most common organism isolated was Escherichia coli. Nephrectomy was not associated with increased survival rate, while conservative management was associated with a good outcome, less morbidity (not dialysis-dependent).
Conclusion: Nephrectomy was not associated with high survival rate. Patients managed conservatively had a better overall performance and better survival. This study will add to other studies, which are encouraging conservative management.

Emphysematous pyelonephritis: Is nephrectomy warranted?

Emphysematous pyelonephritis (EPN) is a necrotizing infection characterized by the production of gas in the renal parenchyma, collecting system or perirenal tissue. Meanwhile, emphysematous cystitis (EC) is a clinical entity characterized by the presence of gas inside and around the bladder wall. Interestingly, although both diseases are common in patients with diabetes mellitus, these are rarely combined. We report a rare case of a 56-year-old diabetic male suffering from fever, headache and vomiting and in which a diagnosis of septic shock was established due to coexistence of EC and bilateral EPN. The emphysematous diseases improved with a conservative treatment approach using antibiotic therapy and glycemic control, we highlight that the nephrectomy was not necessary in our patient despite the fact that he presented risk factors that predict the failure of conservative treatment.

Coexistence of emphysematous cystitis and bilateral emphysematous pyelonephritis: a case report and review of the literature.

Context: Emphysematous pyelonephritis (EPN) is an acute necrotizing inflammation of renal parenchyma and peri-nephric tissue characterized by gas formation. In this study, we evaluated the outcome of EPN cases by medical management with or without renal decompression. Materials and Methods: We have observed prospectively 15 cases of EPN admitted in our institute in respect to age, sex, mode of presentation, presence of comorbidity, laboratory profile, urine-analysis, and outcome after medical management and drainage procedures. Results: All patients improved on medical management and drainage procedure such as JJ stenting and percutaneous nephrostomy. No mortality noted in our 15 cases of EPN. Conclusion: Emphysematous pyelonephritis can be managed conservatively by DJ stenting, wcich is a less morbid procedure or percutaneous nephrostomy with proper antibiotics and adequate glycemic control.

Double J stenting: A rewarding option in the management of emphysematous pyelonephritis.

Background Indiscriminate use of antibiotics in urinary tract infections have led to emergence of 'superbugs' worldwide. Periodical review of antibiograms is of utmost importance for optimum patient benefit. Objective To identify the spectrum of organisms responsible for urinary tract infection and evaluate the pattern of antibiotic sensitivity of the organisms. Methods Urine samples were collected from all consecutive patients getting admitted in the medicine indoor irrespective of symptomatology. The clean-catch technique of midstream urine was used for patients able to void spontaneously while specimens of catheterized patients were obtained prior to catheter change. All samples were sent for routine examination as well as culture sensitivity tests. Descriptive statistical methods were used with the help of SPSS-15th version. Results The total number of patients was 262, 160 (60.3%) were male and 102 (39.7%) female. The age of the subjects varied from 18 to 85 years. Significant bacteruria was found in 35.9% patients. The commonest organisms isolated overall were E coli (59.6%) followed by enterococcus spp (14.9%) and Klebsiella (10.6%). Among catheterized subjects apart from E coli (64%), Klebsiella was found to be commoner (12%) than Enterococcus 10%. Most Enterobactereacae showed good response to aminoglycosides, cephoperazone sulbactam and nitrofurantoin. However, organisms like Enterobacter, Citrobacter, Morganella and pseudomonas were poorly responsive to the above and required higher antibiotics like carbapenems, and polymixin B. Staph. aureus was responsive to linezolid and vancomycin only. Conclusions The progression of resistance of common urinary pathogens to higher antibiotics is an inexorable process. Our study reveals that new generation cephalosporins, quinolones and macrolides cannot keep up with the rapidity of emergence of multiresistant strains.

Pattern of Urinary Antibiograms in a Tertiary Care Hospital of Eastern India.

Emphysematous pyelonephritis (EPN) is a group of potentially life-threatening conditions seen particularly in diabetics, leading to high morbidity and mortality. Our aim was to study the profile of emphysematous infections of the kidney and urinary tract and evaluate the effect of early surgical intervention on mortality. This is an observational study conducted in a tertiary care hospital between January 2009 and January 2013, in which the clinical, laboratory, microbiological and radiological profiles of patients with EPN (diagnosed based on clinical, laboratory and imaging findings) was studied. A total of 12 patients were studied, including 10 with diabetes. A total of 66.6% had pyelonephritis, 25% had both cystitis and pyelonephritis and 8.3% had only cystitis; involvement of the left kidney was more common and bilateral involvement was seen in two cases. The clinical features seen in the patients included fever (100%), features of urinary infection (66.6%) and flank pain (50%). Culture positivity was seen in only 50% of the cases. Ten patients underwent percutaneous drainage (PCD) within 24 h, and two of these patients required nephrectomy subsequently. All patients were followed-up for one month. There was one death (mortality 8.3%), and all other patients responded well and reco­vered. Our study suggests that EPN is a potentially life-threatening condition that requires aggressive and prompt medical therapy with early PCD to reduce morbidity and mortality. Nephrectomy should be reserved for cases that do not respond to PCD.
