PURPOSE: to characterize the population submitted to administration of alternative feeding route admitted to a general hospital in the state of Alagoas METHOD: data from medical records of 229 patients admitted to adult infirmary were investigated Data referring to the administration of alternative feeding route in use and clinical aspects related to swallowing disorders were analyzed RESULTS: there was a predominance of male participants (5502%) in the sample From the total sample, 703% of participants were diagnosed with a neurological disease and most of them used the nasogastric tube (8253%) as feeding route There was an association between the variables pneumonia and dysphagia (p = 00098) However, no association between malnutrition and dysphagia was found (p = 00759) There was also a high frequency of absence of data concerning symptoms of feeding difficulty as well as about justification for the use of the alternative feeding route CONCLUSION: the population studied presented risk factors for development of dysphagia The shortage of data revealed the little importance given to functional manifestation at the hospital context

Perfil de pacientes em uso de via alternativa de alimentação internados em um hospital geral

Percutaneous endoscopic gastrostomy is an alternative way to administer enteral diets. It is a procedure requiring no anesthetic or abdominal incision as required in open surgical gastrostomy. The main objective of percutaneous endoscopic gastrostomy is to prevent deterioration of the nutritional status and improve the quality of life and it is now the method of choice when the patient requires an enteral diet for more than a month. Complications occur in 1.0% to10.0% of the cases and mortality in 0.3% to 1.0%. This paper deals with aspects relevant to this method of enteral feeding, how to improve handling procedures and deal with the patients who use it, commenting on the indications and contraindications for its use, placement techniques, materials used for the probes, prophylactic antibiotics, start-up of its use and costs, in addition to discussing some of its complications and legal and ethical aspects.

Frequência de eventos adversos gastroenterológicos em pacientes com terapia nutricional enteral no Hospital de Clínicas de Porto Alegre (HCPA)

Pacientes cardiopatas complicados com isquemia cerebral e insuficiencia cardiaca necessitam terapia nutricional enteral prolongada por se apresentarem em situacao de risco nutricional. Complicacoes advindas do uso prolongado da sonda nasoenteral poderiam ser evitadas com a execucao da gastrostomia endoscopica. Esta e alternativa tecnica para acesso ao tubo digestivo em nutricao enteral. O objetivo deste estudo foi avaliar o tempo de indicacao para gastrostomia endoscopica em pacientes com nutricao enteral com uso de sonda nasoenteral e as principais complicacoes do procedimento. Doze pacientes foram submetidos a gastrostomia endoscopica, sendo oito do sexo masculino. A idade media foi de 62,42 + 22,10 anos. A indicacao principal foi sindrome isquemica cerebral apos parada cardiorespiratoria por patologia clinica (sete pacientes) ou cirurgia cardiovascular complicada (cinco). O tempo medio para indicacao da gastrostomia endoscopica foi de 35,58 + 26,79 dias apos introducao da nutricao enteral. Nao houve intercorrencias com o procedimento e, no periodo pos-operatorio tardio, ocorreu infeccao do orificio de insercao da gastrostomia endoscopica em um caso, resolvida com cuidados locais. Em conclusao, a gastrostomia endoscopica e tecnica segura, com baixa incidencia de complicacoes. Pode ser realizada em ambulatorio, no leito do paciente, ou em centro de terapia intensiva e sua indicacao deveria ser mais precoce.
