Hintergrund Die chronische Rhinosinusitis (CRS) betrifft weltweit ca. 5–12 % der Allgemeinbevolkerung und wird traditionell eingeteilt in einen Phanotyp ohne (CRSsNP) und einen mit Ausbildung von Nasenpolypen (CRSwNP). Wurden Nasenpolypen bis vor kurzem eher als mechanisches Hindernis mit der Notwendigkeit einer operativen Beseitigung betrachtet, wird die CRSwNP heute als eine multifaktorielle entzundliche Erkrankung der nasalen und paranasalen Schleimhaute angesehen, der als Endotyp haufig eine T2-Inflammation zugrunde liegt. Biologika, die mit diesen Entzundungsmechanismen interferieren, stellen interessante neue Therapiemoglichkeiten dar. Methoden Das aktuelle Wissen zur Immunologie der CRSwNP und Wirkung von Biologika wurde mittels einer Literaturanalyse durch Recherchen in Medline, PubMed sowie den nationalen und internationalen Studien- und Leitlinienregistern und der Cochrane Library zusammengestellt. Ergebnisse Basierend auf der internationalen Literatur und bisherigen Erfahrungen werden von einem Expertengremium Empfehlungen fur die Anwendung von Biologika bei CRSwNP im deutschen Gesundheitssystem auf der Grundlage eines Dokumentationsbogens gegeben. Schlussfolgerung Das Verstandnis uber die immunologischen Grundlagen der CRSwNP eroffnet neue nichtoperative Therapieansatze mit Biologika fur Patienten mit schweren Verlaufsformen.

Summary Background Interferon‐alpha is an important therapeutic option for the treatment of the cutaneous T‐cell lymphomas (CTCL). Since the approved recombinant interferon‐α‐2a (IFN‐α2a) has no longer been produced since January 2020, pegylated interferon‐α2a (pegIFN‐α2a) can be used as an alternative treatment, even though it is not approved for the treatment of CTCL. The aim of this multicentre study was to generate comprehensive data on the efficacy and tolerability of pegIFN‐α2a in the treatment of CTCL. Patients and Methods A multicenter retrospective study was conducted with 70 patients with CTCL from twelve German skin centers. Results In total, 70 patients were included in the study, with 57.2% male and a mean age of 58.8 ± 14.9 years. Mycosis fungoides was present in 71.4% of cases and Sézary Syndrome in 28.6%. An overall response rate of 55.2% was observed with pegIFNα‐2a therapy. In 50% of cases, therapy was discontinued after 63.6 ± 33.5 weeks. The most common reason for discontinuation was adverse events, which occurred in 68.6% of cases and which were classified as severe in 29.2%. Blood count changes, fatigue and liver toxicity occurred most frequently. Conclusions Our analysis provides comprehensive data on the efficacy and tolerability of pegIFNα‐2a therapy in patients with CTCL. In terms of response rates and side effect profile, pegIFNα‐2a appears to be comparable to IFN‐α2a therapy. 

Pegylated interferon‐α2a in cutaneous T‐cell lymphoma – a multicenter retrospective data analysis with 70 patients

Sézary syndrome (SS) is an aggressive variant of cutaneous t-cell lymphoma characterized by the accumulation of neoplastic CD4+ lymphocytes-the SS cells-mainly in blood, lymph nodes, and skin. The tumor spread pattern of SS makes this lymphoma a unique model of disease that allows a concurrent blood and skin sampling for analysis. This review summarizes the recent studies highlighting the transcriptional programs triggered by the crosstalk between SS cells and blood-skin microenvironments. Emerging data proved that skin-derived SS cells show consistently higher activation/proliferation rates, mainly driven by T-cell receptor signaling with respect to matched blood SS cells that instead appear quiescent. Biochemical analyses also demonstrated an hyperactivation of PI3K/AKT/mTOR, a targetable pathway by multiple inhibitors currently in clinical trials, in skin SS cells compared with a paired blood counterpart. These results indicated that active and quiescent SS cells coexist in this lymphoma, and that they could be respectively treated with different therapeutics. Finally, this review underlines the more recent discoveries into the heterogeneity of circulating SS cells, highlighting a series of novel markers that could improve the diagnosis and that represent novel therapeutic targets (GPR15, PTPN13, KLRB1, and ITGB1) as well as new genetic markers (PD-1 and CD39) able to stratify SS patients for disease aggressiveness.

Aktuelle Therapieoptionen bei Mycosis fungoides und Sézary-Syndrom

Zusammenfassung Hintergrund Interferon‐α2a ist eine wichtige Therapieoption in der Behandlung kutaner T‐Zell‐Lymphome (CTCL). Seit Januar 2020 wird das zur Therapie zugelassene rekombinante Interferon‐α2a (IFN‐α2a) nicht mehr produziert. Als empfohlene Alternative steht pegyliertes Interferon‐α2a (pegIFN‐α2a) im off‐label use zur Verfügung. Das Ziel der vorliegenden Arbeit ist die Generierung umfangreicher Daten hinsichtlich Wirksamkeit und Verträglichkeit von pegIFN‐α2a für die Behandlung von CTCL. Patienten und Methodik Es erfolgte eine multizentrische, retrospektive Datenanalyse von 70 Patienten mit CTCL aus zwölf deutschen Hautkliniken. Ergebnisse Von den 70 eingeschlossenen Patienten waren 57,2% männlich, das mittlere Alter betrug 58,8 ± 14,9 Jahre. In 71,4% der Fälle bestand eine Mycosis fungoides und in 28,6% ein Sézary‐Syndrom. Es wurde ein Gesamtansprechen von 55,2% unter Therapie mit pegIFN‐α2a beobachtet. Die Therapie wurde in 50% der Fälle nach 63,6 ± 33,5 Wochen beendet. Der häufigste Grund hierfür waren Nebenwirkungen, die in 68,6% der Fälle auftraten und in 29,2% als schwer klassifiziert wurden. Am häufigsten traten Blutbildveränderungen, Fatigue und Lebertoxizität auf. Schlussfolgerungen Unsere Analyse liefert umfangreiche Daten zur Wirksamkeit und Verträglichkeit von pegIFN‐α2a bei Patienten mit CTCL. Hinsichtlich der Ansprechraten und des Nebenwirkungsprofils scheint pegIFN‐α2a eine effektive Alternative für IFN‐α2a im Rahmen der Therapie der CTCL zu sein. 

Pegyliertes Interferon‐α2a bei kutanen T‐Zell‐Lymphomen – eine multizentrische retrospektive Datenanalyse mit 70 Patienten

Die Mycosis fungoides (MF) und das Sezary-Syndrom (SS) sind primar kutane T-Zell-Lymphome (CTCL), deren Atiologie und Pathogenese noch nicht vollstandig geklart sind. MF und SS werden konzeptionell als eigenstandige Entitaten angesehen, mit unterschiedlichen T-Helferzell-Subtypen als Ursprung. Wahrend die MF den haufigsten Subtyp der CTCL darstellt, ist das SS hingegen sehr selten. Klinisch werden bei der MF das Patch-, Plaque- und Tumor-Stadium unterschieden, wobei sich auch eine Erythrodermie manifestieren kann, welche wiederum pathognomonisch fur das SS ist. Das SS ist durch eine Leukamie und/oder eine nachweisbare Tumorzelllast (Sezary-Zellen) im peripheren Blut gekennzeichnet und per se ein fortgeschrittenes Krankheitsstadium mit ungunstiger Prognose. Bei der MF ist die Prognose im fruhen Krankheitsstadium, der uberwiegenden Verlaufsform, vorteilhaft. Bei bis zu 30 % der Patienten ist jedoch ein Fortschreiten der kutanen Erkrankung bis hin zu extrakutanen Manifestationen dokumentiert. Die histologischen Veranderungen der MF sind im Fruhstadium oft diskret und daher ist eine sorgfaltige klinisch-pathologische Korrelation essenziell. Die Behandlung von MF und SS erfolgt stadiengerecht mit topischen und/oder systemischen Therapieschemata. Abseits der allogenen Stammzelltransplantation (alloSZT) gibt es bisher keine kurative Therapie fur MF/SS. Dementsprechend ist der Behandlungsansatz symptomorientiert und zielt auf eine Tumorlastreduktion und eine Verbesserung der gesundheitsbezogenen Lebensqualitat ab. Die aktuelle Therapielandschaft fur CTCL wird permanent durch die Entdeckung neuer therapeutischer Zielstrukturen erweitert.
