Reference values are used to describe the dispersion of variables in healthy individuals. They are usually reported as population-based reference intervals (RIs) comprising 95% of the healthy population. International recommendations
state the preferred method as a priori nonparametric determination from at least 120 reference individuals, but acceptable alternative methods include transference or validation from previously established RIs. The most critical steps in the determination of reference values are the selection of reference individuals based on extensively documented inclusion and exclusion criteria and the use of quality-controlled analytical procedures. When only small numbers of values are available, RIs can be estimated by
new methods, but reference limits thus obtained may be highly imprecise. These recommendations are a challenge in veterinary clinical pathology, especially when only small numbers of reference individuals are available.

Reference values: a review.

RA is a complex disease that develops as a series of events often referred to as disease continuum. RA would benefit from novel biomarker development for diagnosis where new biomarkers are still needed (even if progresses have been made with the inclusion of ACPA into the ACR/EULAR 2010 diagnostic criteria) and for prognostic notably in at risk of evolution patients with autoantibody-positive arthralgia. Risk biomarkers for rapid evolution or cardiovascular complications are also highly desirable. Monitoring biomarkers would be useful in predicting relapse. Finally, predictive biomarkers for therapy outcome would allow tailoring therapy to the individual. Increasing numbers of cytokines have been involved in RA pathology. Many have the potential as biomarkers in RA especially as their clinical utility is already established in other diseases and could be easily transferable to rheumatology. We will review the current knowledge’s relation to cytokine used as biomarker in RA. However, given the complexity and heterogeneous nature of RA, it is unlikely that a single cytokine may provide sufficient discrimination; therefore multiple biomarker signatures may represent more realistic approach for the future of personalised medicine in RA.

Heterophilic antibody interference in immunometric assays

New facts have recently enhanced interest in the topic of reference intervals. In particular, the International Organization for Standardization standard 15189, requesting that 'biological reference intervals shall be periodically reviewed', and the directive of the European Union on in vitro diagnostic medical devices asking manufacturers to provide detailed information on reference intervals, have renewed interest in the subject. This review presents an update on the topic, discussing the theoretical aspects and the most critical issues. The basic approach to the definition of reference intervals proposed in the original International Federation of Clinical Chemistry documents still remain valid. The use of data mining to obtain reference data from existing databases has severe limitations. New statistical approaches to discard outliers and to compute reference limits have been recommended. On the other hand, perspectives opened by the improvement in standardization through the implementation of the concept of traceability suggest new models to define 'common' reference intervals that can be transferred and adopted by different clinical laboratories in order to decrease the proliferation of different reference intervals not always justified by differences in population characteristics or in analytical methodology.

Reference intervals: the way forward

Background Heterophilic antibodies are still an important source of interference in immunoassays. We have conducted a screening study for interference in a panel of commercially available assays using two sera known to contain high titer Fc-reactive heterophilic antibodies. Methods The sera were distributed to laboratories participating in the Nordic External Quality Assessment cooperation (EQANord). Duplicate samples pre-blocked with aggregated murine monoclonal MAK33 were also supplied. Discrepancies (>50%) between the results for native and blocked samples were used to classify the tested assays as susceptible to interference. A total of 170 different assay kits covering 91 analytes were tested. Results We found that 21 assays, covering 19 different analytes, were susceptible to interference from the heterophilic antibodies in the two sera. Many of these are clinically and commercially important assays. Some of the false results were grossly elevated and could have been detrimental to patient care in a clinical setting. Conclusions Heterophilic antibodies with Fc-reactivity remain a threat. A more widespread use of antibody fragments and aggregated immunoglobulin could potentially improve the heterophilic antibody resistance of assays intended for clinical use.

Heterophilic antibody interference in commercial immunoassays; a screening study using paired native and pre-blocked sera

Automation of reticulocyte counting has decreased the analytical imprecision of this parameter. This has made it possible to use the number of reticulocytes and its derived parameters (reticulocyte maturity and reticulocyte cell indices) in new diagnostic and monitoring situations. For rational use of these parameters, it is important to have knowledge of their biological variability. The biological variability of reticulocytes and its derived parameters was studied in 13 healthy people during a period of 7 wk on 2 different instruments. The within-subject coefficient of variation for the reticulocyte count was about 11%, for the mean reticulocyte volume, mean reticulocyte haemoglobin content and mean reticulocyte haemoglobin concentration it was between 1 and 2%, whereas the coefficient of variations for the subpopulations of reticulocytes with different maturity varied depending on the method used for the measurements. The critical difference, that is the change in a result making it significantly different from the previous result, was about 35% for the reticulocyte count and 5-8% for the reticulocyte cell indices, making these indices excellent to follow changes in erythropoiesis. With a possible exception for the mean reticulocyte volume, the within-subject variation was small compared to the between-subject variation.

Within‐subject biological variation of reticulocytes and reticulocyte‐derived parameters

Background Some clinical chemistry measurement methods are vulnerable to interference if hemolyzed serum samples are used. The aims of this study were: (1) to obtain updated information about how hemolysis affects clinical chemistry test results on different instrument platforms used in Nordic laboratories, and (2) to obtain data on how test results from hemolyzed samples are reported in Nordic laboratories. Methods Four identical samples containing different degrees of hemolysis were prepared and distributed to 145 laboratories in the Nordic countries. The laboratories were asked to measure the concentration of cell-free hemoglobin (Hb), together with 15 clinical chemistry analytes. In addition, the laboratories completed a questionnaire about how hemolyzed samples are handled and reported. Results Automated detection of hemolysis in all routine patient samples was used by 63% of laboratories, and 88% had written procedures on how to handle hemolyzed samples. The different instrument platforms measured comparable mean Hb concentrations in the four samples. For most analytes, hemolysis caused a homogenous degree of interference regardless of the instrument platform used, except for alkaline phosphatase (ALP), bilirubin (total) and creatine kinase (CK). The recommended cut-off points for rejection of a result varied substantially between the manufacturers. The laboratories differed in how they reported test results, even when they used the same type of instrument. Conclusions Most of the analytes were homogeneously affected by hemolysis, regardless of the instrument used. There is large variation, however, between the laboratories on how they report test results from hemolyzed samples, even when they use the same type of instrument.
