Die Bandverletzung des oberen Sprunggelenks ist eine der haufigsten Verletzungen im Sport. Landung auf dem gegnerischen Fus oder direkter Kontakt gehoren zu den haufigsten Verletzungsmechanismen. Initial zahlt vor allem die klinische Diagnostik, je nach Bedarf um Rontgen, Sonografie oder MRT erganzt. Meist wird konservativ therapiert; zunachst mussen Schmerz und Schwellung reduziert, dann die Funktion wiederhergestellt werden, bevor nach Return-to-Play-Testung in den Sport zuruckgekehrt werden kann. Teilweise liegen komplexere Verletzungsmuster mit Begleitschaden vor und trotz aller Erfolge der konservativen Behandlung bleiben bestimmte Falle dem operativen Vorgehen vorbehalten. Nicht nur die reelle Ausfallzeit der Athleten, sondern die Auspragung von Folgeschaden und chronischen Funktionseinschrankungen und auch die damit verbundenen Behandlungskosten legen die Empfehlung nahe, eine Sprunggelenkverletzung nicht zu unterschatzen und die Therapie adaquat auszuwahlen.

Ankle impingement refers to a chronic painful mechanical limitation of ankle motion caused by soft tissue or osseous abnormality affecting the anterior or posterior tibiotalar joint. Impingement can be associated with a single traumatic event or repetitive microtrauma. These syndromes are a possible etiology of persistent ankle pain. An arthroscopic approach to this pathology, when indicated, is considered as ideal treatment with its high safety and low complication rate. We describe the clinical and potential imaging features, and the arthroscopic/endoscopic management strategies, for the 4 main impingement syndromes of the ankle: anterolateral, anterior, antero-medial, and posterior.

Anterior and Posterior Ankle Impingement Syndromes: Arthroscopic and Endoscopic Anatomy and Approaches to Treatment.

Background/Objectives: To assess magnetic resonance image (MRI) findings in children and adolescents with atraumatic non-overload ankle pain and to identify potential anatomic risk factors. Methods: In total, 310 MRIs of 6- to 20-year-old patients were evaluated regarding detectable ankle pathologies. A total of 147 patients (68 males; 79 females) suffered from atraumatic non-overload ankle pain. The findings were compared to a control group (163 patients: 89 males; 74 females), including patients with ankle trauma in the 4 weeks prior to MRI examination. A t-test for unpaired samples and a binary logistic regression model were used to identify significant differences between both groups and determine potential anatomic risk factors. Results: In the group with atraumatic ankle pain, 95 patients (64.6%) showed at least one pathology. Anterolateral impingement of the upper ankle joint was found in 29 patients (19.7%). Its occurrence was significantly higher in atraumatic non-overload patients than in the control group (p = 0.043). Moreover, a significant correlation between anterolateral impingement of the upper ankle and the presence of hindfoot valgus malposition (n = 25; 17.0%) could be proven in atraumatic non-overload patients (p = 0.035). Conclusions: Anterolateral impingement of the upper ankle joint is frequently observed in children and adolescents suffering from atraumatic non-overload ankle pain, whereby a hindfoot valgus malposition seems to present an anatomic risk factor.

Das Impingementsyndrom am oberen Sprunggelenk (OSG) gehort zu den haufigsten chronischen als auch posttraumatischen Sprunggelenkpathologien. Die Einklemmungssymptomatik entsteht aufgrund des Zusammenstosens anatomischer Strukturen, welche zu Schmerzen und Einschrankung der Beweglichkeit im Bereich des OSG fuhren kann. Eingeteilt werden kann das OSG-Impingement anhand der Lokalisation oder auslosender Ursache. Klinisch imponieren neben chronischen Schmerzen die bewegungs- und belastungsabhangige Schwellung des Gelenks sowie die eingeschrankte Plantar- bzw. Dorsiflexion. Wahrend beim Weichteilimpingement die dazugehorige Klinik und die korperliche Untersuchung im Vordergrund stehen, spielen bildgebende Verfahren bei der Diagnose des knochernen Impingements eine grose Rolle. Aus therapeutischer Sicht sollte zunachst ein konservativer Behandlungsversuch erfolgen. Bei dessen Versagen ist die arthroskopische Resektion der zugrunde liegenden Ursache des Impingements die standardmasig angewendete Operationsmethode. Bei einer Komplikationsrate von derzeit etwa 3 % stellt diese eine sichere Operationsmethode dar, die sowohl mit hohen postoperativen Zufriedenheitsraten vonseiten der Patienten als auch signifikantem Beschwerderuckgang einhergeht.
