To decrease the influence of postural sway during spinal measurements, an instrumented fixation posture (called G) was proposed and tested in comparison with the free standing posture (A) using the DTP-3 system in a group of 70 healthy volunteers. The measurement was performed 5 times on each subject and each position was tested by a newly developed device for non-invasive spinal measurements called DTP-3 system. Changes in postural stability of the spinous processes for each subject/the whole group were evaluated by employing standard statistical tools. Posture G, when compared to posture A, reduced postural sway significantly in all spinous processes from C3 to L5 in both the mediolateral and anterioposterior directions. Posture G also significantly reduced postural sway in the vertical direction in 18 out of 22 spinous processes. Importantly, posture G did not significantly influence the spinal curvature.

Optimization of the examination posture in spinal curvature assessment

The main purpose of this research was to develop a new method for differentiating between scoliotic and healthy subjects by analysing the curvatures of their spines in the cranio-caudal view. The study included 247 subjects with physiological curvatures of the spine and 28 subjects with clinically confirmed scoliosis. The curvature of the spine was determined by a computer analysis of the surface of the back, measured with a non-invasive, 3D, laser-triangulation system. The determined spinal curve was represented in the transversal plane, which is perpendicular to the line segment that was defined by the initial point and the end point of the spinal curve. This was achieved using a rotation matrix. The distances between the extreme points in the antero-posterior (AP) and left-right (LR) views were calculated in relation to the length of the spine as well as the quotient of these two values LR/AP. All the measured parameters were compared between the scoliotic and control groups using the Student’s t-Test in case of normal data and Kruskal-Wallis test in case of non-normal data. Besides, a comprehensive diagram representing the distances between the extreme points in the AP and LR views was introduced, which clearly demonstrated the direction and the size of the thoracic and lumbar spinal curvatures for each individual subject. While the distances between the extreme points of the spine in the AP view were found to differ only slightly between the groups (p = 0.1), the distances between the LR extreme points were found to be significantly greater in the scoliosis group, compared to the control group (p < 0.001). The quotient LR/AP was statistically significantly different in both groups (p < 0.001). The main innovation of the presented method is the ability to differentiate a scoliotic subject from a healthy subject by assessing the curvature of the spine in the cranio-caudal view. Therefore, the proposed method could be useful for human posture diagnostics as well as to provide a long-term monitoring of scoliotic spine curvatures in preventive and curative clinical practice at all levels of health care.

Laser triangulation measurements of scoliotic spine curvatures

Background: A number of studies on diseased spine have been published; however, there is a relative paucity of studies investigating spine shape characteristics in healthy populations. Such characteristics are needed for diagnostics of spine disorders and assessment of changes in the spinal shape that may have been caused by influence of the modern life style or intensive sport activity. Objective: The aim of the study was to determine characteristics of the spine shape in a large sample of healthy young adults. Methods: Population cross-sectional study. A non-radiographic surface method (system DTP-3) was used for the assessment of spine shape in the sagittal and frontal planes. A total of 1,020 participants (440 men, 580 women) took part in the study, their mean (± SD) age was 21.8 ± 1.9 years (range 19.1-29.7) for men and 21.9 ± 1.8 years (range 19.3-29.7) for women. All data were checked for normality and are presented as means, standard deviations, ranges, skewness, and kurtosis. Differences between the sexes were assessed with the two-sample t-test. Results: The average sagittal spinal shape was C3 - 12.9° - C7 - 43.0° - T10 - 27.1° - L5 for men and C3 - 12.1° - C6 - 44.5° - T11 - 34.1° - L5 for women. Men showed a significantly smaller thoracic kyphosis and lumbar lordosis curvatures than women. The average curvature due to the lateral deviation in the frontal plane was 6.1° for both sexes, the curvature was larger than 10° in 9.1% of men and 8.8% of women. We found left lateral deviation in 72.5% of men and in 63.6% of women. Conclusions: The study provides characteristics of the spine shape in a large sample of healthy young adults. Such characteristics should be part and parcel of determining the cut-off level for physiological spinal shape. Based on the results of the study, we suggest a lateral deviation of 10° as the maximum for a curvature to be still considered non-pathological.

Purpose of the study Repeated measurements of the spine are absolutely necessary in children and adolescents affected by spinal deformities especially during their growing-up periods. To avoid risks of tissue damage from x-ray exposure, several methods for non-invasive measurement of the spinal curvature have been developed. One of them is the DTP-3 position system allowing for a three-dimensional measurement of anatomical landmarks (spinous processes) and the calculation of curvature angles in both the frontal and sagittal planes. We were interested to know whether the DTP-3 was precise enough to determine the true spinal curvature. Material and methods To determine the precision of the DTP-3 system, we constructed a model of the spine. The model was then repeatedly investigated by both the noninvasive and x-ray methods. The distortion of x-ray images caused by the central projection mechanism was considered and included in the calculation. In addition, a group of patients with scoliosis up to 40° was evaluated by both the DTP-3 system and x-ray (the latter according to Cobb's method). Results Differences in spatial coordinates between DTP-3 and x-ray examinations reached 20.9 mm in the frontal plane and 67.3 mm in the sagittal plane without distortion correction of x-ray images. The differences decreased below 1.5 mm after image distortion correction in each plane. Distortion correction had not the same effect for angle parameters as for coordinates. Differences between the DTP-3 angle parameters and Cobb's x-ray angles were below 4.7°, both without correction and after correction. The difference between DTP-3 angle parameters and Cobb's x-ray angles was -1.8° ± 3.0° (mean ± standard deviation) when measurement was performed on the patients with scoliosis. Discussion The goal of any clinical examination is to obtain data applicable to decision-making analysis. In the case of scoliosis it is necessary to report results in terms of Cobb's angle, which is the problem for all surface-dependent methods, especially in patients with double curves. A solution may be to define the maximal difference between noninvasive and x-ray methods that could be acceptable for good clinical practice. Conclusions In this study we report good concordance between noninvasive and x-ray examinations of a modeled spinal deformity in terms of both angle and linear measurements. The same results were obtained for angle measurements in a group of patients with scoliosis up to 40°. Based on this study and our previous data we believe that the DTP-3 system can be introduced into clinical practice.
