Clinical evaluation of all-ceramic crowns fabricated from intraoral digital impressions based on the principle of active wavefront sampling

Statement of problem After the development of a variety of ceramic restorative systems over the past 20 years, the fabrication of fixed dental prostheses has undergone considerable change. Esthetics and resistance to fracture are two of the main determinants of the success of a restoration; the third is marginal adaptation. Therefore, a systematic review of the literature dedicated to the marginal accuracy of ceramic systems is indicated. Purpose This study reviewed the current scientific literature that pertains to the marginal fit of ceramic crowns fabricated with different systems and investigated the factors that influence marginal adaptation. Material and methods An electronic search was completed by using the PubMed and Scopus databases with the following combination of key words: (discrepancy or fit or gaps or adaptation) and (disilicate or ceramic) and (copings or crowns). The search was limited to English-language peer-reviewed articles published before April 15, 2012. Titles and abstracts were read to identify articles that fulfilled the inclusion criteria designed for this review. Results Of 469 studies identified, 54 satisfied the selection criteria and were included in this review. All were published between 1994 and 2012. A total of 17 ceramic systems were tested in 48 in vitro and 6 in vivo studies. Of all the marginal gaps measured, 94.9% were in the range of clinical acceptability. Study heterogeneity made it impossible to conduct a proper meta-analysis of research findings and to compare and rank the various systems in terms of marginal fit. Four parameters were found to influence marginal adaptation: finish line configuration, value of the predefined cementing space, veneering process, and cementation. Conclusions The systems evaluated in the selected articles generally provide a clinically acceptable marginal fit. The current state of research does not allow for a proper comparison of the various systems in terms of marginal fit. The use of computer x-ray microtomography is recommended for further research on marginal adaptation.

Marginal adaptation of ceramic crowns: A systematic review

The fit of cobalt-chromium three-unit fixed dental prostheses fabricated with four different techniques: a comparative in vitro study

Purpose
To review methods used to investigate marginal adaptation of crowns and fixed dental prostheses (FDPs), and to discuss testing variables employed and their influence on results.

Methods
Online libraries including PubMed, Scopus, and Ovid were searched for articles evaluating the marginal adaptation of crowns and FDPs using a combination of the keywords: “marginal accuracy,” “marginal fit,” “marginal gap,” “marginal discrepancy,” “fitting accuracy,” “crown,” and “FPD.” Peer-reviewed publications in English in the period 1970 to December 2011 were collected, evaluated by their abstract, and included if they met the inclusion criteria. The criteria involved studies evaluating marginal adaptation of crowns and FDPs through clear experimental protocols. Exclusion criteria involved longitudinal prospective and retrospective clinical evaluations, studies using subjective tactile sensation, and other predefined criteria.

Results
A total of 277 papers were identified; only 183 met the inclusion criteria. Direct view technique was used by 47.5% of the articles followed by cross-sectioning (23.5%), and impression replica (20.2%) techniques. The marginal gap values reported by these techniques varied among individual crown systems and across different systems because of variations in study type (in vivo vs. in vitro), sample size and measurements per specimen, finish line design, and stage at which the marginal gap was measured.

Conclusion
There was a substantial lack of consensus relating to marginal adaptation of various crown systems due to differences in testing methods and experimental protocols employed. Direct view technique was the most commonly used method of reproducible results. Also, conducting an experimental set-up of testing a minimum of 30 specimens at 50 measurements per specimen should produce reliable results. Additionally, using a combination of two measurement methods can be useful in verification of results.

Summary
The evolution and development of CAD/CAM systems have led to the production of prosthetic reconstructions by going beyond the use of traditional techniques. Precision adjustment of prosthetic elements is considered essential to ensure sustainable restoration and dental preparation. The purpose of this article was to summarise the current literature on the fitting quality of fixed prostheses obtained by CAD/CAM technology.

A systematic review of CAD/CAM fit restoration evaluations.

Summary Replication of the space between a tooth and its cast crown, using a light-body silicone supported by a heavy-body silicone, is a recognized technique to evaluate the quality of a restoration. This study validates a similar method that is of great clinical and experimental interest. Whatever the type of silicone used, comparison is possible between different technical procedures of crown elaboration (type of impression, type of material, method of spacing, etc.). If an appropriate silicone is used, the cement space may be reproduced and its thickness measured, whatever the localization (cervical, axial, occlusal).
