Summary Background Craniofacial anomalies, although uncommon, can have considerable effects on the individual, their family and society. 1–4 They carry with them a large morbidity and require a highly specialized, multidisciplinary approach to treatment. 5 Facing the World (FTW), was founded in 2002, to offer facial reconstructive surgery to children with complex, craniofacial anomalies with no prospect of local treatment, from developing countries anywhere in the world. Methods We present an 8-year audit of the cases treated by FTW, where children are brought from their own countries to the UK for treatment. Patient selection takes place prior to their arrival in the UK by a multidisciplinary team. Specifically the condition has to be correctable to a degree that justifies the risks involved with the surgery, and the disruption to the child and their family. Results Since inception, FTW has evaluated more than 300 cases and provided treatment in the UK for over 24 cases from 18 different countries. We present our range of cases and complications. We discuss our complication rate of 28% and mortality rate of 4% (1 case). Conclusions Key to the sustainability of FTW is the development of local healthcare infrastructure within the developing countries to facilitate eventual local management of the more straightforward cases and follow up of these patients by well-trained medical staff. By establishing these programs, FTW aims to not only change these children's lives but to raise awareness, and help to expand the global craniofacial network whereby in the future, satellite partners will be present to help manage these conditions locally. Facing the World: audit of activity 2002-2010.

Interplast teams conducted two-week (or less) camps in India over a 14 year period. The majority of the teams were mixed and consisted of German and Dutch plastic surgeons and nurses. In five different villages and cities, 1015 patients were operated: 41% concerned cleft lip and palate and 32% post-burn contractures. The work is rewarding and is considered by some the best holiday imaginable. The teams intend to continue their project in the future.

Most of our rural population does not seek the expensive specialist service of distant city hospitals due to social or financial reasons. Existing health system in rural area is not efficient enough to meet the health demands of such vast population. There is only one Government Plastic surgery unit covering the population of 40 lacs leading to huge backlog of patients waiting for reconstructive surgery. To evade this situation a team of Plastic surgeons from Government Medical College Jabalpur goes to rural areas twice a year and has operated in on nine occasions since last 4 years. The activities and experiences of the last nine visits are reported here.

Humanitarian Basic Plastic Surgery

Abstract Background INTERPLAST, Inc. was founded in 1969 by plastic surgeons in Stanford, CA, to create a financial basis through donations to operate foreign patients at Stanford or send surgical teams to developing countries. With the same financial effort, 50 to 100 times more patients can be operated on locally than in a hospital at home. Stanford’s example was appealing to many plastic surgeons worldwide, who founded similar INTERPLAST NGOs in their own countries. Methods A literature review revealed worldwide humanitarian actions of INTERPLAST teams, whose annual effectiveness is comparable to the operation numbers of large plastic surgery departments. Six patients with complex facial deformities requiring multiple surgical interventions were selected for temporary stay and operations in Germany. Results Repeated missions at the same hospitals with training of local surgeons have increased significantly compared to earlier “parachute missions.” Microsurgical procedures for free flaps with magnifying glasses are now possible for the experienced even in hospitals in developing countries. The most efficient medical aid in the future will be the expansion and establishment of departments or hospitals in developing countries supported or maintained by partner hospitals in our home country. Conclusions Operations of humanitarian plastic surgeons in developing countries are becoming increasingly difficult. Local health authorities require temporary surgical permits, customs offices try to clear surgical material and look at expiration dates of medicines, and pandemics complicate planning of INTERPLAST missions. It therefore seems increasingly necessary to go as single teacher, training engaged local surgeons and assisting operations. The alternative is inviting local surgeons to Western hospitals and introducing them to the basic skills of plastic surgery and empathy with the poor. However, the lack of recognition of medical degrees from all developing countries remains a problem for their training in Europe. Level of evidence: Level V, risk/prognostic

Interplast is a humanitarian charitable organization which sends plastic surgery teams to developing countries or areas. Interplast-Germany has operated in Cochin, southwestern India on eight occasions since 1984. The activities and experiences of the last five visits are reported in an attempt to stimulate others to participate in such projects or undertake new projects on their own.
