We conducted a retrospective case-control study to investigate a possible association between alcohol intake and stroke. Reported recent alcohol consumption and biochemical and hematologic markers of alcohol intake were examined for 230 patients with stroke (20 to 70 years old) and compared with concurrently collected data on controls matched for age, sex, and race. A single estimate of current intake was used as a measure of alcohol consumption. Among men, the relative risk of stroke (adjusted for hypertension, cigarette smoking, and medication) was lower in light drinkers (those consuming 10 to 90 g of alcohol weekly) than in nondrinkers (relative risk, 0.5), but was four times higher in heavy drinkers (consuming greater than or equal to 300 g weekly) than in nondrinkers. Because very few women in our study drank heavily, we were unable to determine whether heavy alcohol intake influenced the risk of stroke in women. With increasing serum concentrations of the biochemical markers of alcohol intake (aspartate aminotransferase, uric acid, and gamma-glutamyl transferase), we observed similar trends in the relative risk of stroke. Only the erythrocyte mean cell volume did not follow this pattern. We conclude that heavy alcohol consumption is an important and underrecognized independent risk factor for stroke in men, but our data are not adequate to settle the issue for women. Our conclusions are qualified by our reliance on reported recent alcohol consumption as the primary measure of intake.

Stroke and alcohol consumption.

The first problem we encounter in trying to analyze the relationship between alcohol use and crimes of violence is in the choice of a theoretical context. One possibility is to look at violent crime as a subset of deviant behavior and put it in this theoretical sociological context, if one such context is possible. On the other hand, we could look at it from the viewpoint of aggressive behavior in general and see violent crimes as a sample of such behavior, a sample biased in favor of extreme forms of aggressive behavior. I have chosen the latter approach, since a framework limited to sociological variables would leave out central explanatory factors that are not relevant on the aggregate sociological level of analysis. Much of the empirical and theoretical outcomes of research on deviance are applicable in the explanations, but the emphasis will be on the connections with research on aggressive behavior. The relevant sociological factors will be seen only as one set of variables in an explanatory framework that encompasses (and must encompass) research findings from several disciplines.

Alcohol and Crimes of Violence

In 1941, Brown and Harvey described hypoglycemia after alcoholic debauch in six chronic alcoholics from the Baltimore area (1). In 1942, Tucker and Porter noted four cases among chronic alcoholics from Richmond, Virginia (2). Since then, fewer than 100 instances of alcoholism associated with hypoglycemia have been reported, and most of these accounts have appeared in the foreign literature (3-12). Certain summary statements about \"alcohol hypoglycemia\" are justified. First, although the condition was originally ascribed to \"smoke,\" or more specifically, denatured alcohol solvents (\"Solox\") 1 (1, 2), later reports have excluded methanol, ethyl acetate, gasoline, or methyl isobutyl ketone (3, 4, 7, 9-12).
