As a consequence of the latest psychiatry-related reform in Sweden and its implementation, relatives and family members have taken over from the formal healthcare system significant responsibility for the care of persons with a mental disability and illness. The aim of this study was to systematically describe and analyze the experiences of parents' informal care responsibility. The questions were, what are the experiences around parents' informal care activities and responsibilities and how do parents construct and manage their caring responsibility and with what consequences? Semistructured in-depth interviews were conducted (16 hours of recorded material) with eight parents who were all members of the Interest Association for Schizophrenia (Intresseforeningen for Schizofreni (IFS)) in Sweden. A mixed hermeneutic deductive and inductive method was used for the interpretation of the material. The parents endow their informal caring responsibility with meaning of being a good, responsible, and accountable parent with respect to their social context and social relationships as well as with respect to the psychiatric care representatives. In this tense situation, parents compromise between elements of struggle, cooperation, avoidance, and adaption in their interaction with the world outside, meaning the world beyond the care provision for their child, as well as with the world inside themselves.

Parents’ experiences of caring responsibility for their adult child with schizophrenia

Committed and voluntary patients were interviewed about their experience of psychiatric care. They were interviewed twice: at discharge or 3 weeks after admission and 4-8 months later. Only a few of the patients thought that they should not have been admitted, but many of them thought they had been admitted too late and discharged too early. Sixty-five per cent of the committed patients and 35% of the voluntary patients understood that they had been subjected to compulsory procedures. The law states that the patients must take part in the planning of the treatment concerning themselves; even so, half of them had the feeling of not being allowed to do so. Only 40% of the patients would like to receive the same treatment again. At the second interview 85% of the committed patients stated that it is necessary to have an opportunity to command people and that doctors are very well adapted to make the decision. Only 4% thought that a lawyer should decide.

In connection with the changes in psychiatry, the situation of the relatives is seldom considered. The great importance of the relatives for the patients in psychiatric care is shown in this study, in which relatives of both committed patients and voluntarily admitted patients' have been interviewed. The mental patients need for care, even committed care, was confirmed by the relatives. The relatives' own situation was greatly influenced by the mental patient. By supporting the patient, the relatives had to give up normal activities of their own. On the other hand, they were seldom asked to take part in the treatment planning of psychiatry, and they did not experience being accepted as an important resource to the mental patient. The research clearly points out the importance of giving support to the relatives and of being actively implicated and accepted as important resources in the treatment of the patients.

A psychiatric study of 197 children living in villages outside Khartoum was made in 1964-1965. In 1983, 104 randomly chosen subjects were investigated again. All were interviewed by a psychiatrist and somatically examined. The overall psychiatric impairment was 14% (males, 18%; females, 8%) according to the psychiatric interview; the Self-rating Questionnaire gave a higher figure (28%). The rate of somatic illness was similar: 14% according to the medical doctor, 22% according to the self-rating of the subjects. Childhood data predicted 17% of the variance in adult mental health. Somatic state of health in childhood and traditional type of work by the father were the most important variables. Data from the life situation of the individual explained 67% of the variance in adult mental health.

Compulsory psychiatric care is a challenge for research. One challenge is the conflict concerning the concept of compulsory care itself, due to different ethical views and different ideological and professional traditions. A Swedish study of 100 committed and 99 voluntarily admitted patient, their relatives and their attendants, gave many interesting answers concerning attitudes to compulsory psychiatric care. The most important conclusions concerns the complexity and ambiguity of the concept of coercion and not least the concept of justifiable coercion.

The patients' experience of violations of their integrity will be analyzed against the psychiatric picture of the committed patients. Conceivable measures of action are discussed. Some measures have already been established to improve the care in the field in which the present study has shown shortcomings of the psychiatric care today. A restudy is planned in a few years to evaluate the outcome of these measures.
