Human Torque teno virus: epidemiology, cell biology and immunology

Tranfusion-transmitted virus (TTV), a single-stranded circular DNA virus that chronically infects humans and other animals, displays a high degree of genetic diversity and was originally thought to be associated with hepatitis. The prevalences of TTV infection among different populations of humans and non-human primates from Venezuela have now been evaluated, using serum samples and three different detection tests. All three tests were PCR-based, one involving a hemi-nested PCR and primers based on the N22 open-reading-frame-1 region (N22-PCR), another employing 55 cycles with primers from the more conserved untranslated region (UTR-PCR), and the other using a hemi-nested PCR with primers from the same region (HUTR-PCR). The overall prevalences of human infection appeared much higher with the HUTR-PCR (52%) than with the N22-PCR (13%) or the UTR-PCR (5%). When the products amplified by N22-PCR from 28 human isolates of TTV were sequenced, only two genotypes of the virus were detected. The non-human sera tested came from primates kept in a zoo in north-western Venezuela. TTV DNA was detected, by HUTR-PCR, in both of the chimpanzee sera tested but not in any of the sera from the 11 New-World primates or the other 12 Old-World primates that were investigated. The results, particularly those of the HUTR-PCR, indicate that TTV infection is common in Venezuela, especially in populations, such as many Amerindian groups, who live under poor sanitary conditions. Although TTV infection may be relatively rare among non-human primates from the New World, this will have to be investigated further, using many more samples collected throughout the Americas.

This study assessed changes in serum levels of cytokines IFNgamma and IL-10 (biomarkers of inflammatory changes) and soluble biomarkers sICAM-1 and sE-selectin (biomarkers of endothelial dysfunction) in diabetic patients with and without nephropathy. IFNgamma and IL-10 were significantly elevated in patients with diabetic nephropathy (DN) and end-stage renal disease (ESRD) compared with controls and diabetic patients without DN. IFNgamma and IL-10 levels were significantly increased after haemodialysis. sICAM-1 and sE-selectin were significantly higher in diabetic, DN and ESRD groups compared with controls, and sICAM-1 but not sE-selectin was increased after haemodialysis.

 The onset of proteinuria in renal allograft recipients is a frequent complication that may be associated with an increased risk of graft failure and mortality. Our aim was to research if proteinuria effects is independent from donor specific antibody (DSA) for transplant graft survival and the changes of proteinuria response to therapy in ABMR group.
 
 
 
 85 transplanted patients were enrolled in our study and followed up till 31 October 2020 or till death, or date of return to dialysis. We created three groups: ABMR group (n = 19, biopsy proven antibody mediated rejection), ABMRDSApos positive group (n = 14) ABMRDSAneg (n = 5), DSA negative group with stabile kidney function without rejection as a reference (n = 52). Differences in patient, donor and transplant characteristics between DSA positive and negative groups were assessed by Fishers exact test for categorical variables. Death censored graft loss was assessed by Kaplan Meier analysis with log risk statistics.
 
 
 
 Proteinuria decrease after treatment in ABMR group (P = 0.0009). Graft failure's frequency increase every 10 mg/mmol elevation of proteinuria means 7% elevation (hazard ratio: 1.07%). Before treatment nephrotic proteinuria was found in group AMBRDSApoz 21%, 14,29% in group ABMR DSAneg 1,92% in reference group. Estimated 3-year graft survival was 87, 5% in ABMR group, 93% in DSA pos group, and 100% in DSA negative group (log-rank probe P = 0.0666).
 
 
 
 The presence of DSA increases graft loss but it is independent to proteinuria. Therapy refractory proteinuria state represents worse graft survival.
