The evaluation of the effect of performing guided lid surgery with enucleation of a cystic lesion; a case report

Background: We recorded all cases presenting with paranasal sinus cysts to draw attention to those with unusual presentations and their management outcome in a retrospective original study. Methods: All patients were subjected to history-taking, ENT examination, endoscopic examination, CT scanning, and managed with endoscopic sinus surgery/or a combined approach. Results: We encountered 54 patients with paranasal sinus cysts. Mucocele/pyocele was the commonest (79.6%), followed by radicular cyst (9.3%), dentigerous cyst (5.55%), and fungocele (5.55%). The frontal sinus was the commonest sinus of cyst origin (55.6%). Maxillary sinus cysts occurred in 16.6% of cases. The sphenoid sinus was the "least" sinus of cyst origin in 7.4% of cases. Conclusion: Ethmoid mucocele occurred mostly by extension from other sinuses, predominantly frontal. With exclusive frontal sinus cysts, it is essential to look for possible underlying pathology or associated complication. Fungocele, a new nomenclature, could complicate those with allergic fungal sinusitis. 

Paranasal Sinus Cysts Encountered Over Twenty Years: Varieties, Management, Recommendations, and Outcome

We report a case of pilar cyst in a 50 year old female with a swelling in the left parietal region. Gross findings revealed a cystic cavity with multiple infoldings. The characteristic histopathologic feature of a pilar cyst with absence of granular layer and swollen cells were evident. Attention to detail, prompt diagnosis and treatment based on the signs and symptoms are a must for patient welfare.

Jaw cysts commonly affect the oral and maxillofacial region, involving adjacent tooth roots. The management of these teeth, particularly regarding root canal therapy and apicoectomy, lacks consensus. This study introduces a novel treatment concept and refined surgical approach to preserve pulp viability in teeth involved in jaw cysts. The objective was to investigate the effectiveness and potential benefits of this approach over a 36-month follow-up period. A conservative management approach prioritized vitality preservation, reserving root canal treatment and apicectomy for cases with post-operative discomfort. A comprehensive follow-up of 108 involved teeth from 36 jaw cyst cases treated with the modified method was conducted. Clinical observation, X-ray imaging, cone-beam computed tomography (CBCT), and pulp vitality testing assessed changes in cyst size, tooth color, pulp vitality, root structure, and surrounding alveolar bone. After 36 months, our modified surgical approach successfully preserved tooth vitality in 84 involved teeth. Adverse symptoms in 19 teeth, such as redness, swelling, fistula, and pain, resolved with postoperative root canal therapy. Follow-up was lost for five teeth in two cases. No cyst recurrences were observed, and in 34 cases, the bone cavity gradually disappeared, restoring normal bone density during long-term follow-up. Our modified surgical method effectively preserves tooth vitality in jaw cysts. This innovative approach has the potential to improve the management of teeth involved in jaw cysts.

Cysts are of various types, and over the years, various classifications are put forward, which are helpful to identify each cyst, based on its origin and its clinical and histopathological presentation. Based on the classification, one can also decide the treatment plan accordingly. The classifications are given by various researchers and also by WHO. In this chapter, various odontogenic and nonodontogenic cysts and their treatment aspect are discussed in detail.
