Investigation of the viability of an integrated cloud-based electronic medical record for health clinics in Free State, South Africa

Pregnancy and the postpartum period offer a unique opportunity to identify patients with risk factors leading to premature cardiovascular disease (CVD), which often go unrecognised.This study investigates self-reported prevalence of CVD-related pregnancy complications and its documentation in electronic medical records (EMRs) in an academic family health team (AFHT).A retrospective cross-sectional survey conducted from 2016 to 2017 in an AFHT.The survey assessed self-reported pregnancy complications and obstetric histories of adult females. EMRs of responders who provided consent were appraised for documented pregnancy complications, and management of traditional cardiovascular risk factors post-pregnancy.Out of 211 responders, 28% (n = 60) had at least one pregnancy complication reported in the survey and/or in the EMR, of which 67% (n = 40) had the complication documented in their EMR. The most prevalent complications were preterm birth (PTB; 12%, n = 25), hypertensive disorders of pregnancy (HDP; 10%, n = 22), and gestational diabetes mellitus (GDM; 7%, n = 14). Twenty-nine per cent (n = 4) of the patients with GDM had a 75 g oral glucose tolerance test result documented post-pregnancy. Of those with HDP, 36% (n = 8) had body mass index and 50% (n = 11) had a blood pressure measurement recorded after delivery.There has been a significant lack of documentation of pregnancy-related cardiovascular risk factors and subsequent management, introducing a missed opportunity for early cardiovascular intervention. Adequate documentation of pregnancy complications in the EMR and better transitions in care between obstetric and primary care teams could potentially enable clinicians to intervene early and better manage females at increased risk of CVD. 

The prevalence of documented cardiovascular-related pregnancy complications: cross-sectional study in an academic primary care centre

Purpose – This paper aims to provide an insight into recent advancements and developments of robotics for Natural Orifice Transluminal Surgery (NOTES) procedures. Design/methodology/approach – Following an introduction that highlights the evolution from Minimally Invasive Surgery (MIS) to NOTES in the medical field, this paper reviews the main robotics systems that have been designed and implemented for MIS and NOTES, summarising their advantages and limitations and remarking the technological challenges and the requirements that still should be addressed and fulfilled. Findings – The state-of-the-art presented in this paper shows that the majority of the platforms created for NOTES are laboratory prototypes, and their performances are still far from being optimal. State of the art in robots used in minimally invasive surgeries. Natural Orifice Transluminal Surgery (NOTES) as a particular case

Electronic Medical Record (EMR) relational database is considered to be a major component of any medical care information system. A major problem for researchers in medical informatics is finding the best way to use these databases to extract valued useful information to and about the patient’s diseases and treatments. Integrating different EMR databases is a great achievement that will improve health care systems. This paper presents an AI approach to extract generic EMR from different resources and transfer them to clinical cases. The utilized approach is based on retrieving different relationships between patients’ different data tables (files) and automatically generating EMRs in XML format, then building frame based medical cases to form a case repository that can be used in medical diagnostic systems. Robotic surgery is a growing technology with the potential to revolutionize surgical procedures, especially in the field of urology. Urology is at the forefront of advances in this field, and has integrated robotics into many procedures including radical cystectomies, surgical nerve grafting and pyeloplasty.1 Robotic surgery has almost entirely taken over radical prostatectomy and the role of robotics is continuously expanding. Robotic surgery helps improve patient outcome by minimizing the surgeon’s movement tremors, increasing range of motion, decreasing blood loss, decreasing length of hospital stay, and decreasing post-operative pain.2,3 Since the field of Urology deals with very difficult and delicate procedures, robotics offers an advantage by allowing for greater accuracy, flexibility, smoother actions, and greater range of motion.
