The perceived risk for pulmonary embolism (PE) can be assessed by oxygenation and calculation of the alveolar–arterial (A-a) oxygen (O2) gradient. We attempt to evaluate the efficacy of A-a O2 gradient for the diagnosis of PE and if it can predict the degree of severity of PE. This study is a prospective study conducted on 70 patients presented by signs or symptoms of suspected acute PE. Arterial-blood gases including arterial partial pressure of oxygen (PaO2), partial pressure of carbon dioxide (PaCO2), and arterial oxygen saturation (SaO2) and computed tomography pulmonary angiography were done on admission. Fifty patients proved to have PE by computed tomography pulmonary angiography. The patients were divided into (a) nonhigh-risk and (b) high-risk groups. There was a significant difference between the two groups regarding pulmonary artery obstructive index. Although A-a gradients were high in all studied patients with positive PE in comparison to negative PE patients, there was no significant difference between high-risk and nonhigh-risk groups regarding PaO2 (mmHg), arterial oxygen saturation, %, A-a O2, PaCO2. In addition, no significant relationship was detected between arterial-blood gas parameters regarding PaO2 and SaO2 with pulmonary artery obstructive index; also PaCO2 and A-a O2 gradients were nonsignificant. The A-a O2 gradient values are clinically important in the diagnosis of patients with PE because it is easy to perform and is a bedside test. However, it may be incapable of detection of severity of PE.

Can alveolar—arterial oxygen gradient predict severity of pulmonary embolism?

Objective The objective of this study was to analyze the causes and outcomes of presentations to the emergency department (ED) due to injuries from motor vehicle crashes and pedestrian strikes along several parameters. Methods Data from 798 patients who were injured due to motor vehicle crashes or pedestrian strikes that occurred in Canakkale Province were retrospectively analyzed. Patient demographic data such as age and gender, emergency service outcomes, and the consulted clinics were also recorded. Distributions of the crashes by weekdays or weekends, national and religious holidays, official holidays, and Ramadan month were comparatively analyzed. Results A total of 253 people injured in motor vehicle crashes and 395 people injured in pedestrian strikes were directly brought to the ED from the crash or strike scene. While 656 patients were discharged from the ED, 142 patients were referred to other clinics for surgery. A total of 538 crashes occurred on weekdays and 206 on weekends, and 54 crashes occurred during official and religious holidays. Most crashes occurred in summer, and the second most occurred in autumn. The rate of pedestrian strikes that occurred in summer and autumn was statistically significantly higher than the rate of injuries from motor vehicle crashes observed in the same seasons. The majority of the weekend crashes were caused by persons who had not consumed alcohol. Conclusion Traffic crashes occur more commonly among young men and in the summer season, while national, official, and religious holidays do not seem to play a role in the frequency of traffic crashes.

Analysis of Emergency Department Presentations due to Injuries From Motor Vehicle Crashes and Pedestrian Strikes.

Amac: Bu arastirmada, Ulakbim Tip Veri tabani ve Web of Science (ISI) icerigindeki dergilerin evde bakim alaninda yapilmis Turkiye adresli yayinlarinin, belirlenen 10 kriter dogrultusunda incelenmesi amaclanmistir. Gerec ve Yontem: Retrospektif olarak planlanmis tanimlayici arastirmada 1980-2015 yili sonuna kadar Web of Science (SCI-Expanded, SSCI, AHCI) ve Ulakbim Tip veri tabani kapsamindaki dergilerde cikmis Turkiye adresli yayinlar incelenmistir. Bu veri tabanlarinda, “home nursing and Turkey”, “home care and Turkey”, “home care services and Turkey”, home care agencies and Turkey” anahtar kelimeleri ile tarama yapilmistir. Veriler arastirmacilar tarafindan belirlenen makalenin yayinlandigi derginin adi, yayin yili, indeks, makale turu, makalenin konu alani, yazarlara iliskin veriler (yazar sayisi, kurumu, alani), arastirma tipini (nitel veya nicel) iceren bir kodlama listesine kaydedildi. Veriler tanimlayici istatistiklerle degerlendirilmistir. Veri tabanlarinda tam metin yayinlanmis Turkiye adresli 110 adet yayina ulasilmistir. Bulgular: Bu yayinlarin %65.5’i 2007 yili ve sonrasinda yayinlanmis olup, %44’u dort ve daha fazla yazarlidir. Yayinlarin %89.1’i nicel arastirma, bunlardan %32.6’si tanimlayici, %15.8’i ise deneysel ve yari deneysel arastirmadir. Calismalarin konu alanlari incelendiginde; yaslinin evde bakimi (%30), evde bakim hizmetleri ve modelleri (%19.1), sagligi gelistirme, saglikli yaslanma ve yaslilik sorunlari (%19.1), gebe ve lohusanin evde bakimi (%12.7), evde anne ve cocuk bakimi (%10.9) ilk bes sirada yer alan konulardir. Cogu arastirmanin tanimlayici desende planlanmasi kanita dayali arastirmalara olan ihtiyacin gerekliligini dusundurmektedir. Cogu arastirmada evde yasli bakiminda sagligi gelistirme, kronik hastaliklar ve yasam kalitesi gibi konularin incelendigi saptanmistir. Sonuc: Evde bakima iliskin, model olusturma, evde bakim hizmetlerinin gelisimine yonelik cabalarin olusmasi, acigi kapatilmaya calisilan bir alanda gereksinimleri ortaya koymasi acisindan onemli oldugu dusunulmektedir.

SUMMARY Objektive: The purpose of our study was to investigate the relationship between the pulmonary artery obstruction index (PAOI) assessed with helical computed tomography (CT) and impairment in blood gases in patients with acute pulmonary embolism (PE). Method: This retrospective study design was carried out in the Tertiary Training and Research Hospital Emergency Clinic over a 6-month period with patients in whom PE was detected. The Alveolar–arterial (A-a) oxygen gradient for oxygen was calculated according to the formula 150 – 1.25 (PaCO2 – PaO2), and the PAOI for each patient was measured with the Qanadli score at the mediastinal window using chest CT angiography (CTA). Results: There were 113 patients included in the study; their mean age was 61.40 (±16.082) years and 51.4% of the patients were female. The mean Alveolar–arterial (A-a) oxygen gradient value was 65.13(±18.78) in massive embolism group. The mean PAOI was determined as 34.027 (min:7.5, max:83.7). The relationship between the radiologically measured PAOI and the Alveolar–arterial (A-a) oxygen gradient were compared, and an increase in the index was detected as the Alveolar–arterial (A-a) oxygen gradient level increased;this difference was found to be statistically significant (r=0.400; p<0.001). Conclusions: This retrospective observational study found a weak but positive correlation between the PAOI and the Alveolar–arterial (A-a) oxygen gradient. Our study showed that consideration of the Alveolar–arterial (A-a) oxygen gradient increases the sensitivity of blood gas analysis in documented pulmonary embolism.All data showed that the Alveolar–arterial (A-a) oxygen gradient values may be clinically beneficial, as such testing is easy to perform and cost-effective.Moreover, it can be performed at the bedside to detect which patients should be treated with thrombolytics. Correlation of alveolar–arterial gradient with Computed Tomography Pulmonary Artery Obstruction index in acute Pulmonary Embolism

Introduction and Aim Being one of the major problems in the world and in Turkey, traffic accidents require being accurately examined and solved . The purpose of this study is to reveal the effects of blood alcohol level on the severity of injury and death as a result of motor vehicle accident traumas and to review and command on the social importance of this issue.Materials and Methods Four hundred forty-five patients who were admitted to Ataturk Training and Research Hospital Emergency Department between January 2010 and December 2011, after motor vehicle accidents are enrolled in the study. Patients’ ages, gender, blood alcohol levels and vehicle types were recorded retrospectively. Abbreviated Injury Score (AIS) and Injury Severity Score (ISS) were detected for each patient according to trauma regions and severity. The patients were divided into two groups according to the blood alcohol levels (<0,5 promile and ≥0,5 promile). Results The blood alcohol levels were <0,5 promile in 341 patients (76,6%) and ≥0,5 promile in 104 patients (23,4%). There was no statistically significant difference between the groups in terms of age (p=0,754), but statistically significant difference was detected in terms of Injury Severity Score (ISS), observation time in the emergency department and cost (p<0,001). Conclusion The severity of injuries in traffic accidents and the costs of treatment to patients increase parallel to the increase in the levels of alcohol in the blood.

Amac: Bu klinik calismada amac pulmoner tromboemboli tanisi alan hastalarda ortalama trombosit hacmi ile emboli yukunun iliskisini ortaya koymaktir. Gerec ve Yontem: Bu retrospektif calismaya pulmoner emboli tanisi alan 120 hasta dahil edildi. Hastalarin ortalama trombosit hacmi, Troponin-I, D-dimer degerleri, arter kan gazlari, gogus radyografisi, elektrokardiyografi, ekokardiyografi ve venoz doppler ultrason bulgulari kaydedildi. Ayrica bilgisayarli tomografi anjiyografi kullanilarak pulmoner arteriyel obstruksiyon indeksi hesaplandi. Bulgular: Ortalama trombosit hacmi ile pulmoner arter basinci arasinda ve pulmoner arteriyel obstruksiyon indeksi arasinda pozitif korelasyon tespit edildi. Ortalama trombosit hacmi 8.5fl olan gruplar arasinda pulmoner arter obstruksiyon indeksi acisindan istatistiksel anlamli bir fark tespit edildi (25.0 and 45.0). Ortalama trombosit hacmi > 8.5fl olan olan grupta pulmoner arter basinci < 8.5fl olan gruptan istatistiksel anlamli olarak yuksek bulundu. Sonuc: Pulmoner arteriyel bilgisayarli tomografi obstruksiyon indeksi ile ortalama trombosit hacmi arasindaki korelasyonun pulmoner embolizm siddetini belirlemedeki degeri net degildir.
