Critical Care Nursing: A Holistic Approach

In the last 25 years, chromogenic culture media have found widespread application in diagnostic clinical microbiology. In the last decade, the range of media available to clinical laboratories has expanded greatly, allowing specific detection of additional pathogens, including Pseudomonas aeruginosa, group B streptococci, Clostridium difficile, Campylobacter spp., and Yersinia enterocolitica. New media have also been developed to screen for pathogens with acquired antimicrobial resistance, including vancomycin-resistant enterococci, carbapenem-resistant Acinetobacter spp., and Enterobacteriaceae with extended-spectrum β-lactamases and carbapenemases. This review seeks to explore the utility of chromogenic media in clinical microbiology, with particular attention given to media that have been commercialized in the last decade. The impact of laboratory automation and complementary technologies such as matrix-assisted laser desorption ionization-time of flight mass spectrometry (MALDI-TOF MS) is also assessed. Finally, the review also seeks to demarcate the role of chromogenic media in an era of molecular diagnostics.

Candida species are the most common commensal fungus that coexists with hundreds of species of bacteria in the human body. Between 24 and 70% of humans harbor Candida species in various body niches, including the oral and vaginal mucosa and the skin (

Out of over 150 Candida species, Candida albicans is the principal pathogenic species that causes infections, especially in patient populations with immune dysfunction due to HIV infection, malignancy, immunosuppressive therapy, and organ transplantation. Therefore, these opportunistic infections of Candida in topical or systemic forms have become widespread and account for 8 to 10% of bloodstream infections in hospitals (

). Nearly 70% of denture wearers experience denture stomatitis, or inflammation of oral mucosa covered by denture prostheses, with C. albicans being a primary etiological factor (

,

). Almost 75% of the female population has experienced an episode of vulvovaginal candidiasis at least once in their lifetime, and many have recurring episodes (

). In many of these conditions, there is a phenotypic change for Candida from harmless commensal to invasive pathogen. Adhesion to various surfaces, morphogenesis, phenotypic and genotypic switching, and production of lytic enzymes are major virulence mechanisms facilitating this conversion (
6
). However, properties of the host are also commiserate in enabling Candida to act as an invasive pathogen since compromise in the interleukin-17 (IL-17)/Th17 arm of the host immune response (e.g., AIDS, Job’s syndrome, etc.) or an imbalance in the host microbiome (
7
) both can contribute to candidiasis (
7
). During this shift, commensal or transient organisms living with Candida species in various locations may play diverse roles in the process of pathogenesis; environmental bacteria may also be introduced via catheters, cannulae, and prosthetic appliances and interact with the already present Candida. Such interactions may be detrimental to the health of the human host, leading to mortality.

Candida-bacteria interactions: Their impact on human disease

Background: Pulmonary embolism is the most important emergency in the cardiovascular system with a high rate of mortality without appropriate treatment. Aim: To investigate the effect of an educational program on critical care nurses' performance regarding pulmonary embolism emergency care. A Quasi-experimental design was utilized in this study. This study was carried out in the medical, surgical and emergency Intensive Care Unit at Al-Thawra Modern General Hospital Authority in Sanaa – Yemen. The sample of this study consisted of 75 nurses' for implementing the program. Tools utilized for data collection were the nursing questionnaire and observation checklist. The results There were statistically significant differences (P<0.000) in the total nurses, knowledge scores regarding emergency care for a patient with pulmonary embolism, pre-implementation of the program compared with the immediately post-implementation and post three months follow up. There were statistically significant differences (P<0.000) in total nurses, practice scores regarding emergency care for patient with pulmonary embolism pre-implementation compared with the immediately post-implementation and post three months follow up. Conclusion: There was a statistically significant difference between the nurse knowledge and practice in pre/post and follow up implementing of the educational program.

Effect of An Educational Program on Critical Care Nurses Performance regarding Emergency Care for Patients with Pulmonary Embolism

Heat stroke is an acute, life-threatening emergency characterized clinically by elevated body temperature and central nervous system dysfunction. The nurse has an important role in critical care team. Therefore, the aim is to evaluate the effect of an educational program on the nurses' performance regarding heat stroke. Quasi-experimental research design was used to conduct this study. This study was carried out at Emergency department and isolation room at Assiut University Hospitals. The sample: A convenient sample included all nurses working at Assuit University Hospital ; 30 nurses in the emergency unit and 12 nurses in isolation room .Tools: Two tools were utilized in this study. Tool I: Nurses' knowledge assessment. Tool II: Nurses’ performance observational checklist questionnaire. Results: The present study revealed that total mean scores of the nurses' knowledge was 13.02±4.49 pre implementation, 27.57±2.38 post implementation of educational program and 26.33±4.06 at follow up. Total mean scores of the nurses’ performance was 112.64±8.84 pre implementation, 177.97±8.35 post implementation of the program and 160.67±14.44 at the follow up .So there is a significant difference between their knowledge before and after the educational program. Conclusion: The study concluded that the educational program led to improvement in nurses' knowledge and performance about heat stroke. Recommendation: The program should be utilized for all new graduated nurses to be orientated to all aspects of care provided for heat stroke patients.

Effect of an Educational Program on .

Nutrition support can result in improved wound healing, a decreased catabolic response to injury, enhanced immune system function, improved gastrointestinal structure and function, and improved clinical outcomes). The appropriately and timely nutritional intervention can improve patient recovery and survival, decrease complication rates, and decrease costs. Gastrointestinal complications (vomiting, diarrhea, constipation, and abdominal distension) are most commonly associated with complications derived from enteral feeding. Aim: this study was carried out to investigate the effect of the intermittent enteral feeding schedule on the occurrence of gastrointestinal complications and the length of the hospital stay among critically ill patients at Assiut University Hospitals. Design: a quasi-experimental design. Setting: trauma ICU at Assiut University Hospitals and the study took approximately one year started from July 2010 till July 2011. Patients: A convenience sample of 80 adults' critically ill patients on enteral feeding constituted the study sample. The patients were assigned randomly into two equal groups (control group and study group, 40 patients each).. Methods: The only manipulation was in the rest period and time interval in which the study group subjects were rested 8hours at night as compared to 6hours for the control ones, as well study group subjects were having 4hours time interval between each two consecutive feeding as compared to 2hours for control group subjects. Results: There was a significant statistical difference between both groups (p=0.000)indicating lesser hospital stay among study group subjects (52.5 % of the study group subjects were hospitalized less than one month as compared to 35 % of the control group subjects were stayed between 30 to less than 45 days). It was also found that, 57.5% of control group patients developed gastrointestinal complications as compared to 45% of the study group patients (n.s). Conclusion: intermittent 4-hour enteral feeding schedule had lowered the incidence of gastrointestinal complication and length of the hospital stay.

Effect of intermittent enteral feeding schedule on the occurrence of gastrointestinal complications and hospital stay among critically ill patients

Cardiac catheterization remains the most definitive procedure for diagnosis and evaluation of coronary artery disease. Aim : this study was carried out to investigate the impact of early ambulation post transfemoral coronary procedures on back pain, urinary discomfort and vascular complications. Design : a quasi-experimental design. Setting : in catheterization and coronary care units. Subjects : A convenience sample of all adult educable and mentally competent male and female patients aged from (18-60 years old) who are scheduled for non-emergency percutaneous coronary intervention (PCI) and coronary angiography (CA) through femoral artery during a period from july 2010 to june2011 were eligible for inclusion in the sample. Tools : Four tools were utilized to collect data pertinent to the study, tool assessment of patients after femoral sheath removal and angioplasty data tool II: assessment of back pain tool III : urinary discomfort assessment tool. tool IV : vascular complications measurement after femoral cardiac catheterization Methods : patients in the study group were ambulate after four hours bed rest post transfemoral PCI and 2 hours after CA, whereas patients in the control group were ambulate after 12–24 hours post transfemoral PCI and 6-8 hours after CA(usual care). Results : Finding of the present study revealed that a significant statistical difference was existed between both studied groups in relation to back pain and urinary discomfort for PCI and CA. As regards to vascular complication, no significant statistical differences were put into evidence between both studied groups. Conclusion : early ambulation is safe and feasible for patients undergoing PCI and CA. Key words : early ambulation, femoral coronary procedures, back pain, urinary discomfort, nurse's role, and vascular complications.

Impact of Early Ambulation on Patients' Outcome Post Transfemoral Coronary Procedures, at Assiut University Hospital

Background Many patients with percutaneous balloon valvuloplasty had multiple complications. Thus, they need special nursing guidelines to reduce complications and improve outcomes needed for those patients. Aim of the study To evaluate the effectiveness of nursing guidelines on outcomes of patients with percutaneous balloon valvuloplasty. Research design A randomized control trial was utilized in this study. Settings The study was conducted in the cardiac catheterization unit at Al Orman University Hospital. Sample A convenience sample of 60 patients who were admitted to cardiac catheterization to receive percutaneous balloon valvuloplasty was divided into two equal groups; Intervention and control groups. Tool Ipatients' assessment sheet, it included four parts; socio-demographic data, patient's assessment measurements, laboratory investigations, and pain scale. Tool II: complications and patient outcome. Methods: the researchers assessed the hemodynamic parameters before and after the procedure. Results more than half (56.7%) of the study group had no large hematoma but most of the control group had < 5 cm of hematoma until discharge of percutaneous balloon valvuloplasty. There was a statistically significant difference between both groups regarding pain, hematoma, and bleeding. Conclusion: Nursing guidelines lead to significant improvement in the outcomes of patients with percutaneous balloon valvuloplasty. Recommendation Keep the nursing guideline of percutaneous balloon valvuloplasty available in the cardiac catheterization unit.
