PURPOSE To assess the degrees of agreement in measuring corneal thickness in normal eyes between 2 noncontact systems based on the dual Scheimpflug system (Galilei; Ziemer) and rotating Scheimpflug imaging (HR Pentacam; Oculus) and also between each of these methods and the gold-standard method of ultrasound pachymetry. METHODS In a prospective study, measurement agreement was assessed in 47 eyes of 47 healthy subjects. All eyes were examined with each of the 3 devices. Measurements made with the Galilei and HR Pentacam were compared with those made with ultrasound. The central corneal thickness (CCT) and thinnest pachymetry of the Galilei and HR Pentacam were also compared. RESULTS The mean values of CCT obtained from Galilei, HR Pentacam, and ultrasound were 560.57 ± 29.10, 542.31 ± 30.50, and 548.61 ± 29.92 μm, respectively. The 95% limits of agreement of each of these devices with ultrasound were -11.03 and +34.94 μm for the Galilei and -33.67 and +20.74 μm for the Pentacam. The 95% limits of agreement of Galilei with Pentacam in measurement of central and thinnest corneal thickness were -3.39 and +33.11 and -10.40 and +27.29 μm, respectively. Although CCT measurements made with the HR Pentacam were thinner (P < 0.001), and those obtained with Galilei were thicker (P < 0.001) than those made with ultrasound, there were significant correlation between Galilei and ultrasound (r = 0.92; P < 0.0001) and between HR Pentacam and ultrasound (r = 0.89; P < 0.0001). The central and thinnest corneal thickness measurements made with the HR Pentacam were thinner (P < 0.0001) than those made with Galilei, but there was a significant correlation between Galilei and HR Pentacam in measuring CCT (r = 0.96; P < 0.0001) and thinnest corneal thickness (r = 0.95; P < 0.0001). Bland-Altman plots showed good agreement between these methods. CONCLUSIONS Although differences between the devices were statistically significant, there was good correlation and agreement between Galilei and Pentacam in measuring central and thinnest corneal thickness. The corneal thickness measurements made with the HR Pentacam and Galilei also showed good correlation and agreement with those made with ultrasound.

PURPOSE Tumor necrosis factor-alpha (TNF-alpha) plays a central role in the pathogenesis of ocular inflammation and the level of TNF-alpha is increased in ocular fluids of patients with uveitis. Intravenous infliximab, a monoclonal antibody against TNF-alpha, has been used for the treatment of uveitis with promising preliminary results. The aim of this study was to evaluate the effect of intravitreal injection of infliximab on experimental uveitis. METHODS Thirty-three white New Zealand rabbits were divided randomly into three groups. Group 1 (n=5) received intravitreal injection of 1 mg/0.1 cc infliximab plus 0.1 cc normal saline, group 2 (n=14) received intravitreal injection of 2 microg Salmonella typhimurium endotoxin plus 1 mg/0.1 cc infliximab, and group 3 (n=14) animals received intravitreal endotoxin 2 microg/0.1 cc plus normal saline 0.1 cc. Inflammation was evaluated by clinical examinations on days 1, 3, 5, and 7 after the injections; measuring the protein concentration and inflammatory cell content of the aqueous humor; and histopathologic examination. RESULTS No inflammation occurred in group 1 animals. There was a statistically significant difference between group 2 and 3 animals with regard to clinical examination on the third, fifth, and seventh postinjection days. The differences between groups 2 and 3 were significant with regard to aqueous cell counts and protein content at day 7 (p=0.02 and p=0.001, respectively). Histopathologic examination results showed less inflammation in group 2 animals compared to group 3 animals (p=0.009). CONCLUSIONS The results provide evidence that intravitreal injection of infliximab suppresses ocular inflammation in a rabbit model of severe endotoxin-induced uveitis.

The objective of this study is to assess the efficacy of oral fluoxetine therapy in improving the visual function of amblyopic patients aged between 10 and 40 years old. In this double-blinded, randomized, controlled trial (IRCT2016052428046N1; registered retrospectively), 40 eligible participants with anisometropic or mixed amblyopia were randomly assigned to either fluoxetine or placebo groups. Participants with anisometropia and logMAR best spectacle-corrected visual acuity (BSCVA) worse than 0.2 logMAR in the amblyopic eye or at least a two-line of difference in the BSCVA between the fellow eyes were included. Participants with significant ocular or systemic diseases were excluded. In both groups, the better eye of each patient was patched for 4–6 h a day during the study period. Participants in the treatment group were treated with oral fluoxetine for 3 months. Change in the Snellen BSCVA (after 3 months) was regarded as the primary outcome measure. Data from 20 participants in the fluoxetine group and 15 participants from the placebo group were analyzed (aged 11–37 years). The magnitude of improvement in visual acuity (from baseline to 3 months after treatment) was significantly higher in the fluoxetine group (0.240 ± 0.068 logMAR; 2.4 line-gain) compared with the control group (0.120 ± 0.086 logMAR; 1.2 line-gain). This study suggests beneficial effects of fluoxetine in the management of adult and adolescent amblyopia.
