Magnetic resonance-guided radiation therapy (MR-GRT) offers great potential to improve radiation treatment outcomes by providing more accurate and patient-tailored therapy. Despite superior soft tissue contrast in MRI, one of the challenges towards MRI-only workflows is that the process often requires some sort of 'MR-invisible' metal-based devices. In this study, the feasibility of quantitative susceptibility mapping (QSM) for visualization of some MR-invisible radiation therapy devices was studied. Our recently proposed QSM-based algorithm for brachytherapy seed visualization was modified and the feasibility of the optimized algorithm for visualization of different devices including: brachytherapy seeds, plastic interstitial needles, CT-markers and obturators, and different types of fiducial markers in agar, prostate and meat phantoms were studied. All phantoms were scanned using 3T MR scanner with a 3D multi-echo gradient recalled echo (ME-GRE) pulse sequence. The QSM results in all phantoms were compared to CT images for spatial accuracy of the QSM. The applied post-processing algorithm was found to be insensitive to the seeds' type; also, presence of nearby calcifications had no effect on seed visualization. QSM successfully generated positive contrast for both types of investigated fiducial markers with high spatial accuracy compared to CT. Interstitial needles containing both aluminum-based CT-maker and titanium-based obturators were accurately depicted on the QSM. The proposed QSM-based technique relies on the standard MR pulse sequences and visualize the conventional MR-invisible metallic devices with CT-like positive contrast solely through post-processing. Upon in vivo validation of the technique, QSM may have the potential to replace CT for an MR-only guided radiation therapy.

Recently, susceptibility based positive contrast MRI technique emerged as an effective method of visualizing the small MR compatible devices, such as brachytherapy seeds. One of the challenges associated with this method is the long scan time. In this work, we present an accelerated susceptibility based positive contrast MR imaging method, in which the susceptibility map can be generated from an under-sampled data. We use a combination of parallel imaging (GRAPPA) and compressive sensing (CS) technique in a hybrid k-space. The results in brachytherapy seeds imaging show that 3-D high-quality images can be achieved at an acceleration factor up to 4, which can decrease the scan time from 4 minutes to 1.4 minutes.

Parallel compressive sensing in a hybrid space: Application in interventional MRI

This study aims to develop an accelerated susceptibility-based positive contrast MR imaging method for visualizing MR compatible metallic devices. A modified fast spin echo sequence is used to accelerate data acquisition. Each readout gradient in the modified fast spin echo is slightly shifted by a short distance T shift. Phase changes accumulated within T shift are then used to calculate the susceptibility map by using a kernel deconvolution algorithm with a regularized l1 minimization. To evaluate the proposed fast spin echo method, three phantom experiments were conducted and compared to a spin echo based technique and the gold standard CT for visualizing biopsy needles and brachytherapy seeds. Compared to the spin echo based technique, the data sampling speed of the proposed method was faster by 2–4 times while still being able to accurately visualize and identify the location of the biopsy needle and brachytherapy seeds. These results were confirmed by CT images of the same devices. Results also demonstrated that the proposed fast spin echo method can achieve good visualization of the brachytherapy seeds in positive contrast and in different orientations. It is also capable of correctly differentiating brachytherapy seeds from other similar structures on conventional magnitude images.

Accelerated susceptibility-based positive contrast imaging of MR compatible metallic devices based on modified fast spin echo sequences.

The invention relates to a positive-contrast magnetic resonance imaging method and device. The positive-contrast magnetic resonance imaging method comprises the steps that two times of data acquisition are conducted on the same level of the same object, and TSE data with echo reading gradient offsets and TSE data without echo reading gradient offsets are respectively obtained; on the same level of the same object, the phases of the TSE data without the echo reading gradient offsets are subtracted from the phases of the TSE data with the echo reading gradient offsets to obtain phase differences produced by local magnetic field influence; the phase differences are utilized to obtain a local magnetic field produced by a metal device in an object region to surrounding organization; an object local magnetic field expression is represented by convolutions of a polarized nuclear and a magnetic susceptibility matrix, Fourier transformation is conducted on the object local magnetic field expression, and the local magnetic field is utilized to conduct regularization constraint reestablishment on the object local magnetic field expression subjected to the Fourier transformation to obtain magnetic susceptibility matrix of the object and achieve positive-contrast magnetic resonance imaging.
