Acinetobacter are a major concern because of their rapid development of resistance to a wide range of antimicrobials, and rapid profundity in transformation, surviving desiccation and persistinge in the environment for a very long time. The organisms are associated with bacteraemia, pulmonary infections, meningitis, diarrhea and notorious nosocomial infections with mortality rates of 20 to 60%. Transmission is via person-to-person contact, water and food contamination, and contaminated hospital equipment. The increasing virulence and rapid development of multidrug resistance by these organisms highlights the need to search for alternatives for chemotherapy. A poor understanding of the organisms and dearth of information about their occurrence especially in developing countries informed the need for this review paper.

The Ecology, Biology and Pathogenesis of Acinetobacter spp.: An Overview

The Candida parapsilosis family has emerged as a major opportunistic and nosocomial pathogen. It causes multifaceted pathology in immuno-compromised and normal hosts, notably low birth weight neonates. Its emergence may relate to an ability to colonize the skin, proliferate in glucose-containing solutions, and adhere to plastic. When clusters appear, determination of genetic relatedness among strains and identification of a common source are important. Its virulence appears associated with a capacity to produce biofilm and production of phospholipase and aspartyl protease. Further investigations of the host-pathogen interactions are needed. This review summarizes basic science, clinical and experimental information about C. parapsilosis.

None of 50 healthy individuals and only 2 of 50 hospitalized patients (with Acinetobacter anitratus elsewhere) yielded A. anitratus on enrichment from fecal samples.

Fifteen out of 24 risk neonates from the premature baby ward (= 63%) and 36 of 79 healthy neonates from the infant nursery (= 46%) at a Zurich hospital were infected within a period of 29 days during a nosocomial epidemic human rotavirus (HRV) outbreak. Forty-two out of 51 of all HRV-infected babies (=82%) manifested clinical symptoms, but these could not all be attributed to an HRV infection. On average, rotavirus could be detected in infected risk neonates five days after their admission to the premature baby ward. Eleven of the 15 risk neonates were infected within 10 days after the admission of the index case. Immediately after detection of the epidemic process, specific detailed countermeasures were instituted, including hand protection and hand disinfection, individual nursing set and cohorting of already infected babies. Early institution of such measures and information of all persons responsible for treatment and care of the babies concerned are crucial for success.

Nosocomial rotavirus infections in neonates: means of prevention and control.

Recurrent contamination of bacteriological specimens with Candida parapsilosis led to epidemiological investigations which indicated persistent colonization of carbon dioxide incubators as the most likely source. Changes in the technical arrangements and institution of a meticulous cleansing protocol eliminated contamination of specimens but not colonization of the incubators. Tests for tolerance of 17% NaCl and survival at 50 degrees C, and SDS-PAGE analysis of crude cell extracts allowed discrimination between epidemic and non-epidemic isolates, while enzyme profile analysis and susceptibility studies failed as typing methods.

During a nine-month period in 1986, we observed five ornithine decarboxylase-negative isolates of Serratia marcescens from four different patients. All isolates were identical in more than 50 biochemical parameters. Four isolates from three hospitalized patients were essentially identical in their susceptibility patterns to 12 antimicrobial agents. Analysis of 35-S-methionine labeled whole cell proteins by SDS-PAGE with the AMBIS system (Automated Microbiology System Inc., San Diego, CA) suggested that only three of these isolates were identical while the fourth hospital strain was more closely related to the isolate of a patient with no contact to the hospital. These results were confirmed by bacteriocin- and serotyping. We conclude that analysis of these protein patterns - which does not require special reagents - was an adequate method for typing S. marcescens .
