What can we learn from the Baduanjin rehabilitation as COVID‐19 treatment?: A narrative review

This article reviews various aspects of COVID-19, including its current status, its side-effects, emergency protective measures and strategies from lifestyle to traditional Chinese medicine (TCM) for fighting against the SARS-CoV-2, and its major variants (Delta and Omicron), with the ongoing global COVID-19 pandemic, which include "Carassius auratus lifestyle" for high effective isolation, social and high-tech medical strategies, traditional Chinese herbs "Bark-Flower-Fruit-Grass-Leaf-Nucleolus(seed)-Root (BFFGLNR)", and the combination of Chinese and western medicine. As a choice, little is known whether the Chinese acupuncture is an effective method for confirming and suspecting COVID-19 patients, which include imported and asymptomatic cases. Definitely, acupuncture has been proven effective treatment for the recovery of COVID-19 cases. However, further animal experiments and clinical trials are required to confirm its effects and disclose underlying mechanisms. In conclusion, these emergency protective measures and strategies for COVID-19 will help to effectively combat the SARS-CoV-2 and its variants during the pandemic and post-COVID-19 era.

Emergency Protective Measures and Strategies of COVID-19: From Lifestyle to Traditional Chinese Medicine

Objective: This study aimed to systematically review and depict the current studies of traditional Chinese medicine for the mental health of patients with coronavirus disease 2019 (COVID-19). Methods: A scoping review was conducted by searching PubMed, Web of Science, CNKI, Wanfang database, VIP database, and SinoMed, with the retrieval time being from the establishment of the database to April 18, 2022. The basic information of the included studies, objective, design, types of patients, interventions, outcomes, etc., was reviewed and summarized narratively. Methodological quality was assessed using the Cochrane Risk of Bias assessment tool, the methodological index for non-randomized studies or the Newcastle–Ottawa scale. Results: We identified 30 traditional Chinese medicine (TCM) studies from six databases. Among them, finished randomized controlled trials (n = 16) accounted for most of the studies, followed by single-arm studies (n = 9). In terms of study theme, 20 studies defined the mental health of patients with COVID-19 as the research theme. Psychological assessment was included in the inclusion criteria (performed before participation) of nine studies, whereas the other studies only mentioned the mental outcomes. TCM interventions included TCM exercises (Yijinjing, Baduanjin, Liuzijue, Taichi), acupoint stimulation (auricular and body points), moxibustion, decoction, or granules based on TCM syndrome differentiation, decoction, or granules with fixed formulae (Baidu Jieduan granules, Xuanfei Baidu decoction, and Qingfei Paidu decoction), Chinese patent medicine (Jinhua Qinggan granules), TCM psychological therapy (TCM ideological therapy, TCM five-tone therapy, and TCM psychological sand table), and TCM nursing (dialectical care, dialectical diet, and psychological counseling). Anxiety and depression were the main outcomes evaluated in regard to mental health in patients with COVID-19. The limitations of methodological quality were predominantly from follow-up, blinding, and registration. Positive results were reported by 27 studies (90%, n = 30). Conclusion: We summarized the existing literature about the impact of TCM on mental health in patients with COVID-19. The number of studies evaluating the impact of TCM on mental health is encouraging, but overall methodological quality was low. Several TCM interventions warrant further evaluation, particularly among populations outside of China, for the purpose of establishing supporting evidence. Effects of Baduanjin on Sleep, Anxiety and Depression of Medical Staff Aid to Shanghai during Omicron Investigate

Dear editor We read with great interest a recently published study by Lisa Taruff et al in Neuropsychiatric Disease and Treatment. This study found that for patients who developed a previous mild form of COVID-19, the majority of them would present persistent neurological and neuropsychological symptoms. Specifically, various neuropsychological disorders have been frequently reported in patients with long COVID, such as anxiety, depression, and sleep disorders. Thus, this paper discusses the potential value of acupuncture and moxibustion as adjunctive therapy for alleviating neuropsychological disorders associated with long COVID in the post-epidemic era. As an important component of traditional Chinese medicine (TCM), acupuncture and moxibustion have been frequently used as complementary treatments for COVID-19 and related complications. To note, several published TCM guidelines and consensuses for COVID-19 in the past three years have specifically focused on the adjunctive use of acupuncture for COVID-19 treatment. Moreover, a recently published scoping review demonstrated that evidence from case reports, clinical trials, and randomized controlled trials (RCTs) supporting the use of acupuncture and moxibustion in COVID-19-associated neuropsychological disorders is growing. For instance, RCTs have shown that, compared with controls, acupuncture and moxibustion can significantly improve anxiety and depressive symptoms in long-COVID patients. In a RCT by Yang et al, auricular acupuncture combined with Baduanjin was found to have a better effect on improving sleep quality, anxiety, and depression conditions in COVID-19 patients with insomnia than pharmacotherapy (ie, oral administration of estazolam). Another encouraging study 4 revealed that adopting a mobile internet-based moxibustion technique for COVID-19 treatment is feasible. Through a mobile internet platform, patients were instructed to perform self-administered moxibustion for COVID-19-related symptoms at home. This internet-based mode of selfadministered moxibustion can not only relieve respiratory symptoms such as cough and fatigue and improve neuropsychological state but also potentially protect front-line medical professionals against COVID-19. Additionally, several ongoing systematic reviews and meta-analyses are investigating the efficacy and safety of acupuncture and/or moxibustion for treating COVID-19-related neuropsychological disorders, and their results are eagerly anticipated. Apart from the aforementioned clinical trials, experimental studies are emerging to explore the possible mechanisms underlying acupuncture and moxibustion for alleviating COVID-19-associated symptoms. Notably, a study based on bioinformatics and topology systematically revealed the multi-target mechanisms of acupuncture therapy for COVID-19. In this study, 180 protein targets and two active compounds produced were identified, in which the results suggested that the effect of acupuncture for COVID-19 was associated with suppression of inflammatory stress, improvement of immunity, and regulation of nervous system function, including activation of neuroactive ligand–receptor interaction, calcium signaling pathway, cancer pathway, viral carcinogenesis, and Staphylococcus aureus infection.

Neurological manifestations of long COVID: a single-center one-year experience [Letter]

OBJECTIVE: To compare the clinical therapeutic effect on coronavirus disease 2019 (COVID-19) with insomnia between the combined treatment of baduanjin and auricular point sticking therapy and the medication with oral estazolam on the base of the conventional treatment METHODS: A total of 90 patients with COVID-19 accompanied with insomnia were randomly divided into an observation group (45 cases, 3 cases dropped off) and a control group (45 cases) In the observation group, baduanjin, a traditional Chinese fitness activity, was practiced everyday Besides, auricular point sticking therapy was exerted at ear-shenmen (TF 4), subcortex (AT 4), heart (CO 15), occiput (AT 3), etc These auricular points were pressed and kneaded three times a day, 30 s at each point each time, consecutively for 12 days In the control group, estazolam tablets were prescribed for oral administration, 1 mg, once daily, consecutively for 12 days Before and after treatment, the score of Pittsburgh sleep quality index (PSQI), the score of self-rating anxiety scale (SAS), the score of self-rating depression scale (SDS) and the score of symptoms in traditional Chinese medicine (TCM) were observed in the two groups and the clinical therapeutic effect was evaluated RESULTS: After treatment, the scores of every item and the total scores in PSQI were all reduced as compared with those before treatment in the two groups (P 005) CONCLUSION: The combined treatment of baduanjin and auricular point sticking therapy improves sleep quality, the conditions of anxiety and depression and the symptoms in TCM for patients of COVID-19 with insomnia The therapeutic effect of this combined treatment is better than the oral administration of estazolam
