OBJECTIVES: To evaluate critically the evidence linking psychotropic drugs with falls in older people.

DESIGN: Fixed-effects meta-analysis.

DATA SOURCES: English-language articles in MEDLINE (1966 – March 1996) indexed under accidents or accidental falls and aged or age factors; bibliographies of retrieved papers.

STUDY SELECTION: Systematic evaluation of sedative/hypnotic, antidepressant, or neuroleptic use with falling in people aged 60 and older.

DATA EXTRACTION: Study design, inclusion and exclusion criteria, setting, sample size, response rate, mean age, method of medication verification and fall assessment, fall definition, and the number of fallers and non-fallers taking specific classes of psychotropic drugs.

RESULTS: Forty studies, none randomized controlled trials, met eligibility criteria. For one or more falls, the pooled odds ratio (95% confidence interval) was 1.73 (95%CI, 1.52-1.97) for any psychotropic use; 1.50 (95%CI, 1.25-1.79) for neuroleptic use; 1.54 (95%CI, 1.40-1.70) for sedative/hypnotic use; 1.66 (95%CI, 1.4-1.95) for any antidepressant use (mainly TCAs); 1.51 (95%CI, 1.14-2.00) for only TCA use; and 1.48 (95%CI, 1.23-1.77) for benzodiazepine use, with no difference between short and long acting benzodiazepines. For neuroleptics in psychiatric inpatients, the pooled OR was 0.41 (95%CI, 0.21-.82); for all other patients, the pooled OR was 1.66 (95%CI, 1.38-2.00). Comparing ≥1 with ≥ 2 falls, mean subject age >75 versus ≥ 75 years old, communities with >35% versus ≥35% fallers, or subject place of residence did not affect the pooled OR. Increased falls occurred in patients taking more than one psychotropic drug.

CONCLUSION: There is a small, but consistent, association between the use of most classes of psychotropic drugs and falls. The evidence to date, however, is based solely on observational data, with minimal adjustment for confounders, dosage, or duration of therapy. The incidence of falls and their consequences in this population necessitate that future large randomized controlled trials of any medication in older persons should measure falls prospectively as an adverse outcome event.

Drugs and Falls in Older People: A Systematic Review and Meta‐analysis: I. Psychotropic Drugs

An important goal of the scientific community is broadening the achievement and participation of racial minorities in STEM fields. Yet, professors’ beliefs about the fixedness of ability may be an unwitting and overlooked barrier for stigmatized students. Results from a longitudinal university-wide sample (150 STEM professors and more than 15,000 students) revealed that the racial achievement gaps in courses taught by more fixed mindset faculty were twice as large as the achievement gaps in courses taught by more growth mindset faculty. Course evaluations revealed that students were demotivated and had more negative experiences in classes taught by fixed (versus growth) mindset faculty. Faculty mindset beliefs predicted student achievement and motivation above and beyond any other faculty characteristic, including their gender, race/ethnicity, age, teaching experience, or tenure status. These findings suggest that faculty mindset beliefs have important implications for the classroom experiences and achievement of underrepresented minority students in STEM.

Joan Williams' Unbending Gender: Why Family and Work Conflict And What To Do About It (Oxford, 1999) is a "theoretically sophisticated and thoroughly accessible treatise" that offers a new vision of work, family, and gender. (Publisher's Weekly, Nov. 1, 1999) It examines our system of providing for children's care by placing their caregivers at the margins of economic life. This system that stems from the way we define our work ideals, notably from our definition of the ideal worker as one who takes no time off for childbearing or childrearing and who works full-time and is available for overtime. The ideal-worker norm clashes with our sense that children should be cared for by parents. The result is a system that is bad for men, worse for women, and disastrous for children. Williams documents that mothers remain economically marginalized, and points out that when mothers first marginalize and then divorce, their children often accompany them into poverty. Williams argues that designing workplaces around the bodies of men (who need no time off for childbearing) and men's life patterns (for women still do 80% of the child care) often constitutes discrimination against women. She also engages the work/family literature to show that "flexible" workplaces are often better than existing practices for employers' bottom line. On the family side, she argues that the ideal worker's wage -- after as well as before divorce -- reflects the joint work of the ideal worker and the primary caregiver of his children, and should be jointly owned. In a comprehensive examination of the theoretical issues surrounding work/family issues, she uses the work of Judith Butler and Pierre Bourdieu to explain why gender has proved so unchanging and unbending, reframing the special treatment/equal treatment debate, the debate over "women's voice," and offering new perspective on how to avoid the persistent race and class conflicts that emerge in debates over work and family issues.

Unbending Gender: Why Family and Work Conflict and What To Do About It (Panel Two: Who's Minding the Baby?)

A life stage perspective is necessary for development of age-appropriate strategies to address substance use disorders (SUDs) and related health conditions in order to produce better overall health and well-being. The current review evaluated the literature across three major life stages: adolescence, adulthood, and older adulthood. Findings: 1) Substance use is often initiated in adolescence, but it is during adulthood that prevalence rates for SUDs peak; and while substance involvement is less common among older adults, the risk for health complications associated with use increases. 2) Alcohol, tobacco, marijuana, and, increasingly, prescription medications, are the most commonly misused substances across age groups; however, the use pattern of these and other drugs and the salient impact vary depending on life stage. 3) In terms of health outcomes, all ages are at risk for overdose, accidental injury, and attempted suicide. Adolescents are more likely to be in vehicular accidents while older adults are at greater risk for damaging falls. Adulthood has the highest rates of associated medical conditions (e.g., cancer, sexually transmitted disease, heart disease) and mental health conditions (e.g., bipolar disorder, anxiety disorders, antisocial personality disorder). Conclusion: Prolonged heavy use of drugs and/or alcohol results in an array of serious health conditions. Addressing SUDs from a life stage perspective with assessment and treatment approaches incorporating co-occurring disorders are necessary to successfully impact overall health.

Substance Use and Associated Health Conditions throughout the Lifespan.

The purposes of this study were to determine: (1) the prevalence of psychoactive medication and alcohol use and (2) the relationship among psychoactive medications, alcohol use, and falls in a sample of 1028 independently living women and men, aged 55 and older. Twenty-six percent of the sample reported falling, 28% were taking one or more psychoactive drugs, and 38% drank alcohol during the past year. Analyses with logistic regression indicate that predictors of falls were psychoactive drug use, age, and number of illnesses. Living alone, frequency of alcohol use, and gender were not significant predictors.

Although there has been a welcome increase in discussion about gender disparities in science, technology, engineering, and mathematics (STEM), broad participation of women from all backgrounds in academic STEM will not be achieved until institutions are transformed. A long-range view is needed to change the rules of the game, such that institutional culture and practices create workplaces where all scientists and engineers want to be. We lay out a six-point plan of what needs to change, who should participate, and how actors outside of the academy should have direct involvement in the process.

Exploring Gender and Economic Development in Appalachia.

Purpose
To implement and assess an intervention designed to promote gender equity and organizational change within STEM departments in two Colleges at a single Research High university. Department climate impacts the retention and success of women faculty.


Methodology/approach
A survey was administered both before and after the department intervention in order to capture departmental change on variables that measure a positive climate for female faculty.


Findings
Across all of the science and engineering departments, levels of Collective Efficacy toward Gender Equity significantly increased while levels of Conflict significantly decreased after the department facilitation. In the science departments, the level of Vicarious Experience of Gender Equity among faculty significantly increased while in the engineering departments levels of faculty Dependence significantly decreased. There was a statistically significant decrease in Optimism about Gender Equity among the science faculty.


Practical implications
Organizational change within universities has been documented as slow and labor intensive. Departmental climate, particularly interactions with colleagues, remains an area wherein women continue to feel excluded. The departmental intervention resulted in measurable improvements in key aspects of climate critical to women’s success (e.g., reductions in conflict and dependence; increases in collective efficacy) as well as more realistic view of the effort needed to attain gender equity (decrease in Optimism).

Organizational Change and Gender Equity in Academia: Using Dialogical Change to Promote Positive Departmental Climates

One university is addressing the gender gap in leadership with an internal model designed to support and inspire groups of women in leadership positions without leaving campus. Based on a coaching method used at Harvard’s Women’s Leadership Forum, our leadership initiative provides an on-campus leadership development and support program for women. This paper assesses the overall impact and effectiveness of this program and compares results of an external coaching model versus an internal facilitator (coach) model. This “grow your own” program is a low cost, effective model for leadership development that could be replicated by other institutions.
