As a traditionally underserved population that faces numerous health disparities, youth who identify as transgender and gender diverse (TGD) and their families are increasingly presenting to pediatric providers for education, care, and referrals. The need for more formal training, standardized treatment, and research on safety and medical outcomes often leaves providers feeling ill equipped to support and care for patients that identify as TGD and families. In this policy statement, we review relevant concepts and challenges and provide suggestions for pediatric providers that are focused on promoting the health and positive development of youth that identify as TGD while eliminating discrimination and stigma.

Ensuring Comprehensive Care and Support for Transgender and Gender-Diverse Children and Adolescents

Background A growing body of research continues to elucidate health inequities experienced by transgender individuals and further underscores the need for medical providers to be appropriately trained to deliver care to this population. Medical education in transgender health can empower physicians to identify and change the systemic barriers to care that cause transgender health inequities as well as improve knowledge about transgender-specific care. Methods We conducted structured searches of five databases to identify literature related to medical education and transgender health. Of the 1272 papers reviewed, 119 papers were deemed relevant to predefined criteria, medical education, and transgender health topics. Citation tracking was conducted on the 119 papers using Scopus to identify an additional 12 relevant citations (a total of 131 papers). Searches were completed on October 15, 2017 and updated on December 11, 2017. Results Transgender health has yet to gain widespread curricular exposure, but efforts toward incorporating transgender health into both undergraduate and graduate medical educations are nascent. There is no consensus on the exact educational interventions that should be used to address transgender health. Barriers to increased transgender health exposure include limited curricular time, lack of topic-specific competency among faculty, and underwhelming institutional support. All published interventions proved effective in improving attitudes, knowledge, and/or skills necessary to achieve clinical competency with transgender patients. Conclusion Transgender populations experience health inequities in part due to the exclusion of transgender-specific health needs from medical school and residency curricula. Currently, transgender medical education is largely composed of one-time attitude and awareness-based interventions that show significant short-term improvements but suffer methodologically. Consensus in the existing literature supports educational efforts to shift toward pedagogical interventions that are longitudinally integrated and clinical skills based, and we include a series of recommendations to affirm and guide such an undertaking.

Transgender health care: improving medical students' and residents' training and awareness.

New analysis of responses to the National Transgender Discrimination Survey (NTDS) shows that transgender respondents who experienced rejection by family and friends, discrimination, victimization, or violence have a higher risk of attempting suicide. 78 percent of survey respondents who suffered physical or sexual violence at school reported suicide attempts, as did 65 percent of respondents who experienced violence at work. The study suggests that several minority stressors – negative experiences related to anti-transgender bias – may contribute to elevated prevalence of suicide attempts among transgender people, such as experiences of harassment, family rejection, housing instability, and discrimination in health care. Over half of those who experienced harassment or bullying in schools reported lifetime suicide attempts, as did 57 percent of those who reported that their family chose not to speak/spend time with them. High prevalence of suicide attempts was also found among those who had ever experienced homelessness (69%) and those who reported a doctor or healthcare provider refused to treat them (60%). The study utilized data collected through the National Transgender Discrimination Survey (NTDS), which was conducted by the National Center for Transgender Equality and the National Gay and Lesbian Task Force. 6,456 transgender and gender non-conforming people in the United States reported on their experiences of discrimination and abuse at work, at home, in school, and in the public sphere, amassing the largest transgender survey sample to date.

Despite recent notable advances in societal equality for lesbian, gay, bisexual, and transgender (LGBT) individuals, youth who identify as trans* or gender nonconforming, in particular, continue to experience significant challenges accessing the services they need to grow into healthy adults. This policy perspective first offers background information describing this population, their unique healthcare needs, and obstacles when seeking care, including case study examples. The authors then provide recommendations for medical education, health systems, and insurance payers, as well as recommendations for school systems and broader public policy changes to improve the health and well-being of gender nonconforming youth.

These obstacles can be particularly difficult for youth who identify as trans* or gender non-conforming. This PolicyLab Evidence to Action brief provides information and recommendations for practitioners, administrators, and policymakers to ensure comprehensive care and support and improve health and well-being for gender non-conforming children and adolescents. Although the authors of this brief work within the health care environment, the recommendations are broader since the health care system will not achieve these recommendations on its own. The brief has four sections: The first section - Background - offers definitions and context related to care for gender non-conforming children and adolescents. The second section - Improving the Health Care System - describes the unique health care needs of gender nonconforming youth and the obstacles encountered when seeking care. This section also proposes recommendations for medical education, health systems, and public and private insurance payers to improve health care for gender non-conforming youth. The third section - Going Beyond Health Care - argues that improving the health of gender non-conforming youth will require actions beyond the health care system and proposes recommendations for school systems, as well as broader public policy changes. The last section - Conclusion - provides a final summary and additional resources. Figure 1 below demonstrates how inclusive health care systems, supportive laws and policies, and LGBT-competent youthserving professionals can intersect to ensure comprehensive care and support for gender non-conforming youth.
