Summary International collaboration and understanding is becoming increasingly important as we face a soaring number of emerging and re-emerging infectious diseases. Management of these conditions calls for a cohesive international effort, with contributions from many infectious disease specialists. To optimise collaborative efforts, an international understanding of training, capabilities, and skills would be valuable. An investigation of postgraduate training programmes in the infectious disease specialties around the world was done. 33 countries contributed information. 26 of these countries had established training programmes—one of which was changing its duration and research component; three were in the process of setting up programmes, two provided specialist training that had no official recognition, and two had no specialist training. In addition to promoting international understanding and collaboration, this article should catalyse a global assessment of postgraduate training programmes within the field of infectious diseases.

Background: India is a populous country housing over a billion people Neurology as a specialty is being practiced in India for over 50 years but the number of physicians devoted to fulltime neurology is limited This fact coupled with the privatized healthcare system and limited infrastructure has led to situations different from the more developed healthcare systems Aim: To study the practice patterns of neurology in India Setting and Design: Questionnaire-based study Materials and Methods: Questionnaire was sent to 250 members of the Indian Academy of Neurology [sample size approximately 25%] using random number table The responses were tabulated and analyzed Results and Conclusions: The neurology group is small and hence is exposed to a large workload The average number of patients seen daily by Indian neurologists is three to four times those seen by the United States and United Kingdom neurologists Neurologists based at district places are more likely to see direct patients; whereas metropolitan neurologists see more referrals Investigative facilities are available to neurologists but affordability is a concern Clinical work leaves less time for academic and research activities, which the consultants are keen to participate in In the privatized health system of India, emergency work constitutes a difficult area to cope with The concept of single specialty group practice is welcomed by the majority with the idea of streamlining their work and life These factors highlight an urgent need for increasing the neurology work force and argue for further involvement of primary physicians and internists in neurological care in India

Resume Contexte La specialite de neurologue est devenue de plus en plus complexe et diversifiee et requiert des competences specifiques. La satisfaction des internes francais vis-a-vis de leur formation et le niveau de connaissance et de competence qu’ils estiment avoir acquis n’ont jamais ete evalues. Methodes Nous avons realise une enquete aupres des neurologues en formation sur : (i) les methodes utilisees pour la formation et l’evaluation, (ii) le niveau de connaissance acquis percu par les internes, et (iii) leur satisfaction et leurs souhaits. Resultats Cent huit internes ont repondu. Les principales sources de formation utilisees par les internes sont l’enseignement pratique local (74 %), le travail personnel (61 %) et les journaux scientifiques (57 %) ou didactiques (54 %). Les internes ne semblent pas acquerir l’ensemble des connaissances contenues dans le programme de formation. En effet, les internes jugeaient connaitre bien ou tres bien seulement 14 des 29 domaines du programme d’enseignement de la neurologie. Les domaines les moins bien maitrises etaient la semiologie neurophysiologique, la neuropsychologie, les pathologies du sommeil et comas, l’oncologie et la psychiatrie. Meme en derniere annee, ils ne maitrisent pas bien la realisation des techniques electrophysiologiques et incompletement leur interpretation. Cinquante-huit pour cent des internes sont satisfaits de leur formation, 16 % ne savent pas tres bien quoi penser et 26 % ne sont pas satisfaits. Dans l’ensemble, ils sont favorables a une harmonisation des modalites de formation et de validation de la specialite au niveau national, a un document de travail et de suivi de type portfolio et a l’acces aux consultations. Conclusion Nos resultats suggerent qu’il serait utile de mieux formaliser les competences attendues d’un neurologue et d’utiliser des outils adaptes pour atteindre ces objectifs de formation.
