While health care quality has been improving on average in OECD members countries, patient safety remains a central priority for policy makers and health care leaders. A growing research body has found that PSC is associated with numerous positive outcomes, including improved health outcomes, improved patient experience, and organisational productivity and staff satisfaction. Tools to measure PSC have proliferated in recent decades and are now in wide-spread use. This report includes findings from OECD countries on the state of the art for measurement practices related to PSC. Overall, measurement of PSC is prevalent across OECD countries, though the application, purpose, and tools vary. International learning and benchmarking has significant potential for better understanding and improvement of patient safety and health care quality.

Culture as a cure: Assessments of patient safety culture in OECD countries

Operating room nurses play a critical role in patient safety. The evaluation of safety attitudes of operating room nurses reflects their awareness and belief of patient safety. Currently, however, the research on the safety attitudes of operating room nurses is hard to track in the existing literature in China. Therefore, this paper was conducted to explore the factors influencing the safety attitudes of operating room nurses and their cognition and attitudes toward adverse event reporting. A total of 711 operating room nurses from 16 tertiary hospitals in Sichuan Province from March 1, 2018, to 2019 were selected. The general information of operating room nurses, such as age, gender, and years of service in the operating room, was obtained through the basic information questionnaire. The Chinese version of the Safety Attitudes Questionnaire (C-SAQ) was used to evaluate the safety attitude of operating room nurses, and the cognition and attitude of the subjects to adverse event reports were assessed through the questionnaire of cognition and attitude toward adverse event reporting. The average score of safety attitudes of operating room nurses was 4.20 ± 0.49. The two dimensions with a lower positive reaction rate of the safety attitudes of operating room nurses were stress recognition and working conditions. The main factors affecting the safety attitude of operating room nurses were night shifts, as well as cognition and attitudes toward adverse event reporting. There was a positive correlation between the total score of C-SAQ and the total score of cognition and attitudes toward adverse event reporting (P < 0.01, r = 0.445). The safety attitude of operating room nurses is at the upper-middle level, but the stress recognition and working conditions need to be improved. Through the allocation of nursing human resources, the strengthening of hospital logistics support, and the establishment of nonpunitive nursing adverse event reporting system, the operating room safety can be significantly enhanced.

Contribution to the assessment of patient safety culture in Algerian healthcare settings: The ASCO project

Patient safety has come a long way since the release of the 1999 Institute of Medicine report To Err Is Human. This report revealed the immense size of the problem of preventable adverse events - events that in the past we assumed were "just complications" occurring in the normal course of diagnosis and treatment. Simultaneously, shining the light on patient safety "took the lid off quality." Those of us involved in healthcare provision always had a commitment to providing high-quality care, yet the focus of many key stakeholders on the importance of high-quality healthcare had been limited. The focus tended to be disproportionately on the rising cost of healthcare rather than a balanced focus on quality. Now, we respect the imperative of achieving high-quality healthcare.

Patient Safety: We've Come a Long Way.

As the co-chair of Patients for Patient Safety Canada (PFPSC), I have had the opportunity to be a guest editor for this Special Issue of Healthcare Quarterly and, consequently, have reviewed and critiqued each article. I also was a patient partner in the National Patient Safety Consortium, the work of which is the basis for the articles in this issue. Patient safety is a serious issue in Canada. In fact, unintended harm while receiving healthcare is the third leading cause of death in Canada (RiskAnalytica 2017). The papers in this issue describe initiatives that have the potential to and/or have contributed to reducing patient harm if implemented across our system and in such a way that patients and families are an integral part of the process.

Patient Safety: Patient Involvement Matters

Senior healthcare leaders are the difference makers as key influencers in ushering in an organizational culture committed to patient safety. Although leaders at all levels are champions of transformation, leaders at the "top" have a unique opportunity - and a responsibility - to foster a culture that supports an organization on its journey to zero harm. Through a literature review of more than 60 resources and validation with thought leaders, national and provincial partners have developed a patient safety culture bundle for CEOs and senior healthcare leaders. The bundle is based on a set of evidence-based practices that must be applied collectively to establish and sustain a culture of quality and safety in order to deliver safe care.
